Peripartum cardiomyopathy in the form of congestive heart disease of unknown etiology appears relatively rarely during the last month of pregnancy or within 5 months after delivery, and it is potentially life-threatening. This case report describes a 34-year-old female, G4P1, at 36 weeks' gestation who was admitted for cesarean section under lumbar epidural anesthesia due to twin pregnancy. Six patients who developed de Quervain's disease during pregnancy or within 12 months of childbirth were treated surgically. Histopathological examination of the tendon sheaths using alcian blue stain showed that myxoid degeneration was responsible for the thickening observed in the sheath. Characteristic intramural deposits of mucopolysaccharides were present, predominantly in the subsynovial region. Acute or chronic inflammatory changes were not seen. These findings were the same as those described in patients with de Quervain's disease unrelated to pregnancy or childbirth.

Retinal detachment is an unusual complication of hypertensive disorder in pregnancy. It has been reported in 1% to 2% of patients with severe preeclampsia and in 10% of patients with eclampsia. Choroidal ischemia may be the cause of retinal detachment. We know that mild arteriolar spasm involving the bulbar conjunctival vessels has been observed in the normal pregnancy, but in pregnancy-induced hypertension the vasospasm may be severe and result in choroidal ischemia. We present one of the rarer causes of pregnancy-associated jaundice in the immediate puerperium. A 30-year-old multipara with pyruvate kinase deficiency became jaundiced within a few hours of delivery, following an uneventful pregnancy and labour at term. She responded to conservative management, the jaundice clearing and the altered liver function showing a steady return to normal. Pyruvate kinase deficiency may cause pregnancy-associated jaundice. A thorough investigation is still necessary to exclude other causes of jaundice in pregnancy and the puerperium.

CASE SUMMARY: A 36-year-old woman presented with an acute myocardial infarction 2 weeks after the birth of her first child. The patient had smoked two packs of cigarettes a day for 12 years and had been taking bromocriptine to suppress lactation. While in the emergency room, the patient went into ventricular fibrillation, but defibrillation successfully restored sinus rhythm. Coronary angiography revealed several atherosclerotic lesions. The patient refused to undergo coronary artery bypass grafting, and she was discharged receiving medical therapy.

Over a five year period, 11 cases of Sheehan's syndrome were seen in the Endocrinology Unit of a teaching hospital in Ibadan, Nigeria. The mean age at diagnosis was 35.1 years; the mean parity was four and the average interval between the obstetric injury and diagnosis was 6.9 years. Four patients were in middle socio-economic class and seven were in low socio-economic class. Urinary leakage was reported in 53.5% of our patients at least once during pregnancy. Multigravidae and women older than 30 were affected more often than primigravidae or women younger than 30. 6.2% of all women, who were continent before pregnancy, developed permanent stress incontinence after vaginal delivery. Hypopituitarism depends on the history of the obstetric cause, on the subsequent amenorrhoea and the usual absence of lactation, on the biochemical abnormalities found later, and on the effect of therapy. Nevertheless, an initial clinical inspection of the untreated patient gives important clues to the diagnosis. Cachexia does not occur in true hypopituitarism, unless there is some intercurrent factor which causes emaciation, such as carcinoma, tuberculosis, ulceration in the alimentary tract etc.

Severe ectopic decidual reaction of the peritoneal surface as the etiology of unexpected intraabdominal hemorrhage is rare. Such a situation occurred in one of our patients after an uneventful vaginal delivery. The bleeding was discovered during postpartum sterilization, and hysterectomy was performed to control it.
