Preoperative antithrombotic therapy and risk of blood transfusion and mortality following hip fracture surgery: a Danish nationwide cohort study.

UNLABELLED: Hip fracture surgery is associated with high risk of bleeding and mortality. The patients often have cardiovascular comorbidity, which requires antithrombotic treatment. This study found that preoperative use of oral anticoagulants was not associated with transfusion or mortality following hip fracture surgery, whereas increased risk may exist for antiplatelet drugs. INTRODUCTION: Hip fracture surgery is associated with high bleeding risk and mortality; however, data on operative outcomes of hip fracture patients admitted while on antithrombotic therapy is sparse. We examined if p

Termination patterns of complex partial seizures: An intracranial EEG study.

PURPOSE: While seizure onset patterns have been the subject of many reports, there have been few studies of seizure termination. In this study we report the incidence of synchronous and asynchronous termination patterns of partial seizures recorded with intracranial arrays. METHODS: Data were collected from patients with intractable complex partial seizures undergoing presurgical evaluations with intracranial electrodes. Patients with seizures originating from mesial temporal and neocortical regions were grouped into three groups based on patterns of seizure termination: synchronous only (So)

Preoperative prediction of meningioma consistency using tumor-to-cerebellar peduncle intensity ratios on T2WI, DWI, and ADC maps.

Meningioma consistency is a critical factor in surgical planning, as firmer tumors are more difficult to resect and may increase operative time and risk. This study aimed to evaluate whether tumor-to-cerebellar peduncle T2-weighted imaging intensity (TCTI) ratios derived from T2-weighted imaging (T2WI), diffusion-weighted imaging (DWI), and apparent diffusion coefficient (ADC) maps can serve as reliable, noninvasive predictors of tumor consistency. This retrospective study included 102 patients with supratentorial WHO grade I meningiomas who underwent surgical resection between January 2021 an

Robotic-specific patient education reduces preoperative anxiety and improves outcomes after total knee arthroplasty.

The purpose of this study was to evaluate whether standardized robotic-specific patient information was associated with sustained reduction in preoperative anxiety in patients undergoing robotic-assisted TKA, and whether the reduction in anxiety was associated with improved postoperative outcomes and clinically meaningful recovery. This retrospective cohort study evaluated 106 patients who underwent primary unilateral robotic-assisted TKA. Anxiety was assessed using the Hospital Anxiety and Depression Scale-Anxiety subscale(HADS-A) at baseline, immediately after robotic-specific patient inform

Preoperative anorectal manometry is associated with cuffitis but not proximal pouch inflammation after IPAA creation.

BACKGROUND: Patients undergoing ileal pouch-anal anastomosis (IPAA) for inflammatory bowel disease (IBD) commonly experience postoperative inflammatory complications, including pouchitis and cuffitis. While pelvic floor dysfunction has been associated with these complications, the predictive value of preoperative anorectal manometry (ARM) remains unclear. We evaluated the association between abnormal preoperative ARM and postoperative inflammatory outcomes in IPAA patients. METHODS: In this historical cohort study we assessed IPAA patients who underwent preoperative ARM with ileostomy closure

Risk stratification with pre-operative myocardial perfusion imaging.

PURPOSE: Myocardial perfusion imaging (MPI) is frequently used to improve cardiac risk prediction in patients undergoing non-cardiac surgery. However, the data supporting this practice is derived from single center studies and predates current therapy for coronary artery disease (CAD). We evaluated the association between pre-operative testing indication and outcomes as well as predictors of death or myocardial infarction (MI) in these patients. METHODS: We include patients from the international, multicenter REFINE-SPECT registry (13 sites). Based on referral indication, patients were classi

Preoperative prediction of severe short-term complications in patients with bladder cancer undergoing radical cystectomy.

BACKGROUND AND OBJECTIVE: Radical cystectomy (RC) is associated with a high risk of postoperative complications. The prediction of individual patient risk for severe complications can facilitate preoperative shared decision-making. Patients with elevated risk may be referred to prehabilitation with the aim to mitigate the risk to improve perioperative outcomes. We developed models to predict severe short-term postoperative complications using preoperatively available clinical variables. METHODS: Data from a prospective cohort of 1313 RC patients treated between 1999 and 2021 was used. Preoper

Predicting Tumor Volume in Primary Hyperparathyroidism From Preoperative Clinical Data.

CONTEXT: Primary hyperparathyroidism (PHPT) is an endocrine disorder that is treated surgically, and some correlation between the size of the responsible lesion and preoperative clinical data is assumed. OBJECTIVE: The purpose of this study was to predict tumor volume of the lesion responsible for PHPT from preoperative clinical data. METHODS: Participants comprised patients with surgically treated PHPT who underwent initial surgery in our department between January 2000 and December 2021. The volume of parathyroid gland removed was used as the primary outcome and associations with preoperat

The presence of a Segond fracture in ACL-injured patients is associated with increased internal tibial rotation on preoperative MRIs.

BACKGROUND: Segond fracture is considered a component of the anterolateral complex (ALC) injury, yet the underlying cause and clinical outcomes of this bony avulsion remain subjects of debate. Additionally, MRI measurements of altered tibiofemoral position in anterior cruciate ligament (ACL)-injured patients with a Segond fracture have not been reported. The purpose of this study is to measure the rotational tibiofemoral position on MRI in ACL-injured patients with a Segond fracture. METHODS: A total of 44 patients with a primary ACL injury and a concomitant Segond fracture were included in t

Optimal preoperative autologous blood storage volume required in surgeries for placenta previas and low-lying placentas.

AIM: The high rate of stored preoperative autologous blood wastage is concerning. This study analyzed patients who provided preoperative autologous blood donations (PABDs) for massive bleeding during surgery for placenta previas and low-lying placentas, and investigated the optimal PABD storage volume required to avoid allogeneic transfusion. METHODS: Of 386 patients who provided PABDs at our hospital from 2008 to 2013, 269 patients with placenta previas or low-lying placentas were retrospectively analyzed. The PABD storage volumes were stratified into four groups based on the amounts stored,

[Preoperative evaluation of adult patients prior to elective, non-cardiac surgery: joint recommendations of German Society of Anesthesiology and Intensive Care Medicine, German Society of Surgery and German Society of Internal Medicine].

Evaluation of the patient's medical history and a physical examination are the cornerstones of risk assessment prior to elective surgery and may help to optimize the patient's preoperative medical condition and to guide perioperative management. Whether the performance of additional technical tests (e.g. blood chemistry, ECG, spirometry, chest-x-ray) can contribute to a reduction of perioperative risk is often not very well known or controversial. Similarly, there is considerable uncertainty among anesthesiologists, internists and surgeons with respect to the perioperative management of the pa

Preoperative Serum Transient Receptor Potential Ankyrin 1 (TRPA1) Levels in Class III Obesity: A Prospective Controlled Observational Study.

Transient Receptor Potential Ankyrin 1 (TRPA1) is a membrane cation channel activated by oxidative stress products that are chronically elevated in class III obesity. Although tissue-level TRPA1 biology is increasingly well described, the biological meaning of circulating immunoreactive TRPA1 remains poorly established. This single-centre, prospective, controlled observational study compared preoperative serum TRPA1 between adults with class III obesity scheduled for laparoscopic sleeve gastrectomy and contemporaneous controls, and explored metabolic and inflammatory correlates of the circulat

Preoperative swallowing muscle mass is associated with 1-year skeletal muscle loss and subsequent survival in lung cancer.

BACKGROUND & AIMS: Cancer cachexia is associated with systemic skeletal muscle loss and poor survival; however, early muscle changes preceding systemic muscle loss remain unclear. Therefore, this study aimed to determine whether tongue-complex muscle area (TCMA) can serve as an early indicator of systemic skeletal muscle loss in patients with lung cancer. Specifically, we examined its association with preoperative cachexia and the percentage change in erector spinae muscle cross-sectional area at 1 year after surgery (pcESMcsa). We further evaluated the prognostic significance of pcESMcsa usin
