Local Coverage Determinations for Peripheral Nerve Injections and Procedures for Chronic Pain: Policy Changes, Clinical Limitations, and Health-Equity Implications.

Five Medicare Administrative Contractors (MACs)-CGS, Palmetto GBA, Wellpoint Federal (formerly National Government Services), Wisconsin Physicians Service, and Noridian-participated in a collaborative policy-development process addressing peripheral nerve injections and related procedures for chronic pain. Novitas Solutions and First Coast Service Options did not participate. The September 2025 draft policies would have limited coverage principally to radiofrequency neurolysis for trigeminal neuralgia and corticosteroid injections for carpal tunnel syndrome and Morton neuroma, with lifetime ca

The Effect of Different Types of Music on Pain and Anxiety during Wound Care Procedures in Patients with Venous Ulcers: A Randomized Controlled Trial.

OBJECTIVE: To evaluate the effects of different types of music (instrumental Turkish music and Western music without lyrics) played during wound care on pain, anxiety, and patient satisfaction among individuals with venous ulcers. METHODS: In this randomized controlled trial, 97 patients with venous ulcers were assigned to three groups: Western music ( n  = 32), Turkish music ( n  = 32), and Control ( n  = 33). Patients in the music groups listened to their assigned music during routine wound dressing, while all patients received individualized, evidence-based care. Pain and anxiety were meas

Pro-Con Debate: Air or Saline for Loss-of-Resistance Technique of Interlaminar Epidural Access with Fluoroscopy-Guided Pain Medicine Procedures.

In this Pro-Con commentary article, the Pro side advocates that using the loss of resistance (LOR) to air technique for interlaminar epidural access is safe as the risk of pneumocephalus is exceptionally low. They note that case reports detailing complications typically utilize at least 1 mL of air, frequently exceeding this amount. Even a 1-mL dosage surpasses the requirement, particularly when fluoroscopy is applied in the context of chronic pain management. The proponents also suggest a modified technique, where a syringe containing a drop of saline or contrast acts as a visual cue, minimiz

Age-Related Variations in Postoperative Pain Intensity across 10 Surgical Procedures: A Retrospective Study of Five Hospitals in South Korea.

Age-related differences in pain perception have been reported in various contexts; however, their impact on postoperative pain intensity remains poorly understood, especially across different surgical procedures. Data from five hospitals were retrospectively analyzed, encompassing patients who underwent 10 distinct surgical procedures. Numeric rating scale scores were used to assess the worst postoperative pain intensity during the 24 h after surgery. The multivariate linear regression model analyzed the relationship between age and pain intensity. Subgroup analyses were performed according to

Substance P concentrations in the blood plasma and serum of adult cattle and calves during different painful procedures and conditions - a systematic review.

BACKGROUND: Pain in cattle is a major welfare problem, as cattle mask their pain. Subjective and objective parameters to assess pain in cattle have been described. Among the objective parameters to evaluate pain in cattle is substance P (SP). SP is a neurotransmitter, which is involved in the processing of noxious information to the brain; it seems to be a more objective indicator for nociception than cortisol, which has long been used as a biomarker for pain and stress in cattle. The objective of this systematic review was to assess the existing literature about SP during painful procedures,

Physicians Prescribe More Opioid Than Needed to Treat Pain in Children After Outpatient Urological Procedures: An Observational Cohort Study.

BACKGROUND: The epidemic of nonmedical use of prescription opioids (NMUPOs) has been fueled in part by the availability of leftover, legitimately prescribed opioids. In children, outpatient urological procedures are among the most common surgeries performed, but data are lacking to guide appropriate postoperative opioid prescribing. The aim of this study was to compare the amount of prescribed opioid medication to the amount taken for acute pain after minor pediatric urological surgery and to determine the disposition of excess opioid. In addition, we explored whether distinct patient characte

Detecting pain in traumatic brain-injured patients with different levels of consciousness during common procedures in the ICU: typical or atypical behaviors?

PURPOSE: Pain behaviors such as grimacing and muscle rigidity are recommended for pain assessment in nonverbal populations. However, these behaviors may not be appropriate for critically ill patients with a traumatic brain injury (TBI) depending on their level of consciousness (LOC). This study aimed to validate the use of behaviors for assessing pain of critically ill TBI adults with different LOC. METHODS: Using a repeated measure within subject design, participants (N=45) were observed for 1 minute before (baseline), during, and 15 minutes after 2 procedures: (1) noninvasive blood pressure

Postoperative pain medication requirements in patients undergoing computer-assisted (“Robotic”) and standard laparoscopic procedures for newly diagnosed endometrial cancer.

PURPOSE: Laparoscopy (LSC) offers superior patient outcomes compared to laparotomy. Small retrospective/prospective series have suggested robotics offers further reduction in postoperative pain and pain medication use compared to standard LSC. Our objective was to compare postoperative pain in patients undergoing robotically assisted (RBT) versus standard LSC for newly diagnosed endometrial cancer. METHODS: All preoperative endometrial cancer cases scheduled for RBT and LSC from May 1, 2007 to June 9, 2010 were identified. For this analysis, we only included cases not requiring conversion to

Comparison of "standard" and "navigated" procedures of TMS coil positioning over motor, premotor and prefrontal targets in patients with chronic pain and depression.

Since about 15 years, transcranial magnetic stimulation (TMS) is used as a technique to investigate the function of specific cortical regions. Single pulse TMS studies have targeted the dorsolateral premotor cortex (dlPMC) to characterize premotor-motor interactions in movement disorders. Repetitive TMS (rTMS) trials have targeted the dorsolateral prefrontal cortex (dlPFC) to treat depression. In almost all previous studies, these targets have been defined according to a "standard" scalp distance to the site of stimulation evoking motor responses of maximal amplitude in the contralateral hand

Virtual Reality for Pain, Anxiety, and Patient Experience in Awake Urological Procedures: A Systematic Review and Meta-Analysis of Randomised Controlled Trials.

Awake urological procedures, including cystoscopy, transrectal ultrasound-guided prostate biopsy (TRUS-Bx), extracorporeal shock wave lithotripsy, vasectomy, and urodynamic studies, are consistently associated with procedural pain and anxiety, with downstream consequences for surveillance compliance and patient experience. Head-mounted display (HMD) virtual reality (VR) has emerged as a non-pharmacological adjunct, but existing reviews have been narrative or restricted to a single procedure type. This systematic review and meta-analysis evaluated head-mounted display VR across the full breadth…

Music Intervention for Anxiety and Pain During Breast Biopsy Procedures: A Comprehensive Systematic Review and Meta-Analysis of Randomized Controlled Trials.

INTRODUCTION: Breast biopsies often cause anxiety and pain that influence patients' experience and procedure outcomes. Music interventions offer a practical alternative to pharmacological methods for reducing these effects. METHODS: We conducted a PRISMA-compliant systematic review and meta-analysis to evaluate the efficacy of music during breast biopsies. We searched PubMed, Cochrane CENTRAL Library, EMBASE, and ClinicalTrials.gov up to May 2025. We included randomized controlled trials (RCTs) comparing music interventions with routine care without music intervention. The primary outcomes we

Interventional Pain Procedures: A Narrative Review Focusing on Safety and Complications. Part 4 - Peripheral Nerve, Joint, Musculoskeletal, and Regenerative Interventions.

The first three installments of this series examined the safety and complications of injections for spinal pain (Part 1), interventional procedures for back pain (Part 2), and sympathetic and ganglion-targeted techniques (Part 3). This fourth and concluding article addresses the interventions performed away from the neuraxis and the sympathetic chain, which together account for much of everyday pain practice: peripheral and cranial nerve procedures (blocks, chemical neurolysis, radiofrequency, cryoneurolysis, and peripheral or occipital nerve stimulation); joint, bursal, and tendon injections;
