Rheumatic fever and rheumatic heart disease: primary prevention is the cost effective option.

OBJECTIVE: To measure the economic output/input ratios for the various options of prevention of rheumatic fever/rheumatic heart disease (RF/RHD) and check the viability of primary prevention vis-à-vis secondary and tertiary preventions. METHODS: Cost accounting of the various prevention options was calculated for each variable as available in literature. Actual data as obtainable for the financial year ending March 2006 were computed for the Pondicherry population. Both direct and indirect costs (including community/social costs) were worked out using mostly primary data and wherever necessar…

Skin cancer prevention in the primary care setting: assessment using a standardized patient.

Our objective was to utilize the standardized patient technique in assessing the ability of primary care physicians to identify and counsel primary prevention for patients at high risk for skin cancer. A secondary goal was to test the feasibility of this technique as a measure of actual physician behaviors in the outpatient setting. We used a convenience sample of 15 primary care physicians. The standardized patient was an 18-year-old woman with skin phototype I. She presented to physicians as needing a general physical examination for a summer lifeguard job at a beach. She stated a family his…

HPV and cervical cancer prevention counseling with younger adolescents: implications for primary care.

PURPOSE: Primary care clinicians will continue to play an important role in cervical cancer prevention, particularly with regard to administration of the newly licensed human papillomavirus (HPV) vaccine and continued administration of Papanicolaou tests. Little is known about the factors that influence cervical cancer prevention counseling, particularly in the adolescent encounter. We conducted a qualitative study to better understand the implications for counseling about cervical cancer prevention by primary care clinicians who care for adolescents. METHODS: We conducted in-depth interviews

Anxiety Sensitivity Amelioration Training (ASAT): a longitudinal primary prevention program targeting cognitive vulnerability.

Fear of arousal symptoms, often referred to as anxiety sensitivity (AS) appears to be associated with risk for anxiety pathology and other Axis I conditions. Findings from a longitudinal prevention program targeting AS are reported. Participants (n=404) scoring high on the Anxiety Sensitivity Index (ASI) were randomly assigned to receive a brief intervention designed to reduce AS (Anxiety Sensitivity Amelioration Training (ASAT)) or a control condition. Participants were followed for up to 24 months. Findings indicate that ASAT produced greater reductions in ASI levels compared with the contro

[Evidence-based, best-practice guidelines for primary prevention of osteoporosis and osteoporotic fractures].

Community health interventions aimed at increasing peak bone mass and decreasing lifelong bone loss could substantially reduce the incidence of osteoporosis and osteoporotic fractures. Before now, no evidence-based practice guidelines for primary prevention of osteoporosis and osteoporotic fractures have been produced for public health center workers, primary care providers, and community health specialists. We constructed guidelines that present the effects of life-style factors, such as food and nutritional intake, in (1) increasing peak bone mass among young adults, (2) minimizing bone loss…

Audit of bloodborne virus prevention activity with drug users seen in primary care.

Drug misuse is increasingly being managed in general practice. It has been proposed that better use could be made of this contact to identify people with bloodborne virus (BBV) morbidity and to deliver prevention strategies. The Hepatitis B and C Prevention Project was designed to enhance the work of primary healthcare teams in preventing transmission of BBVs in people known to have a history of problem drug use. As part of this work a baseline audit of current provision was undertaken and the results are reported here. Primary care records of 1278 people with a history of illicit drug use wer

Epidemiology of early primary hypertension and implications for prevention: the Bogalusa Heart Study.

Epidemiological studies of BPs in children and young adults over the past 20 years have contributed considerably to understanding the early onset of primary hypertension. Observations from autopsies and echocardiographic studies, together with long-term BP studies, of children clearly indicate that primary hypertension begins in early childhood. Although abnormal BP levels in children are much lower than the adult criteria used for clinical diagnosis of hypertension, essential hypertension is identifiable in early life. Complex haemodynamic and metabolic mechanisms related to essential hyperte

Strategies for modifying sexual behavior for primary and secondary prevention of HIV disease.

Sexual behavior change is needed for both the primary and secondary prevention of HIV disease. Remarkable progress has been made in modifying high-risk behaviors in selected populations (e.g., gay men living in the epicenters of the epidemic). However, many at-risk populations (e.g., gay men living outside these epicenters, homosexual men who are young or Black, ethnic minorities--especially women of childbearing age, and persons over age 50) have received relatively little research attention and may be at risk for further infection. A community-level approach to behavior change represents one

Age-dependent versus single SCORE2 thresholds for identifying carotid atherosclerosis in primary prevention.

BACKGROUND AND AIMS: European Society of Cardiology guidelines permit either age-dependent or single age-independent SCORE2 thresholds for atherosclerotic cardiovascular disease (ASCVD) risk classification. This study examined their concordance with carotid atherosclerosis. METHODS AND RESULTS: We studied 954 adults aged 40-69 years without ASCVD, diabetes, or chronic kidney disease from community screening programs (mean age 56.7 ± 8.0 years, men 42.9%, antihypertensive/lipid-lowering drug treatment 40.9%/43.7% respectively, SCORE2 5.4 ± 3.4%). Eligibility of real-world patients for aspirin primary prevention trials in cardiovascular disease.

BACKGROUND: Evidence for the net benefit of aspirin for primary prevention of cardiovascular disease (CVD) is finely balanced, leading to variation in guideline recommendations internationally. External validity of randomised clinical trial (RCT) evidence may therefore be of particular importance. The aim of this study is to characterise real-world patients according to their eligibility for guideline-cited aspirin RCTs for primary CVD prevention. METHODS: Eligibility criteria from 14 RCTs were applied to a linked primary care/hospital discharge dataset of people ≥ 40 years without CVD. Propo…

Modifiable Lifestyle and Environmental Determinants of Ovarian Cancer Risk: Implications for Primary Prevention.

Ovarian cancer (OC) remains the most lethal gynecologic malignancy worldwide, largely due to late-stage diagnosis and the absence of effective screening strategies. As a result, primary prevention is a critical approach to reducing disease burden. This narrative review summarizes current evidence on modifiable lifestyle, reproductive, and environmental factors associated with OC risk, based on a comprehensive PubMed, MEDLINE, Scopus, and Web of Science search conducted through April 2026. Consistent protective associations have been reported for reproductive factors, including parity, breastfe…
