Implant Rehabilitation Following Loss of Tooth Restorability Associated With Poorly Controlled Type 2 Diabetes Mellitus: A Case Report.

Type 2 diabetes mellitus is associated with an increased risk of periodontal disease, impaired bone metabolism, delayed wound healing, and progressive alveolar bone loss, all of which may adversely affect the prognosis of compromised teeth. This case report describes the management of a 42-year-old man with type 2 diabetes mellitus who presented with a severely compromised maxillary canine. The patient had previously been advised to undergo definitive restorative treatment with a full-coverage crown because of extensive structural deterioration. However, financial limitations, inconsistent med

Early-Onset Syringomyelia: A Rare Complication of TB Meningitis.

Syringomyelia is a rare but recognized complication of tuberculosis (TB) meningitis, typically associated with spinal arachnoiditis and disruption of cerebrospinal fluid dynamics. Although the exact mechanisms are not fully understood, early onset during the acute phase of illness is uncommon. We report the case of a 30-year-old male who presented with progressive neurological symptoms one week after initial evaluation for headache and fever following travel to a TB-endemic region. He developed gait unsteadiness, tremors, and nystagmus and was subsequently found to have a contrast-enhancing le

Hospitalist management of vaso-occlusive pain crisis in patients with sickle cell disease using a pathway of care.

BACKGROUND: Patients with sickle cell disease (SCD) suffer from intermittent vaso-occlusive pain crises (VOCs). These crises lead to frequent hospitalizations, significant morbidity, and increased mortality risk. Care pathways can enhance efficiency and quality of care. Our study sought to evaluate the development and implementation of a care pathway for patients with SCD experiencing VOCs. METHODS: The University of North Carolina (UNC) Comprehensive Sickle Cell Program provides all levels of care for a large population of patients with sickle cell anemia. All patients admitted to UNC Hospit

Comprehensive care in Huntington's disease: a physician's perspective.

Huntington's disease is a slowly progressive neurodegenerative disorder with wide-ranging effects on affected individuals and their families. Until a cure is found for the disease, patients and their families will continue to need care over years, even generations. The ideal care for HD is provided by a team, led by a physician, with input from rehabilitation therapists, nurses, psychologists, genetic counselors, social workers, and other health care providers. The goals of care are to maximize the quality of life at all points through the course of the disease, in part by anticipating problem

[Extracranial vascular anomalies (hemangiomas and vascular malformations) in children and adolescents--diagnosis, clinic, and therapy].

The field of extracranial vascular anomalies is considered as special focus of pediatric otolaryngology and it has shown a rapid development during the last years. The reason for this interest is finally also due to the global acceptance of the classification introduced by the ISSVA (International Society for the Study of Vascular Anomalies). Hemangiomas are the most frequently observed vascular tumors. Today the systemic propranolol therapy is mostly used for therapy of hemangiomas requiring treatment. Increasingly, the topical application of beta blocker is discussed while the benefit in the

Charting the future: A multi-setting medical chart study of advance care planning in Parkinson's disease and atypical Parkinsonism.

BackgroundAdvance Care Planning (ACP) enables people with progressive diseases to document and communicate future treatment preferences. For Parkinson's disease (PD) and atypical parkinsonism (AP), ACP is particularly important because of disease complexity, symptom burden, the progressive nature of the disease, and cognitive decline, yet its implementation across care settings remains unclear.MethodsWe conducted a medical chart review of 189 deceased patients with PD or AP who died in nursing homes (n = 91), hospitals (n = 58), or at home (n = 40) in the Netherlands. For patients who died at

Esophageal Exclusion as a Damage-Control Strategy for Acute Esophageal Necrosis Secondary to Ruptured Stanford Type B Aortic Dissection: A Case Report.

Acute esophageal necrosis (AEN) is a rare and life-threatening condition most commonly associated with systemic hypoperfusion and critical illness. Aortic-related AEN is exceptionally uncommon and may result from direct mediastinal compression and vascular compromise rather than generalized hemodynamic insufficiency. We report the case of a 77-year-old man presenting with a Stanford type B aortic dissection associated with aneurysmal degeneration complicated by rupture. The patient presented with progressive dysphagia, back pain, and hemoptysis. Computed tomography angiography demonstrated a l

Guillain-Barré Syndrome Emerging From Syringomyelia Management: A Case Report.

The coexistence of Guillain-Barré syndrome with a spinal cord syrinx is extremely rare. This report presents the emergency admission of a 57-year-old female presenting with an abrupt and progressive loss of motor function and abnormal sensation in her lower extremities. MRI of the cervical spine revealed a syrinx extending from C2 to C7. Neurology referred the case to neurosurgery, who assessed the patient and recommended nonoperative and supportive treatment given the size of the syrinx and the patient's status as a poor surgical candidate. She was subsequently discharged to a local extended

Reanalysis of membranoproliferative glomerulonephritis patients according to the new classification: a multicenter study.

BACKGROUND: All types of membranoproliferative glomerulonephritis (MPGN) are progressive diseases with poor prognoses. Recently, a newly proposed classification of these diseases separated them into immune complex- and complement-mediated diseases. We investigated the frequency of C3 glomerulonephritis among previously diagnosed MPGN patients. METHODS: We conducted a retrospective study of patients diagnosed with MPGN at three tertiary care institutions between 2001 and 2010. We investigated the incidence of complement-mediated disease among patients diagnosed with MPGN. Progressive renal dys

Modifying Neuropsychiatric and Motor Trajectories in Huntington's Disease Through an Integrated Neurobehavioral Clinic Model.

INTRODUCTION: Huntington's disease (HD) is a progressive neurodegenerative disorder characterized by worsening motor dysfunction, cognitive decline, and psychiatric symptoms. Psychiatric symptoms in HD arise from progressive frontostriatal circuit dysfunction and often precede motor manifestations, contributing significantly to disability and functional decline. In the absence of disease-modifying therapies, clinical care focuses on symptom management and functional preservation. METHODS: This was a retrospective pre-post chart review comparing longitudinal clinical outcomes before and after

Neurofibromatosis Type 2: A Rare Case of Multiple Intracranial Schwannomas, Meningiomas, and Ependymomas (MISME) Syndrome and Literature Review.

Neurofibromatosis type 2 (NF2) is a rare genetic disorder affecting the nervous system, primarily characterized by benign tumor formation, including bilateral vestibular schwannomas, meningiomas, and ependymomas. These tumors are collectively known as MISME (multiple intracranial schwannomas, meningiomas, and ependymomas) syndrome. We report a case of a 35-year-old woman with no notable medical or family history, who presented with progressive hearing loss and right upper limb weakness. Magnetic resonance imaging revealed multiple central nervous system tumors, confirming the diagnosis of NF2.

An Atypical Presentation of Creutzfeldt-Jakob Disease With a Heidenhain Variant and Balint's Syndrome.

Creutzfeldt-Jakob disease (CJD) is known for a rapidly progressive decline in cognitive functions due to an underlying infection from a prion. This case involves a unique and atypical variant of CJD that was difficult to diagnose for the investigating medical teams. As such, it highlights the particular challenge of using traditional diagnostic algorithms for atypical variants of CJD, in terms of time-appropriate diagnostics. The patient presented with a sudden episode of vertigo which was treated as an isolated symptom. While succeeding investigations involving neurology and otolaryngology sp

Multidisciplinary management of refractory malignant upper gastrointestinal bleeding in metastatic gastric adenocarcinoma.

Upper gastrointestinal bleeding (UGIB) secondary to malignancy is frequently refractory to conventional endoscopic therapies. We report a man in his 60s who presented with severe anaemia, melaena and haemodynamic instability. Urgent investigation demonstrated a friable, actively bleeding fundal mass with imaging features suspicious for metastatic gastric malignancy. Standard haemostatic measures, including epinephrine injection and haemostatic clips, failed to achieve bleeding control, resulting in progressive transfusion dependence. After multidisciplinary review, salvage endoscopic haemostas

The utility of boxing for cardiac prehabilitation.

A 56-year-old man with end-stage heart failure performed a 4-week, symptom-limited, progressive inpatient cardiac prehabilitation program while confined to the cardiovascular intensive care unit awaiting heart transplantation. Mobility was limited by an acute gout flare and multiple central venous access lines. He received a tailored prescription of intermittent boxing, supervised hallway ambulation, stair training, and golfing on a putting green on four consecutive weekdays and was encouraged to mobilize with nursing on the remaining days. The patient progressed and by the last week demonstra

Lung Cancer With Vertebral Metastases Presenting as Low Back Pain in the Chiropractic Office: A Case Report.

Lung cancer commonly metastasizes to the skeletal system, and when affecting the spine, it may initially be mistaken for a typical musculoskeletal source of back pain. We report a previously healthy 52-year-old male non-smoker with an eight-week history of low back pain that radiated into his left thigh and recent weight loss, yet no respiratory symptoms. Initially, the patient visited his primary care physician, who suspected a musculoskeletal condition and prescribed a nonsteroidal anti-inflammatory drug and muscle relaxant, then referred the patient to the chiropractor. Based on the patient

Nonopportunistic infection leading to rapidly progressive dementia in a patient with HIV/AIDS: A case report.

RATIONALE: Cognitive dysfunction is a common presenting symptom in patients with HIV/AIDS. It is usually directly associated with HIV infection or due to opportunistic infection. Rapidly progressive dementia, however, is rarely observed in acute HIV infection or during immune reconstitution. Recently, a case of Creutzfeld-Jakob disease (CJD) has been reported in a patient with chronic HIV infection. The incidence of CJD is not known to be increased among immunocompromised patients. PATIENT CONCERNS: We here report the case of a 59-year-old male patient with a recent diagnosis of HIV/AIDS and

Enhancing Patient-Centered Home Palliative Care for Heart Failure: A Concept Analysis to Guide Nursing Practice and Care Coordination.

To identify and describe components of the home-based palliative care model for patients with heart failure, re-conceptualize the process, and update the existing model to inform nursing research and clinical practice. Heart failure, as a chronic progressive disease, has a continuously increasing incidence and medical burden, and requires multidimensional clinical management strategies. Although palliative care is recommended for improving the quality of life of heart failure patients, its utilization rate is still low, and the core components and implementation pathways of home-based palliati
