BACKGROUND: Prosthetic joint infection (PJI) due to rapidly growing mycobacteria (RGM) is only occasionally encountered in clinical practice. Therefore, the optimal clinical management for this condition is unknown.
METHODS: The medical records of patients who had PJI due to RGM during 1969-2006 were reviewed to summarize its clinical characteristics, treatment, and outcome.
RESULTS: Eight patients developed 9 episodes of PJI (7 episodes involving the knee and 1 each involving the hip or elbow) due to RGM at a median of 312 weeks (range, 1-170 weeks) after prosthesis implantation. Patients p

When treating an infected hip prosthesis, there are a number of medical, technical and biomechanical considerations. No ideal solution for all problems is available. In individual cases an optimal compromise must be found. The means for curing the infection and the one for maintaining hip stability can counteract each other. Basically, three options are available: removal of the prosthesis without replacement and one- or two-stage replacement. With all three options thorough surgical debridement is vitally important. The introduction of antibiotic cement and cementless titanium implants, toget

The overall incidence of osteomyelitis following primary knee arthroplasty is 1-2%. Major risk factors are large prostheses, rheumatoid arthritis, postoperative wound-healing complications, skin infections, and prior deep infections. The major infecting organism is Staphylococcus aureus. Healing of the infection is to be expected in 20% when systemic antibiotics alone are used, in 24% when soft tissue surgery is used, in 50% when resection arthroplasty is used, in 76% when revision arthroplasty is used, in 90% when arthrodesis is used, and in 100% when amputation is used. (1) Background: Megaprosthetic reconstruction of the knee is frequently required in limb salvage surgery for oncologic indications and in complex revision arthroplasty. The Modular Universal Tumor and Revision System (MUTARS) Total Knee prosthesis is widely used in these situations. The aim of this study was to evaluate revision-free implant survival following MUTARS Total Knee implantation and to compare outcomes between primary implantation and use as a conversion procedure after failure of a previous knee prosthesis. (2) Methods: A retrospective cohort study was performed including 36 patie

We describe a case of endocarditis and aortic root abscess caused by Bartonella clarridgeiae bacteria in a patient in Australia. The patient initially sought care for leg pain and was found to have bilateral tibioperoneal trunk mycotic aneurysms. 16S rRNA PCR on excised aneurysm tissue identified the cause as B. clarridgeiae.

BACKGROUND: The DAIR procedure (debridement, antibiotic therapy, and implant retention) represents a treatment option for acute periprosthetic joint infections (PJI). Compared to revision arthroplasty, it is technically less invasive and, under appropriate indications, can preserve a well-fixed endoprosthesis. However, treatment success depends on numerous patient-, pathogen-, and procedure-related factors.
METHODS: Based on a comprehensive review of current international studies and guidelines, we systematically analyzed the key factors influencing the success of DAIR. In addition, clinical

BACKGROUND Haemophilus influenzae is a gram-negative bacterium that normally inhabits the mucosal surfaces of the upper-respiratory tract system of many healthy individuals. The capsular types of H. influenzae are found to be responsible of invasive infections such as meningitis, epiglottitis, orbital cellulitis, and bacteremia. H. influenzae is rarely described as a causative agent of periprosthetic joint infection (PJI). Here, we report a case of H. influenzae causing prosthetic knee joint infection 1 year after total knee arthroplasty. CASE REPORT A 59-year-old woman was admitted with a 1-d

Cutibacterium acnes is a commensal skin bacterium, regularly implicated in prosthetic joint infection, particularly of the shoulder. Diagnosis has been improved by progress in bacteriological techniques: longer culture time, liquid medium culture, MALDI-TOF mass spectrography, and universal 16S rRNA PCR, associated by some authors to sonication of ablated implants. C. acnes pathogenicity involves many virulence factors, notably including biofilm formation. C. acnes may lead to infection that is clinically evident or, frequently, relatively asymptomatic. C. acnes is an anaerobic Gram-positive b

BACKGROUND: In the context of an increase number of primary and revision total hip and total knee arthroplasty performed yearly, an increased risk of complication is expected. Prosthetic joint infection (PJI) remains the most common and feared arthroplasty complication. Ralstonia pickettii is a Gram-negative bacterium, that has also been identified in biofilms. It remains an extremely rare cause of PJI. There is no report of an identification of R. pickettii on an extracted spacer loaded with antibiotic.

Streptococcus bovis is a very rare cause of infection of joint arthroplasty. Infections with S. bovis have been reported to be associated with various gastrointestinal conditions. We present a case of total knee arthroplasty infected with S. bovis 40 months after a successful knee replacement. The diagnosis was established on joint fluid as well as blood cultures. Early diagnosis, arthroscopic wash out and appropriate antibiotics helped salvage the prosthesis. Early colonoscopy revealed carcinoma of the sigmoid colon that was surgically resected.

Endocarditis caused by Bartonella species is difficult to diagnose and still remains a rare entity. Therefore, a young male patient undergoing aortic valve replacement for culture-negative endocarditis is reported in whom the diagnosis of a Bartonella quintana infection was made with a great delay postoperatively.

With the emergence of staphylococci as important hospital pathogens, administration of vancomycin has increased dramatically. Economic and other factors have led to the development of once-daily dosing protocols to simplify and standardize the administration of antibiotics. A patient with Staphylococcus aureus prosthetic valve endocarditis and normal renal function failed therapy with vancomycin once/day. The infection was cured when the regimen was changed to the conventional 12-hour dosing interval.

Penile prosthesis surgery is an effective and durable treatment modality for patients who have failed conservative management for erectile dysfunction (ED). Thorough patient counseling and appropriate preoperative workup lay the foundation for a successful outcome. While the risk of infection of penile prosthesis is rare, it is a dreaded complication with dire consequences. The goal of the prosthetic surgeon is to minimize the risk of preventable complications. Given the common prevalence of benign prostatic hyperplasia (BPH) in this patient population, it is essential that providers are famil

BACKGROUND: There is little evidence on techniques for management of peri-prosthetic infection (PJI) in the context of severe proximal femoral bone loss. Custom-made articulating spacers (CUMARS) utilising cemented femoral stems as spacers was described providing better bone support and longer survival compared to conventional articulating spacers. We retrospectively report our experience managing PJI by adaptation of this technique using long cemented femoral stems where bone loss precludes use of standard stems.

Metal-on-polyethylene (MoP) total hip arthroplasty (THA) prostheses are known to release metal debris. Basic science studies suggest that metal implants induce a pro-inflammatory response that ultimately chemoattracts leukocytes including macrophages and neutrophils to the surgical site. This raises concern of higher risk of infection with these prostheses through the "trojan horse" mechanism by which neutrophils and macrophages transport intracellular pathogens from a remote site. This study compared the infection occurrence between MoP and ceramic-on-polyethylene (CoP) implants to determine
