Testing for carotid atherosclerosis.

BACKGROUND: Optometrists frequently examine patients with the signs and symptoms of carotid artery disease. Referral for appropriate testing is mandatory in such patients. METHODS: A review of the noninvasive tests which an optometrist can perform in the office is presented. The paper also describes the noninvasive tests which are performed in vascular clinics on patients in whom carotid atherosclerosis is to be ruled out. Arteriography, both conventional and digital subtraction studies, is also described. RESULTS: A discussion of the appropriate medical and surgical treatment of patients wi

Diagnostic approaches for detecting deep vein thrombosis.

Acute deep vein thrombosis remains a disease for which the diagnosis must be confirmed by an objective testing procedure. The current methods in use include continuous-wave Doppler, impedance plethysmography, and ultrasonic duplex scanning. The Doppler and plethysmographic methods are of value for the detection of proximal venous occlusion but do have limitations below the knee and fail to provide specific information concerning the exact location and extent of involvement. Duplex scanning, which has both imaging and flow detection capabilities, can be used from the level of the inferior vena …

Relationship between diaphragm length and abdominal dimensions.

We examined the relationship between changes in abdominal cross-sectional area, measured by respiratory inductive plethysmography, and changes in length in the costal and crural parts of the diaphragm, measured by sonomicrometry, in nine supine, anesthetized dogs. During passive inflation, both parts of the diaphragm shortened and abdominal cross-sectional area increased. During passive deflation, both parts of the diaphragm lengthened and abdominal cross-sectional area decreased. We subsequently used the relationship between costal and crural diaphragmatic length, respectively, and abdominal …

Hemodynamic assessment of venous problems.

Objective assessment of venous function based on venous refilling time recorded by photoplethysmography (PPG) was done in 89 patients with postphlebitic syndrome (47), primary varicose veins (29), and after an episode of acute deep vein thrombosis (13). Limbs with symptomatic postphlebitic changes had a shortened venous refilling time of 15.6 +/- 11 seconds (normal greater than 20 seconds). When venographic findings (ascending and descending) in 47 patients with postphlebitic syndrome were analyzed, it was noted that femoral valve incompetence demonstrated by descending venography was a common…

Noninvasive neurovascular tests for carotid artery disease.

Indirect noninvasive neurovascular tests provide information on hemodynamic changes cephalad to the carotid bifurcation, whereas direct tests measure anatomic or physiologic changes at the bifurcation itself. Batteries of tests are often done and should combine both indirect and direct methods. Results with two ultrasonic techniques and digital subtraction intravenous angiography suggest a larger role for these methods. Carotid arteriography is the definitive procedure for evaluating the carotid artery, although it should only be done when carotid endarterectomy is contemplated. The relative r

Impedance cardiographic assessment of symptomatic patent ductus arteriosus.

A modified impedence cardiographic technique was developed using a tetrapolar apnea monitor and minicomputer system. Evaluation of this technique in premature infants with and without symptomatic patent ductus arteriosus indicates that the magnitude of the cardiac-related deflection in the impedance signal is useful in the assessment of ductus shunting. Since the infant under study is involved only by the attachment of two double electrodes to the thorax, continuous assessment of ductur shunting is possible without disturbing routine care.

Hypothenar hammer syndrome.

The diagnosis of hypothenar hammer syndrome (HHS) should be considered in the case of hand ischemia in people who occupationally or recreationally use the hypothenar region literally as a “hammer”. Routine diagnostics should consist of physical examination including Allen’s test, acral plethysmography and duplex sonography. According to the prevailing opinion angiography remains the «gold standard test» for establishing the diagnosis of HHS. Early diagnosis allows more effective therapeutic strategies and is important to prevent long-term negative medical sequelae. Several basic principles app…

[Raynaud syndrome complicated by digital gangrene during treatment with interferon-alpha].

In a 43-year old male suffering from idiopathic hypereosinophilic syndrome (HES) since 1984, successfully treated with alpha interferon (alpha IFN) for 32 months, a severe Raynaud's phenomenon of the four extremities occurred and eventually evolved into digital necrosis within a few weeks. The arterial echography/doppler and plethysmography patterns were suggestive of isolated small-to medium-size digital artery occlusions. An extensive search for an aetiology of digital necrosis, including complete tests of autoimmunity, remained negative. Two months later, despite alpha IFN withdrawal and in…

Cardiovascular effects of apnoea in preterm infants.

Cardiovascular reflex responses have been studied in 9 newborn preterm infants with apnoeic episodes and in 2 preterm infants with periodic breathing. Respiration, blood pressure, heart rate, and peripheral blood flow were simultaneously recorded. Peripheral blood flow was measured in the leg by venous occlusion plethysmography. During apnoea, bradycardia and peripheral vasoconstriction occur. There is little change in blood pressure though pulse pressure increases. No cardiovascular changes were seen before the onset of apnoea. Periodic breathing had little effect on peripheral blood flow. Pr…

[Clinical and functional assessment in COPD: from case finding to follow-up].

A spirometry with bronchodilator test is needed for the diagnosis of COPD. It is recommended to detect COPD only in subjects with symptoms ( … nce of incompletely reversible airflow obstruction, assess severity (using clinical questionnaires, plethysmography, exercise testing and arterial blood gases when indicated) and define future management. The level of FEV1 is associated with individualized assessment of symptoms and evaluation of exacerbation risk in the management strategy of stable COPD.

The hemodynamic effect of venous hypertension in the microcirculation of the lower limb.

Air-plethysmography has been used in order to measure resting arterial inflow in 25 normal limbs and 126 limbs with various grades of venous disease. Twenty five limbs had venous ulcers. Resting arterial inflow was 60-95 ml/min in normal limbs (95% range), 60-110 ml/min in 45 limbs with uncomplicated varicose veins, 65-162 ml/min in limbs with swelling and/or lipodermatosclerosis/pigmentation and 100-235 ml/min in limbs with venous ulceration. There was an increase in the incidence of ulceration with increasing values of arterial inflow. The abnormal increase in resting arterial inflow is cons…
