A two-marker technique was used to determine duodenogastric reflux in fasting dogs with normal or surgically modified gastroduodenal junctions. All nine dogs had an esophagostomy for gastric marker perfusion. The duodenal marker was given via a duodenal fistula. In two dogs a Heineke-Mikulicz pyloroplasty was performed, and in four dogs extramucosal circular pylorectomy was performed in addition. The mean fasting duodenogastric reflux rate in dogs with a normal pylorus was 1.1 +/- 0.5 (SE) ml/10 min; after pyloroplasty it was 1.6 +/- 0.3 ml/10 min (P greater than 0.1), and after pylorectomy it…

It was shown with the aid of thymidine-H3 that the mitotic cycle of mucous-forming cells (superficial epithelial mucosal cells of the neck) of the stomach pyloric glands of mice lasted 13.5 hrs (G1+1/2M = 7.6 hrs, S = 5.3 hrs; G2+1/2M = 0.6 hrs). With the administration of a physiological dose of hydrocortisone (0.1 mg) the duration of the mitotic cycle of mucous-forming cells of the stomach pyloric glands increased by 6.7 hrs (G1+1/2M = 11.6 hrs, S = 7.8 hrs; G2+1/2M = 0.8 hrs). A high dose of the hormone had a similar effect and increased the presynthetic period to 12.9 hours and the postsyn

Two new cases of patients with so called muscular pyloric band ("double pylorus") are reported. Both possible genetic mechanisms of this malformation are discussed in detail: primary or congenital, and secondary or acquired by inflammation. After analysis of the hitherto related cases, the opinion is expressed that the so called pyloric band represents a congenital malformation. According to the authors, the striking frequency of simultaneous occurrence of pyloric band and ulcers does not necessarily favour the theory of inflammatory genesis. The accompanying ulcer seems rather to be a "second…

Congenital gastric ectopic pylorus (CGEP), also known as gastric ectopic pyloric opening, is a rare congenital gastric abnormality. It was first reported and termed by Yu and Zhao from China in 1983, and in 2007, Uraz et al published the first report of CGEP in the English literature. We conducted a systemic review of the literature of CGEP published in English or Chinese, and found that CGEP occurred more frequently in the 6th and 7th decade of life, with a male predominance (89.7%). The majority of reported cases (89.2%) were from East Asia, and the underlying mechanism remains unknown. Most…

Laparoscopic pylorus-preserving pancreaticoduodenectomy (PPPD)/pancreaticoduodenectomy (PD) is cautiously regarded as a safe and effective approach in well-selected patients with periampullary cancer. However, postoperative pancreatic fistula (POPF), delayed gastric emptying (DGE), postoperative bleeding, and sepsis and detrimental complications that can follow PPPD/PD. These complications can result in poor oral intake for a significant period of nothing per oral (NPO) or deterioration of long-term function. A 65-year-old man underwent laparoscopic PPPD for ampulla of vater (AoV) cancer. Afte…

PURPOSE: This study evaluates the effect of an enhanced recovery after surgery (ERAS)-based nutrition support protocol on oral intake and weight change in patients who underwent pylorus-preserving pancreaticoduodenectomy (PPPD).
METHODS: A 14-day postoperative nutrition support protocol was developed to initiate oral intake after 1 week of enteral tube feeding and parenteral nutrition (early oral feeding, EOF). Forty-eight patients who underwent PPPD participated in the study (non-EOF, n = 23; EOF, n = 25). General information, nutrition supply route and amount, blood chemistry, and weight ch…

To clarify the basis for postprandial abdominal fullness(PAF)in patients after pylorus-preserving gastrectomy(PPG), the authors investigated the relationship of PAF with postgastrectomy disorder(PGD)and gastric emptying function(GEF)in PPG patients. A total of 22 patients(14 men and 8 women, average age 64.8 years)were divided into 2 groups[Group A, PAF-positive(n=12); Group B, PAF-negative(n=10)]at 1 year after PPG for early gastric cancer. The relationships of PAF with PGD and GEF were examined. Length of the antral cuff(LAC)was significantly shorter in group A than in group B(p< 0.05). Appe…

Tracking pylorus in ultrasonic image sequences is an important step in the analysis of duodenogastric reflux (DGR). We propose a joint prediction and segmentation method (JPS) which combines optical flow with active contour to track pylorus. The goal of the proposed method is to improve the pyloric tracking accuracy by taking account of not only the connection information among edge points but also the spatio-temporal information among consecutive frames. The proposed method is compared with other four tracking methods by using both synthetic and real ultrasonic image sequences. Several numeri…

BACKGROUND/AIMS: Pancreatic head resection is performed with low mortality, but morbidity remains high. Extensive preparation, long operating times, intraoperative blood loss and the need for blood transfusions are risk factors for postoperative morbidity. The aim of our study was to evaluate the feasibility and safety of the ultrasonic dissection device in pylorus-preserving duodenopancreatectomy (PPPD).
METHODS: Fifty consecutive patients who underwent PPPD with an ultrasonic dissection device (group 1) were compared with a match-controlled group of 50 consecutive patients who underwent PPP…

Acquired double pylorus (DP) is an uncommon condition consisting of two communicating channels between the gastric antrum and the first part of duodedum. Little is known about the origin of DP. As there is no specific gastrointestinal symptom due to DP, most often it is diagnosed by gastroscopy while performing for other indicationsFew data are also known about the clinical course of DP. In the patients with peptic ulcer symptoms, the pyloroplasty-like drainage effect, improving gastric emptying after the estabilishment of the fistula, could relieve these symptoms. This represents an unresolve…

BACKGROUND/AIMS: The anastomosis performed during laparoscopy-assisted pylorus-preserving gastrectomy (PPG) is usually handsewn; however, the technique is sometimes difficult because it is done through a small incision and the level of skill of each operator may vary. The present study describes a triangulating stapling technique used during an anastomosis in laparoscopy-assisted pylorus-preserving gastrectomy.
METHOD: The 80-mm stapler was used 3 times for the triangulating stapling technique. The posterior walls, including the stay sutures, were joined by firing the stapler, and the anterio…

INTRODUCTION: Pancreaticoduodenectomy is one of the most challenging procedures performed by general surgeons.
DISCUSSION: Many studies have been performed looking at the technical aspects of reconstruction after pancreaticoduodenal resection. Multiple randomized trials have failed to convincingly demonstrate the superiority of any single approach or technique. Here, we illustrate our approach to reconstruction, with an emphasis on technical aspects and details.
CONCLUSION: The fine points can help avoid technical errors that result in anastomotic failure.
