Lower Extremity Weakness in an Adolescent Female - A Rare Presentation of Ewing Sarcoma.

BACKGROUND: Ewing sarcoma, a rare cause of cord compression, is predominantly of osseous origin but can also originate in soft tissues. Soft-tissue manifestations account for <15% of all Ewing sarcoma tumors, and even fewer cases of Ewing sarcoma originating in the epidural space have been documented. CASE REPORT: A 19-year-old female presented to the emergency department for worsening low-back pain during the previous 6 months and numbness and weakness in her legs during the prior 2 weeks. Magnetic resonance imaging revealed an epidural mass at the L4-L5 level. Intravenous steroids were star

Subungual Ewing sarcoma/PNET tumor family of the great toe: a case report.

Ewing's sarcoma is seen mainly in patients less than 18. This aggressive tumor generally affects the axial skeleton and only rarely involves the acral regions. Ewing's sarcoma in the foot is inordinately scarce. Clinical features are uncertain and can imitate other common diseases. This paper presents a case of 62-year-old malewith complaints of pain and swelling of the subungual area of his right great toe. The lesion was excised, and histopathological diagnosis of Ewing's sarcoma was made. Histopathological examination, supported by immunochemical methods, remains the mainstay of diagnosis.

Primary Ewing sarcoma/primitive neuroectodermal tumor of the renal pelvis: a case report.

Ewing sarcoma/primitive neuroectodermal tumor (ES/PNET) is a childhood malignancy, typically occurring in the bone and rarely in any other part of the body. We herein present a case of ES/PNET of the renal pelvis. A 37-year-old male patient presented with a chief complaint of pain in the left flank and gross hematuria. The tumor had caused moderate hydronephrosis, and ureteroscopic biopsy findings were highly suspicious of sarcoma. Subsequently, radical nephroureterectomy was performed. On the basis of the pathological and cytogenetic findings, a final diagnosis of primary ES/PNET of left rena

A very rare cause of neck pain: primary Ewing sarcoma of the axis.

We report the case of a 7-year-old boy who presented with a 1-month history of neck pain, left-sided torticollis, and no neurological deficit. Computed tomography and magnetic resonance imaging revealed an expansile lesion in the axis, with epidural and prevertebral soft tissue components. Histopathologic examination of the biopsy specimen revealed primary vertebral Ewing sarcoma. This is the first case of primary vertebral Ewing sarcoma that has presented with torticollis. It is essential for physicians to be familiar with this condition and the associated imaging findings because early diagn

Epiphyseal primary location for osteosarcoma and Ewing sarcoma in patients with open physis.

Conventional osteosarcoma and Ewing sarcoma are the most common childhood malignant tumors of bone. These tumors arise in the metaphysis and diaphysis of long bones and may extend secondarily into the epiphysis. Primary epiphyseal location for these tumors is rare and may constitute a diagnostic and therapeutic challenge. The authors report two skeletally immature patients with an osteosarcoma and Ewing sarcoma limited to the epiphysis. Clinical, radiographic and histopathologic findings are discussed.

2-Methoxyestradiol induces apoptosis in Ewing sarcoma cells through mitochondrial hydrogen peroxide production.

The Ewing sarcoma is the second most common bone tumor in children and young adults. Despite the advances in therapy, the 5-year survival rate for patients with metastatic disease is poor, indicating the need for alternative treatments. Here, we report that 2-methoxy-estradiol (2-Me), a natural estrogen metabolite, induced a caspase-dependent apoptosis of Ewing sarcoma-derived cells independently of their p53 status. 2-Me-induced apoptosis occurred through the mitochondrial death pathway as evidenced by reduction of the mitochondrial transmembrane potential, cytochrome c release and caspase-9

Addition of zoledronic acid to consolidation chemotherapy in Ewing sarcoma-EURO EWING 2012 (EE2012): an international, open-label, randomised controlled phase III trial.

BACKGROUND: Pre-clinical data demonstrated anti-tumour effect in Ewing sarcoma (ES) for zoledronic acid. This provided the rationale for the second randomisation (R2) in the EURO EWING 2012 (EE2012) trial, whether the addition of zoledronic acid to consolidation chemotherapy improved clinical outcomes. METHODS: EE2012 was a European academic, open-label, randomised controlled phase III clinical trial. Patients with ES or Ewing-like sarcomas were included (R1). Patients meeting the R2 eligibility criteria (age 5-49 years) were randomised (1:1) to receive either nine cycles of zoledronic acid o

Extraskeletal Ewing sarcoma with uterine cornua attachment mimicking high grade endometrial stromal sarcoma: A case report and brief literature review.

BACKGROUND: Ewing sarcoma is an EWSR1-rearranged aggressive malignancy that occurs commonly in bone and has small round blue cell morphology. A diagnostic challenge is presented in the cases of extraskeletal Ewing sarcoma involving solid organs, such as the uterus. CASE REPORT AND BRIEF LITERATURE REVIEW: We present the case of a 54-year-old female with a large pelvic mass connected to the uterine cornua and retroperitoneal soft tissue. Upon surgical debulking, the mass was characterized by infiltrative tumor cells with high nuclei-to-cytoplasmic ratio invading from the serosal surface into t

A Spinal Cord Sarcoma With EWSR1::WT1 Fusion Exhibited Ewing Sarcoma-Like Pattern Rather Than Desmoplastic Small Round Cell Tumor.

We received a case of a 22 years old female, with a spinal cord sarcoma. The tumor exhibited a morphology similar to that of a small blue round cell tumor, lack of desmoplastic stroma. Immunohistochemical analysis demonstrated diffusely membranal expression of CD99 and cytoplasmic expression of Desmin, but was negative of NKX2.2. Fluorescence in situ hybridization indicated the EWSR1 gene rearrangement. Next generation sequencing (NGS) identified a gene fusion of EWSR1(E9)::WT1(E8). The fusion gene was confirmed by RNA NGS as well. There was a challenge on diagnosis whether should it be diagno

GPR64, Screened from Ewing Sarcoma Cells, Is a Potential Target for Antibody-Based Therapy for Various Sarcomas.

Ewing sarcoma is an aggressive and the second most common bone tumor in adolescent and young adult patients. The 5-year survival rate is 60-70% for localized disease but 30% for patients with metastases. Here, we aimed to identify a therapeutic target for Ewing sarcoma and evaluate antibody-based therapeutic agents using in vitro and in vivo models. We identified G protein-coupled receptor 64 (GPR64) as a therapeutic target for Ewing sarcoma via next-generation RNA-sequencing. GPR64v205 mRNA was expressed in HTB166, A673, MG63, 143B, HS-Sy II, and HT1080 cell lines as well as in Ewing sarcoma,

Targeting of the oncogenic fusion EWSR1-FLI1 in Ewing sarcoma by CRISPR/dCas9 silencers.

Despite the revolutionary impact of genome engineering tools in medicine, the safe and effective intracellular delivery of CRISPR remains a major obstacle to clinical applications. Here, we utilize precision molecular targeting and delivery strategies based on CRISPR-nuclease-dead Cas9 (dCas9) systems adapted for epigenetic repression (dCas9-Krüppel-associated box [KRAB]) to silence oncogenic drivers with high selectivity. As proof of principle, we target the EWSR1-FLI1 translocation, which encodes a chimeric and hard-to-drug oncogenic transcription factor driving approximately 85% of the case

Androgen Receptor Positive EWSR1::FEV-Rearranged Prostatic Ewing Sarcoma Mimicking High-Grade Neuroendocrine Carcinoma.

Ewing sarcoma (ES) is a rare aggressive neoplasm that is the second most common primary bone tumor of childhood and adolescence, with less frequent extraskeletal presentations. ES with EWSR1::FEV translocation is extremely rare and is characterized by extraskeletal location, varying morphology and immunophenotype, and an aggressive clinical course. We present a prostatic ES confirmed by EWSR1::FEV fusion, detailing its clinical presentation, histopathologic and immunophenotypic features, molecular profile, and management. A man in his mid-50s presented with urinary frequency and difficulty voi

Differential biological effects of scattered versus scanned clinical proton beams on Ewing sarcoma models.

BACKGROUND: Proton therapy (PT), first delivered passively using the Double Scattering (DS) technique, has shifted to active delivery through the Pencil Beam technique (PBS). However, the biological effectiveness of DS versus PBS proton beams on cancer cells, remain largely unknown. Given that PBS-PT is now commonly used to treat young patients with Ewing sarcoma, this point is of major importance in clinical practice and raises the question of the same effectiveness of both proton modalities on tumor control. METHODS: This study aims to compare the biological effects of clinical PBS vs. DS p

TGFβ Inhibition during Radiotherapy Enhances Immune Cell Infiltration and Decreases Metastases in Ewing Sarcoma.

UNLABELLED: Ewing sarcoma is an aggressive cancer diagnosed in adolescents and young adults. Inhibition of TGFβ is being tested in limited clinical trials for relapsed Ewing sarcoma. TGFβ is an immunosuppressive cytokine that exists in latent and active states. The functional impact of TGFβ inhibition on the Ewing tumor microenvironment (TME) and on Ewing tumor behavior remains largely unknown. In this study, we use single-cell RNA sequencing analysis of human Ewing tumors to demonstrate that immune cells are the largest contributors of TGFB1 expression in the human Ewing TME. We utilize a hum

Utility of Protein Kinase C Beta II Immunohistochemistry in Differential Diagnosis of Ewing Sarcoma.

The diagnosis of Ewing sarcoma can be challenging, particularly when the tumor is present in an atypical location and resembles histologic mimics. The hallmark feature of Ewing sarcoma is chromosomal translocation, t(11;22)(q24;q12), involving EWSR1 and ETS gene family members. For decades, fluorescence in situ hybridization with a break-apart EWSR1 probe has been the diagnostic gold standard. However, EWSR1 rearrangements have been identified in other malignancies; thus, the detection of chimeric EWSR1 transcripts has become a preferable approach. Occasionally, insufficient tissue, severe RNA

Pediatric thoracic spinal Ewing sarcoma/primitive neuroectodermal tumor: a case report and literature review.

The Ewing sarcoma/primitive neuroectodermal tumor (ES/PNET) is highly malignant neoplasms composed of undifferentiated small round cells with the ability to differentiate into various tissue types. We reported the case of an 11-year-old boy who presented with unsteady gait, progressive back pain, bilateral lower limb weakness (more pronounced in the left leg), and urinary retention. Magnetic resonance imaging (MRI) demonstrated a uniformly intense extradural lesion at the T3-5 vertebral level on the left posterolateral side of the spinal canal. The lesion measured 1.35 cm × 4.8 cm on pre-contr
