Carotid endarterectomy--an evidence-based review [RETIRED]: report of the Therapeutics and Technology Assessment Subcommittee of the American Academy of Neurology.

OBJECTIVE: To assess the efficacy of carotid endarterectomy for stroke prevention in asymptomatic and symptomatic patients with internal carotid artery stenosis. Additional clinical scenarios, such as use of endarterectomy combined with cardiac surgery, are also reviewed. METHODS: The authors selected nine important clinical questions. A systematic search was performed for articles from 1990 (the year of the last statement) until 2001. Additional articles from 2002 through 2004 were included using prespecified criteria. Two reviewers also screened for other relevant articles from 2002 to 2004

Body roundness index and the incidence of kidney disease: A prospective cohort study from the China Health and Retirement Longitudinal Study.

Kidney disease (KD) poses a growing global health burden, with obesity contributing to its development. The body roundness index (BRI), a novel anthropometric measure reflecting visceral adiposity, may better predict KD risk than body mass index, but longitudinal evidence is limited. In this prospective cohort study, we analyzed 8401 adults aged ≥ 45 years from the China Health and Retirement Longitudinal Study, free of KD at baseline. BRI was calculated from waist circumference and height. Incident KD was ascertained via physician-diagnosed questionnaires during 4 follow-ups (2013-2020). Cox …

The association between cystatin C and hip fracture risk: A longitudinal cohort study of China Health and Retirement Longitudinal Study (CHARLS).

PURPOSE: Cystatin C, a cysteine protease inhibitor linked to kidney function, cardiovascular disease, and osteoporosis, has unclear associations with hip fracture risk. This study examines this relationship using longitudinal data from the China Health and Retirement Longitudinal Study (CHARLS). METHODS: We conducted a prospective cohort analysis using five waves of CHARLS data (2011-2020) among participants aged ≥60 years. Serum cystatin C concentration served as the primary exposure variable, with incident hip fracture as the outcome measure. Cox proportional hazards models quantified the a…

Qualitatively exploring opportunities and challenges for sexually transmitted infection prevention aimed at Canadian international retirement migrants wintering in the United States.

BACKGROUND: Older populations globally are experiencing a rise in sexually transmitted infections, and those who travel internationally and engage in unsafe sex practices are particularly at risk. International retirement migrants, who live abroad for extended periods each year, pose unique challenges for public health efforts to prevent, track, control, and treat these infections. To date, no research has examined the specific opportunities and challenges for sexually transmitted infection prevention among Canadian international retirement migrants who winter in the United States. This study …

The link between dental care use and incident diabetes, cancer, and Alzheimer disease among older Americans: the health and retirement study.

Oral health is crucial for overall well-being, especially in older adults at higher risk for chronic diseases. This study examines the link between regular dental care and 10-year incidence of major conditions-Alzheimer's, dementia, stroke, diabetes, cancer-using data from the Health and Retirement Study (HRS), a representative survey of older Americans. Data from 15,320 dementia‑free participants aged 50 years and older interviewed in 2008 were analyzed. Regular dental visits in the previous 2 years were the primary exposure, and a composite dental condition measure (poor versus good, combini…

Depressive symptom trajectories around incident hip fracture in older adults: an event-centered matched analysis from the Health and Retirement Study.

OBJECTIVE: To examine depressive symptom trajectories around incident hip fracture and determine whether excess burden reflects pre-fracture worsening, peri-fracture change, or post-fracture trajectory differences. METHODS: We used waves 3-13 (1996-2016) of the Health and Retirement Study. Eligible participants were aged 65 years or more, free of baseline hip fracture, and had repeated depressive symptom assessments. Incident hip fracture was defined as the first positive report after a prior negative report. Depressive symptoms were measured using the 8-item Center for Epidemiologic Studies …

Obstructive sleep apnea risk is associated with poor physical performance: a cross-sectional analysis of the U.S. health and retirement study.

STUDY OBJECTIVES: Obstructive sleep apnoea (OSA) may be linked to poor physical performance and fall risk, yet this association remains underexplored. This study examined associations between OSA risk, balance, gait speed and handgrip strength (HGS) in community living adults across age-groups and sexes. METHODS: Cross-sectional data from the 2016 Health and Retirement Study were analysed. Probable OSA was estimated with an adapted STOP-Bang questionnaire. Poor balance was defined as the inability to hold a semi-tandem stance for 10 s; slow gait speed as walking < 0.8 m/s over 2.5 m; and weak…

Early-life nutritional environment is associated with late-life cognition in the Health and Retirement Study, a pellagra epidemic natural experiment.

Early-life exposures are important to several late-life health outcomes. We sought to study the effect of an in utero nutritional environment and its interaction with Alzheimer's disease (AD) genetic risk on late-life cognitive function. We used a natural experiment created by the pellagra epidemic, a nutritional disease caused by a vitamin B3 deficiency, to evaluate the association between in utero pellagra epidemic exposure and late-life cognitive function in the Health and Retirement Study (N = 18,285). We also evaluated whether the in utero exposure could modify the AD polygenic score's (P…
