Priapism developed in a cat 2 days after castration and strangury. The priapism was unresponsive to massage and antimicrobial treatment. Because of penile engorgement and necrosis, a perineal urethrostomy was performed. Clinical signs of priapism resolved after urethrostomy. Histologic examination of tissues revealed vascular stasis within the corpus cavernosum penis and funiculitis of the right spermatic cord.

The ultrastructure of erectile tissue from the corpora cavernosa penis of patients suffering from stasis priapism and high-flow priapism has been studied. Trabecular interstitial edema was confirmed as the first reaction of the tissue to the hemodynamic impairment. At the cellular level trabecular smooth muscle cells were found to be the first affected by the altered environmental conditions. A 26-year-old man with post-traumatic priapism was treated successfully by occlusion of the left internal pudendal artery with an autologous clot. The recovery was marked by the return of entirely normal sexual function. This excellent result suggests that autologous clot injection of the internal pudental artery should be considered as an appropriate form of therapy for the treatment of selected patients with priapism.

A patient developed priapism following a single intramuscular injection of chlorpromazine for hiccups. Conservative management was unsuccessful. During surgical intervention on the 15th hospital day, the corpora cavernosa of the turgid penis was irrigated and aspirated with heparinized saline using No14 gauge needles. The patient did well postoperatively. The physiology of the erection and the pathophysiology of priapism are discussed.
