The role of fibronectin in aqueous humor and its association with retinal and choroidal vascular parameters in branch retinal vein occlusion.

PURPOSE: To evaluate the relationship between fibronectin concentration in the aqueous humor and quantitative imaging parameters in patients with branch retinal vein occlusion (BRVO). METHODS: This retrospective study included 40 patients with BRVO treated for BRVO macular edema. Aqueous humor samples were collected during intravitreal injection of anti-vascular endothelial growth factor (VEGF) or steroids. Fibronectin concentration was measured using an enzyme-linked immunosorbent assay (ELISA) kit. Parameters included central macular thickness (CMT), choroidal thickness (CT), choroidal vasc…

Current Understanding of Retinal Vein Thrombosis in Older Adults: A Review of the Literature.

BACKGROUND: Retinal vein thrombosis (RVT) represents a global public health problem, ranking as the second position in prevalence among all retinal vascular pathologies and becoming an "epidemic" with the advanced age of patients. The prevalence of this pathology increases 20 times in patients aged over 80 years, compared to the age of 40-49 years. This pathology can lead to visual impairment, blindness and causes disability, but it is reversible within the first 7 days with timely diagnosis and treatment. The response to anti-VEGF treatment is better with earlier initiation. VEGF suppression …

Faricimab for retinal vein occlusion: a review of current evidence and future perspectives.

Retinal vein occlusion (RVO) is the second most common retinal vascular cause of vision loss after diabetic retinopathy and can lead to substantial visual impairment due to retinal ischemia, hemorrhage, vascular leakage, and macular edema. Intravitreal anti-vascular endothelial growth factor (VEGF) therapy is currently the first-line treatment for RVO-associated macular edema, with its efficacy confirmed by numerous large-scale randomized controlled trials. However, VEGF is not the only pathological driver in RVO. Angiopoietin-2 (Ang-2) has emerged as a key factor that contributes to disease p…

Retinal Vein Occlusion in a Patient on Dabrafenib and Trametinib Therapy for Metastatic Melanoma.

The purpose of this case report is to highlight the ocular complications of dabrafenib and trametinib treatment. We discuss the case of an 81-year-old female treated with dabrafenib and trametinib for metastatic melanoma, who developed a retinal branch vein occlusion with macular edema in the right eye. The other eye was healthy. The treatment was discontinued and her macular edema was managed with a loading dose of three injections of anti-vascular endothelial growth factor (anti-VEGF) medication with a good response. The use of BRAF and MEK inhibitors is increasingly becoming widespread, and

Status of Choriocapillaris in Fellow Eyes of Patients With Unilateral Retinal Vein Occlusions.

BACKGROUND AND OBJECTIVE: To study the status of the choriocapillaris in fellow eyes of patients with unilateral retinal vein occlusions (RVOs). PATIENTS AND METHODS: Thirty-two healthy eyes of patients with unilateral RVO and 16 eyes of healthy age-matched controls were included. Choriocapillaris flow voids and device-specific choriocapillaris total flow areas were quantified based on 3-mm optical coherence tomography angiography (OCTA) scans and their correlation with the number of resolved para-central acute middle maculopathy (PAMM) lesions on 6-mm OCTA scans was calculated. RESULTS: In …

Retinal vein occlusion with cerebral infarction in a preterm neonate: a case report.

BACKGROUND: Retinal vein occlusion (RVO) is a common disease that causes blindness in elderly patients, and cerebral infarction is also a severe disorder impairing the health of individuals. Both diseases are not common in neonates and are related to thrombosis. To date, only one case of simultaneous occurrence of RVO with intracranial haemorrhage in a full-term neonate has been reported. CASE PRESENTATION: A preterm neonate was diagnosed with cerebral infarction and RVO. Retinal haemorrhage and macular oedema were detected in the left eye after the onset of ipsilateral stroke. Although the r…

Update on the Use of Anti-VEGF Intravitreal Therapies for Retinal Vein Occlusions.

The use of anti-vascular endothelial growth factor (VEGF) therapy in ophthalmology has profoundly changed our management and treatment of conditions such as cystoid macular edema, diabetic macular edema, choroidal neovascularization, and other proliferative retinopathies. Although initially used for the treatment of choroidal neovascularization in neovascular age-related macular degeneration, their application has spread rapidly for other indications as their outcomes have often outperformed previously existing treatments. Retinal vein occlusion (RVO) continues to be one of the leading causes …

Spontaneous remodelling of retinal veins accidentally severed during vitreous surgery.

BACKGROUND: To report the spontaneous remodelling of retinal veins inadvertently severed during vitreous surgery. METHODS: Four diabetic patients who underwent vitrectomy during which retinal veins were damaged were followed for at least 6 months. Haemorrhages from severed veins were controlled by raising intraocular pressure and by endodiathermy of the distal ends of the veins. This was followed by fluid/gas exchange. RESULTS: Remodelling of venous channels in the damaged areas was observed in all eyes. In three of the four eyes we found evidence of collateral veins. In the other eye we not…

[Experimental retinal vein obstruction induced by transadventitial administration of thrombin in the rabbit].

Retinal venous obstruction with typical flame-shaped hemorrhage was experimentally produced in the rabbit by transadventitial dropping of thrombin on target vessels by vitreous surgery techniques. The changes were studied ophthalmoscopically, light and electron microscopically. Flame-shaped retinal hemorrhage appeared within 24hr after the maneuver of thrombin dropping, following the initial appearance of small hemorrhage during the first 8 to 12hr of the experiment. Microscopic study revealed the process of subendothelial fibrin-thrombus formation in the target venules. Thrombus formation beg

Combined Central Retinal Artery Occlusion (CRAO) and Central Retinal Vein Occlusion (CRVO) in a Celiac Disease Patient: A Case Report.

Celiac disease (CD) is a digestive disorder caused by an abnormal immune reaction to gluten, leading to severe malabsorption syndrome. Central retinal vein occlusion (CRVO) was reported in a couple of cases worldwide in patients with this disease entity. Herein, we are reporting a rare case of combined central retinal vein and artery occlusion in a young female celiac disease patient presented with a counting finger vision at six feet and improved to 20/60 vision after conservative management.

Anti-VEGF Therapy and Retinal Photocoagulation to Prevent Recurrence of Central Retinal Vein Occlusion: Two Case Reports of Young Patients.

Case 1: An 18-year-old man. On initial examination, he was diagnosed with central retinal vein occlusion (CRVO) due to optic papillitis. He had no previous systemic and ocular medical history. His best corrected visual acuity was 20/20 for the right eye, but macular edema accompanied by serous retinal detachment was observed about 2 months after the initial examination. Intravitreal anti-VEGF injection was performed, and the symptoms improved. Then, additional photocoagulation was applied to the retinal nonperfusion area, to maintain its normal state. Case 2: A 36-year-old man. He was diagnose…

Branch retinal artery occlusion and non-ischemic central retinal vein occlusion due to hyperhomocysteinemia in a 14-year-old child.

A 14-year-old girl presented with sudden, painless loss of vision in the left eye. Complete ophthalmologic examination including fluorescein angiography revealed an impending central vein occlusion and a branch retinal artery occlusion inferotemporally. One month later, there was a non-ischemic central retinal vein occlusion of the same eye. Systemic evaluation led to the diagnosis of hyperhomocysteinemia. This case report underscores the importance of excluding hyperhomocysteinemia in vascular occlusive disease.
