Two case reports examine a little-known cause of dental pain and sensitivity to percussion. Contrary to the traditional assumption that pain and sensitivity to percussion almost always are diagnostic of pulpal inflammation and/or necrosis, these symptoms actually may be referred to the sensitive tooth from trigger points in the masticatory muscles. Therefore, myofascial pain syndrome must be ruled out in patients who have dental pain and display sensitivity to percussion.

Neck pain is a common condition with a high prevalence worldwide. Neck pain is associated with significant levels of disability and is widely considered an important public health problem. Neck pain is defined as pain perceived between the superior nuchal line and the spinous process of the first thoracic vertebra. In some types of neck conditions, the pain can be referred to the head, trunk and upper limbs. This article aims to provide an overview of the available evidence on prevalence, costs, diagnosis, prognosis, risk factors, prevention and management of patients with neck pain.

BACKGROUND: Back pain is a major health problem and the most important disorder associated with sickness absence. This report presents experiences from a back pain outpatient clinic.
MATERIAL AND METHODS: We collected demographic data, diagnoses on the referrals (ICPC) and in the outpatient clinic (ICD-9) and recommendations given to 206 patients, referred 1 January to 30 April 1998.

Myofascial pain (MFP) is a regional muscle pain disorder characterized by localized tenderness in taut muscle bands and referred pain. Frequently, MFP is overlooked as a common cause of chronic pain because of the frequent association with joint dysfunction and other pain disorders and the multiple behavioral and psychosocial contributing factors that are often present. Nonetheless, studies have reported that MFP is present in a significant number of people. This article describes current concepts for the diagnosis and management of MFP.

Ear pain is a diagnostic problem when examination of the ear shows no pathology. Referred otalgia may be caused by neoplasms; dental abnormalities or infections; temporomandibular joint dysfunction; sinus, pharyngeal or salivary gland infections; temporal arteritis; cervical arthritis, or one of the neuralgias. Ear pain may also be psychogenic.

Mastalgia (breast pain) is the commonest breast problem women present with in general practice. Mastalgia can be apparent, when due to referred pain or costochondritis, for instance; or real, when it can be localised or diffused. Diffused pain can be cyclical or non cyclical. An important association of cyclical mastalgia is benign mammary dysplasia (fibroadenosis). It is important to exclude malignancy and to reassure patients.

Three patients with lung cancer and shoulder pain for which no local cause could be found are described. All three benefited from a course of palliative radiotherapy to ipsilateral mediastinal disease remote from the site of the pain. It is suggested that the pain is referred from intrathoracic involvement of the phrenic nerve by cancer, and that palliative irradiation of the mediastinum should be considered if investigations fail to reveal a local cause for ipsilateral shoulder pain.
