This study aimed to assess the application of ultrasonography for preoperative diagnosis of retroperitoneal fibrosis (RPF). A total of 51 patients with clinically suspected RPF underwent transabdominal ultrasonography and ultrasound-guided histopathologic biopsy (tAU-UGHB). OBJECTIVE: To evaluate the therapeutic effect of tamoxifen monotherapy in patients with retroperitoneal fibrosis (RPF).
PATIENTS AND METHODS: From 2007 on, 31 patients with idiopathic RPF were treated with tamoxifen monotherapy. Follow-up investigations included magnetic resonance imaging, laboratory measurements, registration of side effects and changes or removal of ureteral stents. The pathogenesis of idiopathic retroperitoneal fibrosis (IRPF) has been attributed to an autoimmune response to atherosclerotic lipid material leaking from blood vessels. Corticosteroids and cytotoxic agents have been used for therapy. Based on the immunosuppressive and anti-fibrotic action of mycophenolate, we administered this agent to a patient with biopsy-proven IRPF and achieved a rapid, complete and sustained remission with a 6-year follow-up.

Retroperitoneal fibrosis (RPF) is characterized by replacement of the normal tissue of the retroperitoneum with fibrosis and/or chronic inflammation. Idiopathic retroperitoneal fibrosis (RF) is a histologically benign, nonspecific inflammatory process of the fibroadipose tissue in the retroperitoneum with an unknown etiology. It may be complicated due to encasement and obstruction of the retroperitoneal structures. Tamoxifen has been known to be an effective agent in regression of desmoid tumors which include similar mesenchymal elements to those seen in RF. Herein, we reported a case of RF presented with ureteral obstruction and acute renal failure. The patient was successfully treated with tamoxifen.

Retroperitoneal fibrosis is an uncommon inflammatory condition in which the retroperitoneal tissue is transformed into a tight sheet of fibrous tissue. It can occur in association with various diseases, including rheumatic conditions. We report a new case in a 66-year-old man with spondyloarthropathy. Nine similar cases have been reported. Axial manifestations predominated, and half the patients carried the HLA-B27 antigen. The relation between retroperitoneal fibrosis and spondyloarthropathy remains controversial.

OBJECTIVE: To present a case of rupture of the renal pelvis in a patient with idiopathic retroperitoneal fibrosis and no previous dilatation of the left excretory system.
METHODS/RESULTS: A patient with retroperitoneal fibrosis underwent surgery for ureterolysis and intraperitonealization of the right ureter. The control IVP was unremarkable. The authors describe an uncommon case of a localized form of idiopathic retroperitoneal fibrosis in a 70-year-old patient. They considered the tumour in the right hypogastrium (20 x 18 x 10 cm) malignant. The result of histological examination was surprising--Ormond's disease. The process could not be removed without damaging the common iliac artery. Simple suture of the artery proved unsuitable. During the postoperative period occlusion of the artery developed which was successfully resolved by an aorto-femoral bypass. Half a year after surgery the patient did not suffer a relapse.

Retroperitoneal fibrosis has an unknown pathogenesis. Medical treatment should be preferred to surgery. However, surgical treatment is required when an ureteral involvement is present. A case of retroperitoneal fibrosis with spontaneous regression is reported. Current knowledge on pathogenesis, diagnosis and preferred treatment is outlined.

A 56-yr-old man was known with retroperitoneal fibrosis. Seven years later he developed a painful swelling of the left thyroid lobe, which turned out to be Riedel's thyroiditis. The associations between Riedel's thyroiditis, retroperitoneal fibrosis, mediastinal fibrosis, sclerosing cholangitis and fibrosis in other organ systems are thought to be manifestations of multifocal idiopathic fibrosclerosis.
