Open-Irrigated Helical Radiofrequency Septal Ablation for Obstructive Hypertrophic Cardiomyopathy.

BACKGROUND: Septal reduction therapy is recommended for symptomatic obstructive hypertrophic cardiomyopathy (oHCM) refractory to guideline-directed medical therapy. CASE SUMMARY: A 60-year-old man with drug-refractory oHCM underwent Percutaneous Helical Ablation of Septal hypErtrophy, using a novel, dedicated, open-irrigated helical radiofrequency catheter via subclavian venous access. The provoked left ventricular outflow tract gradient decreased immediately after ablation from 130 to 30 mm Hg, with sustained improvement at 12 months, septal fibrosis on 6-month cardiac magnetic resonance, sy

Ultrasound-Guided Percutaneous Radiofrequency Thermal Neuroablation for the Treatment of Adductor and Rectus Femoris Spasticity.

Spasticity is a very frequent complication of spinal cord injury (SCI) that can limit activities of daily living, reduce the quality of life, and augment caregiver burden. This problem has many treatment options that should be selected according to the clinical and functional scenario.  This case study presents a 60-year-old female patient with complete spastic paraplegia after a spinal stroke. Spasticity interfered with activities of daily living, mainly with intermittent catheterization and transfers, and botulinum toxin injections failed to efficiently treat this issue. It was decided to pe

Pulsed radiofrequency treatment of articular branches of the obturator and femoral nerves for management of hip joint pain.

Groin and thigh pain are frequently the major symptoms of hip joint pathology. The hip joint is innervated by articular branches of the obturator, femoral, superior gluteal, and sciatic nerves. The nerve responsible for hip joint pain can be determined by a diagnostic nerve block. Radiofrequency ablation of the identified articular branches of the hip was demonstrated to provide relief of hip pain. However, continuous radiofrequency denervation by thermal coagulation carries the potential risk of neuritis. We report on two patients with groin and thigh pain related to hip joint pathology treat

Photoepilation: a growing trend in laser-assisted cosmetic dermatology.

Excess hair is an age-old condition plaguing both men and women alike, of all races. Conditions such as hirsutism or hypertrichosis, procedures that involve grafted donor sites, transsexual transformations from male to female, and genetics are all responsible for excess or unwanted hair. Previous options for people seeking to remove or lessen the presence of hair have either been painful or resulted in short-term hair removal. With the recent advent of laser technology, hair removal has been added to the many capabilities of the new generation nonablative lasers and light systems. Lasers are n

Enhancing the therapeutic impact of sublethal radiofrequency hyperthermia in malignant solid tumor treatment.

Radiofrequency ablation (RFA) is an effective alternative to surgery for managing some malignant solid tumors. However, for medium-to-large tumors (>3 cm), tumors adjacent to large blood vessels, and certain irregular tumors, sublethal radiofrequency hyperthermia (RFH) often produces a margin of ablated tumor owing to the "heat-sink" effect. This effect typically leaves behind viable residual tumors at the margin. Several studies have reported that a sublethal RFH can significantly enhance the efficacy of chemotherapy, radiotherapy, immunotherapy, and gene therapy for malignant solid tumors. T

Performance Assessment of a Radiofrequency Powered Guidewire for Crossing Peripheral Arterial Occlusions Based on Lesion Morphology.

Endovascular wires and devices for peripheral arterial disease therapy have evolved greatly, yet failure rates of these procedures remain high. Information on lesion composition may inform device selection to improve the success rates of these procedures. This paper, presents an approach for informed guidewire selection. The objective of this study is to quantitatively assess the performance of a radiofrequency powered guidewire in the crossing of various morphology types of peripheral chronic total occlusions. Samples taken from amputated patient limbs are characterized by magnetic resonance

Stellate ganglion block therapy in management of ventricular electrical storm: A case report.

Sympathetic overactivity is a recognized underlying mechanism contributing to the pathogenesis of ventricular electrical storm (VES). The growing body of evidence supports the efficacy of stellate ganglion block (SGB) in attenuating myocardial sympathetic tone, rendering it a valuable adjunctive therapy for managing VES. This case report presents the clinical details of a 60-year-old patient admitted for ventricular tachycardia (VT), necessitating the implantation of an implantable cardioverter defibrillator (ICD) to mitigate the risk of fatal ventricular arrhythmias (VAs). Subsequently, the p

Endoscopic ablation therapy for biliopancreatic malignancies.

Biliopancreatic malignancies such as cholangiocarcinoma (CCA) has notoriously been diagnosed late. As such most therapy have been palliative in nature. Cholangioscopy allows for an earlier diagnosis to be made. Brachytherapy with the insertion of catheter with iridium-132 seeds, percutaneously or through endoscopic retrograde cholangiopancreatography (ERCP) was the earliest ablative techniques used. It has been shown to have a beneficial effect only in prolonging survival. Photodynamic therapy (PDT) has also been used for several years. stenting with PDT versus stenting alone for unresectable

Combination of interventional therapies in hepatocellular carcinoma.

Many interventional techniques aimed at achieving nonsurgical ablation of hepatocellular carcinoma have been developed and clinically tested over the last decade. Percutaneous image-guided therapies such as ethanol injection and radiofrequency thermal ablation provide an effective means for treating hepatocellular carcinoma lesions smaller than 3 cm, but do not ensure successful ablation of larger tumors. In view of the limitations of available interventional therapies, there is currently a focus on a multimodality strategy for the treatment of large hepatocellular carcinomas. Combination of t

Chemical Ablation of Ventricular Tachycardia Using Coronary Arterial and Venous Systems.

Radiofrequency catheter ablation (RFCA) is the first-line therapy for treatment of drug refractory ventricular arrhythmias (VAs), however, creating a safe, transmural lesion can be difficult. Ethanol in the arterial system has been used as an adjunctive therapy to RFCA since 1986, but with limited use due to technical and efficacy limitations. Venous ethanol is emerging as powerful alternative.

Treatment of meralgia paresthetica with ultrasound-guided pulsed radiofrequency ablation of the lateral femoral cutaneous nerve.

A 23-year-old female with an 18-month history of left anterolateral thigh paresthesias and burning pain consistent with meralgia paresthetica was referred to our clinic after failing trials of physical therapy, nonsteroidal anti-inflammatories, gabapentin, and amitriptyline. We performed 3 lateral femoral cutaneous nerve blocks with corticosteroid over a 4-month period; however, each block provided only temporary relief. As this pain was limiting the patient's ability to perform her functions as an active duty service member, we elected to perform a pulsed radiofrequency treatment of the later
