In a prospective study twenty patients with benign intracranial hypertension, 15 females and five males with mean age 34 years (range 12-61 years), were followed up from 12 to 61 months (mean 22 months). Initially all patients showed marked papilledema, normal visual acuity, considerable enlargement of the blind spot area, and significantly delayed pattern reversal visual evoked potentials. From 1972 to 1982, 22 patients were admitted for prosthetic valve endocarditis on homografts (3), bioprostheses (2) or mechanical prostheses (17). Staphylococcus epidermidis was the most common aetiologic agent in early-onset endocarditis and streptococci in late-onset. Echo-cardiographic or cinefluoroscopic findings enhanced diagnosis mainly in advanced stages of the disease. Despite medical therapy and prompt reoperation in most cases, mortality remains high: 66%, 80% in the early group, 50% in the late group; the prognosis was definitely improved, however, by surgical treatment.

Multiple injury is often associated with an unstable serial rib fracture and a pulmonary contusion under the contact area which both lead to respiratory insufficiency. This has to be treated at an ICU by respirator with IMV and PEEP from the beginning. Eighty tarsal dislocations treated between 1954 and 1979 are presented. Our of 48 cases reviewed after a mean follow-up time of 7 years, satisfactory results were achieved in 30 (62%). Uterine bleeding in early pregnancy represents a definite threat to the developing embryo and constitutes a source of anxiety to both the patient and her clinician. In a prospective study of women with threatened abortion (TA) in the first and second trimesters of pregnancy in Ibadan, Nigeria, 48.8% of them aborted their pregnancies and there was no significant association between the various sociomedical factors considered and the outcome of pregnancy after TA. Adequate antihypertensive therapy will lower blood pressure to normotensive or near normotensive levels in 80-85% of patients. Long-term treatment results in a marked decrease in strokes and stroke recurrence, heart failure, renal failure, and progression to accelerated hypertension. The effects of long-term therapy on the occurrence of coronary artery disease are unclear.

Out of more than 5200 struma resections 2.2% were treated because of thyroid carcinoma. Over a 12 year' period records of 117 patients were analysed. Treatment consisted in radical thyroidectomy, radioactive iodine or radiation therapy and hormone-substitution. Differentiated carcinomas were observed more frequently, primarily extended disease decreased. The mean 5 year survival rate of differentiated tumours was 90%. The authors report a case of lymphoblastic lymphoma in a woman of 41 years, with characteristic skin lesions, long evolution and relative benignity (good general condition, lymph nodes, liver and spleen not palpable, and the bone marrow study was normal). We wish to underline the existence of a Monoclonal Disglobulinemia (Paraproteins in the zone of Inmunoglobulin M); this observation was made in some cases of leukemia. This laboratory data would be of a prognostic value, indicating a rapid malign evolution, in contrast with other clinical and laboratory manifestations.

698 children below age sixteen years with severe mental handicap (below IQ50) who were admitted to hospital between 1956 and 1959 were surveyed for genetic prognosis. In a series of 645 patients with epilepsy only 38 had attacks exclusively during sleep. A further 111 had attacks while waking and sleeping. The total number of patients whose epilepsy started with sleep attacks and who later developed attacks when waking increased with each successive year. The prognosis of sleep epilepsy is important in relation to the granting of driving licences.
