[Increased frequency and spread of restlessness as the early manifestation of Parkinson's disease in a woman with restless legs syndrome].

A 57-year-old woman had been suffered from insomnia due to restlessness and abnormal sensation of the left side of the body for 33 years. Since the preceding year of the first visit frequency of the symptoms increased, and the abnormal sensation was spread to the right leg. Her daughter had restless legs syndrome (RLS) since age 20. Neurological examination showed no abnormality. Laboratory test results showed normal ferritin levels. There was no renal dysfunction or anemia. A diagnosis of RLS was made, and her symptoms responded well to pramipexole treatment. However, the patient developed ri…

Leg movement activity during sleep in multiple sclerosis with and without restless legs syndrome.

STUDY OBJECTIVES: To carry out an analysis of leg movement activity during sleep in a polysomnography dataset of patients with multiple sclerosis (MS) in comparison to idiopathic restless legs syndrome (iRLS) and healthy controls. METHODS: In this cross-sectional, observational, instrumental study, 57 patients (males/females: 11/46; mean age 46.2 ± 10.2 years) with a diagnosis of MS underwent a telephone interview assessing the 5 standard diagnostic criteria for RLS and polysomnography. Sleep architecture and leg movement activity during sleep were subsequently compared: 1) 40 patients with M…

Restless legs syndrome and periodic leg movements in patients with movement disorders: Specific considerations.

Restless legs syndrome is a frequent neurological disorder with potentially serious and highly distressing treatment complications. The role and potential implications of periodic leg movements during sleep range from being a genetic risk marker for restless legs syndrome to being a cardiovascular risk factor. The diagnosis of restless legs syndrome in patients with daytime movement disorders is challenging and restless legs syndrome needs to be differentiated from other sleep-related movement disorders. This article provides an update on the diagnosis of restless legs syndrome as an independe…

Effects of gabapentin enacarbil on restless legs syndrome and leg pain in dementia with Lewy bodies.

Restless legs syndrome (RLS) is a common neurological disorder. Dementia with Lewy bodies (DLB) is the second most common form of neurodegenerative dementia after Alzheimer's disease. Both RLS and DLB can be effectively treated by dopaminergic medications, suggesting the role of dopamine dysfunction in the pathogenesis of both diseases. Here, I report on a Japanese woman with probable DLB and RLS who was treated with gabapentin enacarbil, a non-dopaminergic agent. Because a dopamine agonist, a first-line therapy for moderate to severe RLS, caused the occurrence of metamorphopsia, an alternativ…

Periodic leg movements during sleep in narcoleptic patients with or without restless legs syndrome.

We compared periodic and non-periodic leg movements during sleep and polysomnography in patients with narcolepsy with cataplexy (NC) with or without restless legs syndrome (RLS) with matched idiopathic RLS (iRLS) and control subjects. We enrolled 100 patients with NC: 17 having RLS were compared with 34 sex- and age-matched patients without RLS and with 17 normal controls and 17 iRLS subjects. Periodic leg movements were highest in iRLS and lowest in controls, with those in NC with RLS very close to iRLS, but higher than those in NC without RLS. The periodicity indexes showed the highest value…

Risk of Parkinson Disease Among Patients With Restless Leg Syndrome.

IMPORTANCE: The association between restless leg syndrome (RLS) and Parkinson disease (PD) remains unclear. Clarifying this association and the role of the dopaminergic pathway may improve understanding of the pathophysiology between these 2 diseases. OBJECTIVES: To assess whether RLS is a risk factor for developing PD and whether the dopamine pathway is meaningfully associated with RLS and PD. DESIGN, SETTING, AND PARTICIPANTS: This retrospective cohort study used data from the Korean National Health Insurance Service Sample Cohort from 2002 to 2019. Statistical analyses were performed betw

Restless Leg Syndrome and Association with Serum NT Pro-CNP.

INTRODUCTION: Restless leg syndrome (RLS) pathophysiology is yet unclear. The role of iron deficiency, dysfunction of the dopaminergic system, and hypoxia are well-known mechanisms related to pathophysiology. C-type natriuretic peptide (CNP) is a neuropeptide that is released from the cerebral structures and endothelium. In current literature, serum concentrations of NT pro-CNP were correlated to concentrations of the central nervous system. In this study, we aimed to investigate the relationship between serum NT pro-CNP and other biochemical markers with RLS. MATERIALS AND METHODS: A total o

Prevalence and correlates of restless leg syndrome in psychiatric outpatients in Lebanon.

Restless Legs Syndrome (RLS) is a prevalent yet underdiagnosed neurological disorder that significantly affects quality of life. It is commonly associated with psychiatric conditions, particularly depression and anxiety. Epidemiological data on RLS prevalence among psychiatric populations are limited and virtually absent in Lebanon. This study estimated RLS prevalence among psychiatric outpatients and assessed its impact on sleep quality, daytime sleepiness, anxiety, and depression, comparing findings to international literature. In this cross-sectional study, adult psychiatric outpatients wer…

Restless leg syndrome in spinal cord injury: case report.

CASE DESCRIPTION: Restless leg syndrome (RLS) is a condition infrequently reported in spinal cord injury that causes an uncomfortable sensation in the legs and an urge to move them. We report a case involving a 63-year-old man with incomplete paraplegia with an onset of RLS four years post injury. FINDINGS: Based upon history, pramipexole was prescribed for the presumptive diagnosis of RLS, with good effect. Initial workup revealed an anemia (hemoglobin of 9.3 gram/deciliter (g/dl)) and iron deficiency (ferritin of 10 microgram/liter (μg/L)), necessitating further evaluation. CONCLUSION: Due

Restless leg syndrome in the differential diagnosis of entrapment and peripheral neuropathies.

Restless leg syndrome (RLS) is an abnormal sensation disorder. Defining the syndrome is difficult. It is transmitted autosomal dominant genetically, is especially prevalent in the lower limbs, and is seen in both genders. In the differential diagnosis of RLS, nocturnal leg cramps, akathisia, peripheral neuropathy, entrapment neuropathy, and vascular disease (for example, deep vein thrombosis) should be considered. A 52-year-old woman was admitted to our clinic with signs of paresthesia, she had abnormal sensation disorder in both legs and the right arm, which she had difficulty defining. She h

The genetics of sleep disorders in humans: narcolepsy, restless legs syndrome, and obstructive sleep apnea syndrome.

Sleep disorders are a group of neurological disorders known to cause public health problems associated with interference with daily activities including cognitive problems, poor job performance and reduced productivity. There is strong evidence emerging for the presence of genes influencing sleep disorders, such as narcolepsy (NRCLP), restless legs syndrome (RLS), and obstructive sleep apnea syndrome (OSAS). NRCLP is typically characterized by excessive daytime sleepiness, cataplexy, sleep paralysis and hallucinations. RLS is manifested by compelling need to move the legs and usually experienc…

Selected sleep disorders: restless legs syndrome and periodic limb movement disorder, sleep apnea syndrome, and narcolepsy.

Sleep disorders, including restless legs syndrome and periodic limb movement disorder, sleep apnea syndrome, and narcolepsy, are prevalent medical conditions, likely to be seen by practicing psychiatrists. Awareness of these conditions and their presentations, pathophysiology, and treatment allows psychiatrists to treat these conditions where appropriate, to minimize complications and health consequences associated with delayed diagnosis, and to reduce the burden of disease that these conditions may place on patients already experiencing primary psychiatric disorders.

Serum Biomarkers in Restless Legs Syndrome: Beyond the Classical Iron Paradigm-A Scoping Review.

Restless legs syndrome (RLS) is one of the most prevalent sleep disorders, yet its diagnosis continues to rely almost entirely on subjective symptom descriptions. This persistent dependence on phenomenology reflects the absence of reliable biological markers to aid in the process of diagnosis or monitoring. However, there is accumulating molecular evidence that suggests that RLS is associated with systemic biological alterations. These extend beyond the traditional paradigm of iron deficiency. The present scoping review synthesizes the current research on circulating serum biomarkers investiga…

Treatment of restless legs syndrome by acupuncture combined with medicine based on pathophysiological mechanism.

Restless legs syndrome (RLS) is a neurological disorder characterized by bilateral discomfort in the lower extremities, intense urge to move, and exacerbation of symptoms during the night. Its pathogenesis is closely associated with dysfunctions in the dopaminergic system, abnormal iron metabolism, and genetic predisposition. RLS frequently co-occurs with conditions, such as chronic kidney disease and iron deficiency anemia, significantly impairing patients' sleep quality and daytime functioning. This review systematically examines the definition, classification, epidemiological features, and …
