The role of vestibular cues in postural sway.

Controlling posture requires continuous sensory feedback about body motion and orientation, including from the vestibular organs. Little is known about the role of tilt vs. translation vs. rotation vestibular cues. We examined whether intersubject differences in vestibular function were correlated with intersubject differences in postural control. Vestibular function was assayed using vestibular direction-recognition perceptual thresholds, which determine the smallest motion that can be reliably perceived by a subject seated on a motorized platform in the dark. In study A, we measured threshol…

Subtypes of Persistent Postural-Perceptual Dizziness.

Background: Persistent postural-perceptual dizziness (PPPD) is a persistent chronic vestibular syndrome exacerbated by upright posture/walking, active or passive motion, and exposure to moving or complex visual stimuli. PPPD has four precursors: phobic postural vertigo, space-motion discomfort, visual vertigo, and chronic subjective dizziness. These four diseases share clinical features that form the basis of the diagnostic criteria for PPPD. Semiological similarities do not necessarily mean that PPPD is a single entity. However, if PPPD is not a single disorder but just a composite of four pr…

Postural nausea due to platypnea orthodeoxia.

Platypnea orthodeoxia is an uncommon condition but given its associations with severe lung disease and advancing age, it might be seen in the palliative care population. Presentation is classical in the association of symptoms with postural change. It should be considered when patients present with breathlessness or desaturation given the minimally invasive tests of echocardiography, and the potential for procedural resolution of the defect and thus symptoms. Further, there has not previously been reported a case presenting with nausea (rather than dyspnoea) secondary to platypnea orthodeoxia

Intravascular lymphoma presenting with postural hypotension.

An 84-year-old woman presented with severe postural hypotension. Further assessment revealed weight loss, fatigue and fever at night. On examination, she had bilateral skin lesions on the inner thighs and skin biopsy revealed intravascular high grade B cell lymphoma. This was successfully treated with curative chemotherapy. The cause of the postural hypotension in this case was felt likely to be autonomic neuropathy caused by neurovascular infiltration by intravascular lymphoma. Treatment of the lymphoma has resolved the postural hypotension, although some symptoms of postural instability pers

Evaluation of postural tachycardia syndrome (POTS).

The diagnostic evaluation of a patient with suspected postural tachycardia syndrome (POTS) requires a thoughtful diagnostic approach utilizing a careful clinical history and examination, laboratory, and autonomic testing. This article outlines the importance of a thorough history in identifying mechanism of symptom onset, clinical features, associated clinical conditions or disorders, and factors that may result in symptom exacerbation. The clinical examination involves an assessment of pupillary responses, an evaluation for sudomotor and vasomotor signs, and an assessment for joint hypermobil

The effects of brief swaying on postural control.

Postural control can be improved with balance training. However, the nature and duration of the training required to enhance posture remains unclear. We studied the effects of 5 min of a self-initiated balance exercise along a single axis on postural control in healthy individuals. Postural control was measured before and after a 5-min period where members of the experimental group were asked to lean their entire body forward and backward and members of the control group were asked to remain seated. A significant improvement for sway velocity, a postural control variable significantly associat…

Birthing postures and birth canal lacerations.

OBJECTIVE: This study was performed to assess the differences in the birth canal lacerations following the lateral and fours posture deliveries compared with those following the supine posture deliveries. METHODS: We examined the birth canal lacerations of our "low risk" pregnant women under the midwife-led delivery care at Japanese Red Cross Katsushika Maternity Hospital between April 2006 and March 2015. RESULTS: There were 3826, 1754 and 719 women who delivered with supine, lateral and fours postures. The rate of no laceration in the women who delivered with lateral posture was significan…

Cognitive dysfunction in postural tachycardia syndrome.

Mental clouding is an almost universal complaint among patients with postural tachycardia syndrome (POTS) but remains poorly understood. Thus, we have determined whether POTS patients exhibit deficits during neuropsychological testing relative to healthy subjects. A comprehensive battery of validated neuropsychological tests was administered to 28 female POTS patients and 24 healthy subjects in a semi-recumbent position. Healthy subjects were matched to POTS patients on age and gender. Selective attention, a primary outcome measure, and cognitive processing speed were reduced in POTS patients

Reversible postural orthostatic tachycardia syndrome.

Postural orthostatic tachycardia syndrome (POTS) is a relatively rare syndrome recognised since 1940. It is a heterogenous condition with orthostatic intolerance due to dysautonomia and is characterised by rise in heart rate above 30 bpm from base line or to more than 120 bpm within 5-10 min of standing with or without change in blood pressure which returns to base line on resuming supine position. This condition present with various disabling symptoms such as light headedness, near syncope, fatigue, nausea, vomiting, tremor, palpitations and mental clouding, etc. However there are no identifi

Visceral sensitization in postural tachycardia syndrome.

OBJECTIVE: Both psychogenicity and organicity have been asserted in postural tachycardia syndrome (PoTS). We studied the genesis of palpitations to dissect the biologic nature of PoTS. METHODS: Eleven PoTS patients and 10 control subjects were asked to discriminate types of palpitations when supine and in response to two sympathetic stimuli [Valsalva maneuver (VM) and 10-min head-up tilt (HUT)] and one vagolytic stimulus (atropine administration). Participants rated the 10 items of the somatosensory amplification scale to assess symptom exaggeration. Their time-dependent heartbeat counts were

Integrated postural analysis in children with haemophilia.

The maintenance of a correct posture in haemophilic boys might contribute to prevent joint bleeds, chronic pain and dysfunction. This single-centre study was aimed at evaluating whether or not postural alterations are more common in haemophilic than in non-haemophilic boys and whether they are related to the orthopaedic status. Posture and balance were investigated in boys with severe/moderate haemophilia (cases) and in age-matched non-haemophilic peers (controls). Thirty-five cases (89% with haemophilia A: 74% with severe disease) were included in the study and compared with 57 controls. Post…

Impaired postural balance in turner syndrome.

An impaired body balance has been found in Turner syndrome (TS) in clinical tests like Rombergs's test and walking on a balance beam. The aim of the study was to assess postural balance in TS subjects with specific balance testing using dynamic posturography and relate to body composition. Nineteen TS subjects (20-57 years) were included. Balance was measured with dynamic posturography (Equitest) and compared with 19 sex and age-matched controls (22-59 years). Equitest, visual, vestibular, and somatosensory systems were provoked with increasing difficulty (6 tests, SO1-SO6) and body sway was m

Postural and locomotor contributions to affordance perception.

The authors sought to evaluate the relative importance of locomotor control and postural control in the perception of affordances. While seated in a stationary wheelchair, participants made a series of judgments about the minimum lintel height under which they could roll in the wheelchair. Prior to making judgments, participants were given brief (∼2 min) experience with wheelchair locomotion. They expected that this practice would influence the accuracy of subsequent affordance judgments. During practice, participants moved under their own power (using their hands on the wheels) or with an exp

Effects of dyslexia on postural control in adults.

Dyslexia has been shown to affect postural control. The aim of the present study was to investigate the difference in postural stability measured as torque variance in an adult dyslexic group (n=14, determined using the Adult Dyslexia Checklist (ADCL) and nonsense word repetition test) and an adult non-dyslexic group (n=39) on a firm surface and on a foam block and with eyes open and eyes closed. Another aim was to investigate the correlation between ADCL scores and postural stability. Findings showed that ADCL scores correlated with torque variance in the anteroposterior direction on foam wit

Underdevelopment of the postural control system in autism.

OBJECTIVE: To determine if abnormalities exist in postural control in autism and if they are related to age. METHODS: Dynamic posturography was performed in 79 autistic individuals without mental retardation and 61 healthy volunteers between ages 5 and 52 years. Both the sensory organization and the movement coordination portions of the test were performed. RESULTS: The autistic subjects had reduced postural stability (p = 0.002). Examination of age effects revealed that the development of postural stability was delayed in the autistic subjects (p < 0.001) and failed to achieve adult levels

Postural control as assessed with virtual reality.

We studied the use of virtual reality technology as a stimulus in balance examinations. A pilot study was made using a small group of healthy subjects to investigate the effect of alcohol and virtual reality stimulus on the subjects' balance. The tests showed that blood alcohol concentration accounted for almost 50% of the increased lateral body sway velocity. The new stimulus technique based on virtual reality technology seems to be effective and flexible for postural investigations.

Imitation and matching of hand and finger postures.

Converging evidence from patients with unilateral brain lesions, from a patient with callosal disconnection, and from functional imaging in healthy subjects suggests different competencies of both hemispheres for imitation and matching of hand and finger postures. Whereas the left hemisphere is fully competent for processing hand postures, an additional right hemisphere contribution is needed for finger postures. I propose that the left hemisphere is responsible for coding gestures with reference to knowledge about the structure of the human body, whereas a right hemisphere contribution is nee…

The neural representation of postural control in humans.

Lesion of the "vestibular cortex" in the human posterior insula leads to a tilted perception of visual vertical but not to tilted body posture and loss of lateral balance. However, some stroke patients show the reverse pattern. Although their processing of visual and vestibular inputs for orientation perception of the visual world is undisturbed, they push away actively from the ipsilesional side (the side of lesion location), leading to a contraversive tilt of the body (tilt toward the side opposite to the lesion) and falling to that side. Recently, the origin of contraversive pushing was ide…

[Idiopathic postural orthostatic tachycardia syndrome].

Stimulated by the widespread use of head-up tilt testing, transient episodes of neurocardiogenically mediated hypotension and bradycardia have become a well recognized cause of recurrent syncope and near syncope (generally referred to as neurally mediated syncope). On the other hand, a large subgroup of patients was identified, who appeared to have a less severe hypotension and orthostatic intolerance that is characterized by postural tachycardia, exercise intolerance, disabling fatigue, lightheadedness and dizziness. This form of disability has been recognized as postural orthostatic tachycar
