Epidemiological investigative report on ocular morbidity in children in rural Kenya.

AIM: To ascertain the pattern of ocular morbidity in a population of primary school children in rural Kenya as it is a prerequisite for planning effective preventive and therapeutic strategies. METHODS: A cross-sectional survey of ocular symptoms and clinical eye examinations were performed in a sample of 35 rural primary schools in the semi-arid region of Kajiado West sub-county in S.W. Kenya, amongst a semi-nomadic tribe (Maasai). Students in Grades 1-8 were included. Visual acuity was measured using the Snellen "tumbling E" chart at 6 m. Children with symptoms of refractive error underwent

Updates on Refractive Surgery: Benefits, Risks, and Costs of Modern Treatment Options.

PURPOSE OF REVIEW: To provide updates on modern refractive procedures including benefits, risks, costs, and clinical considerations. RECENT FINDINGS: Refractive surgery demonstrates excellent safety and efficacy in appropriately selected patients. Recent studies demonstrate differences between cornea- and lens-based procedures in postoperative visual recovery, refractive predictability, ocular surface impact, side effect profiles, enhancement strategies, and cost-efficiency. These distinctions highlight the importance of careful patient selection and screening across the wide range of refract

Corneal Refractive Surgery Considerations in Patients with Cystic Fibrosis and Cystic Fibrosis Transmembrane Conductance Regulator-Related Disorders.

This article discusses common ocular manifestations of cystic fibrosis (CF) and cystic fibrosis transmembrane conductance regulator-related disorders (CFTR-RD). A structured approach for assessing and treating patients with CF/CFTR-RD seeking corneal refractive surgery is proposed, as well as a novel surgical risk scoring system. We also report two patients with various manifestations of CFTR dysfunction who presented for refractive surgery and the outcomes of the procedures. Surgeons seeking to perform refractive surgery on patients with CF/CFTR-RD should be aware of mild to severe clinical m

Ocular dimensions by three-dimensional magnetic resonance imaging in emmetropic versus myopic school children.

BACKGROUND: Magnetic resonance imaging (MRI) has been used to investigate eye shapes; however, reports involving children are scarce. This study aimed to determine ocular dimensions, and their correlations with refractive error, using three-dimensional MRI in emmetropic versus myopic children. METHODS: Healthy school children aged < 10 years were invited to take part in this cross-sectional study. Refraction and best-corrected distance visual acuity (BCDVA) were determined using cycloplegic refraction and a logarithm of the minimum angle of resolution (logMAR) chart, respectively. All childre

Ocular outcomes and comorbidities in preterm infants enrolled for retinopathy of prematurity screening: A cohort study from western India.

BACKGROUND: Retinopathy of prematurity (ROP) is emerging as an important cause of childhood blindness in middle-income countries such as India. Although blindness can be prevented in most cases with timely screening and treatment, certain ocular comorbidities can lead to visual impairment. We evaluated and compared 1-year visual, refractive, and structural outcomes and comorbidities in three subsets of preterm infants enrolled for screening of ROP. SUBJECTS AND METHODS: Preterm children enrolled in the hospital's ROP screening program and with diagnosis of no ROP, mild ROP, or severe ROP were

A review of the changes in the ophthalmic and visual system in pregnancy.

Pregnancy is often associated with ocular changes which may be more commonly transient but occasionally, permanent. It may be associated with the development of new ocular conditions, or can exacerbate pre-existing conditions. The ocular effects of pregnancy may be physiological or pathological or may be modifications of pre-existing conditions. Adnexial changes include chloasma, spider angiomas and ptosis. Anterior segment changes include a decrease in conjunctival capillaries and an increase in the granularity of conjunctival venules and in corneal curvature, changes in corneal thickness, re

Diagnostic accuracy of the Parr vision test, single crowded Lea symbols and Spot vision screener for vision screening of preschool children aged 4-5 years in Aotearoa/New Zealand.

PURPOSE: Preschool children in New Zealand undergo vision screening to detect amblyopia at 4-5 years of age. The current test, the Parr vision test, does not meet international visual acuity chart guidelines and has not been validated against other commonly used paediatric vision tests. New Zealand vision screening protocols are also not targeted for detecting other eye conditions such as uncorrected refractive error, which may affect school performance. We compared the Parr vision test with the single crowded Lea symbols and the Spot vision screener for detecting ocular pathology, refractive

Update on contraindications for laser-assisted in situ keratomileusis and photorefractive keratectomy.

PURPOSE OF REVIEW: Laser refractive surgery (LRS) is one of the most common elective procedures performed in the USA today. Patients should be rigorously screened for certain ocular and systemic conditions, which may represent contraindications to LRS. Before treating, the refractive surgeon should be aware of the Food and Drug Administration (FDA) labeling for the procedure as well as what other evidence, such as clinical studies, case series, cases reports, and anecdotal evidence, exists. The purpose of this article is to review contraindications to having LRS. RECENT FINDINGS: Ocular contr

A Comparative Study to Assess the Accommodation and Vergence Relationship of Myopia in Indian Adolescent.

BACKGROUND: Accommodation and Vergence disorder are diverse visual anomalies which can interfere with a child's school performance and impair one's ability to function efficiently. Its association with refractive error and its intervention were studied less in Indian myopia children; hence, there is a need for research in such setting. METHOD: One hundred and fifty Indian adolescents aged 10 to 17 years were divided into three refractive error groups (high, moderate, and low myopia). Baseline vision examination and a comprehensive binocular vision assessment were performed on all eligible ado

Optical performance of progressive addition lenses (PALs) with astigmatic prescription.

The progressive addition lens (PAL) is a spectacle lens design with progressive refractive power changes across the lens surface to provide sharp vision at different viewing distances for patients with reduced accommodative strength. It has gained in popularity not just for presbyopic patients, but also patients with occupational (office, driving, or digital device) and therapeutic (e.g., myopia control) needs. However, despite the increasing prevalence of astigmatism in adults > 40 years old who rely on PAL correction, no metric is available to reflect the optical variation in PALs with astig

Variations in accommodation and convergence responses in a minimally controlled photorefractive setting.

PURPOSE: A remote haploscopic photorefractor, designed for assessment of accommodation and convergence in infants and clinical groups, was used to determine heterophoria accommodative convergence/accommodation (AC/A) ratios in normal naïve adults. These were compared with conventional clinical measures. METHODS: Twenty-one naïve subjects were used to compare occluded and unoccluded prism cover test responses with the remote haploscopic photorefractor using a text and picture target. RESULTS: Although luminance was generally low for both targets, binocular vergences were appropriate for targe

The effect of prism adaptation on the response AC/A ratio.

Vergence adaptation, also known as prism adaptation, is a phenomenon in which a patient's heterophoria changes after prolonged viewing through prism. The effect of prism adaptation on the accommodation-convergence relationship, quantified by the AC/A ratio, is not known. Previous studies of AC/A ratio stability and alterability have used only stimulus AC/A ratio calculations, or have measured accommodative responses to only one or two stimuli. The ideal study of AC/A ratio stability and alterability would measure accommodative responses to several accommodative stimuli, and use these along wit

Tear Proteases and Protease Inhibitors: Potential Biomarkers and Disease Drivers in Ocular Surface Disease.

Tears are highly concentrated in proteins relative to other biofluids, and a notable fraction of tear proteins are proteases and protease inhibitors. These components are present in a delicate equilibrium that maintains ocular surface homeostasis in response to physiological and temporal cues. Dysregulation of the activity of protease and protease inhibitors in tears occurs in ocular surface diseases including dry eye and infection, and ocular surface conditions including wound healing after refractive surgery and contact lens (CL) wear. Measurement of these changes can provide general informa
