Aplasia of the vertex without scalp defect.

We report a case of an extremely rare craniofacial condition, which, to our knowledge, has previously been reported once only. A male infant presented with a giant congenital bone defect of the skull, in the vertex region (10 x 20 cm) and without scalp deficiency. Minimal turricephaly and moderate orbital hypertelorism were associated with slight limb abnormalities, but psychometrical assessments appeared normal. Nonsurgical treatment was initially decided upon, but spontaneous reossification was so moderate that skull reconstruction was carried out at 28 months of age because of the risk of t…

Acquired arteriovenous fistulas of the scalp in hemophiliacs.

The development of arteriovenous fistulas following multiple intravenous infusions in hemophiliacs has been previously reported. In this report, we discuss two patients in whom arteriovenous fistulas developed following the use of scalp veins as sites of infusion. Because of the rich vascularity of this area and the proximity of the arterial and venous systems we recommend that this site not be used for factor infusion.

Successful replantation of a totally avulsed scalp.

A case of successful replantation of a totally avulsed scalp in a 19-year-old woman is presented. Although severely traumatized, about 90 percent of the surface of the replanted scalp survived. Only one artery (left superficial temporal) and four veins were anastomosed, sufficient enough to revascularize the entire scalp.

Response of scalp psoriasis to oral ketoconazole.

A randomised double-blind placebo-controlled study showed improvement of scalp psoriasis with oral ketoconazole, although the study had to be stopped before completion because of the possibility of drug toxicity. The common appearance of psoriasis in the scalp and certain other sites may be due to pityrosporal colonisation that also causes seborrhoeic dermatitis and dandruff.

[Dowling-Degos disease with involvement of the scalp].

Dowling-Degos disease is described in a 40 year-old man, involving the nape of the neck, axillary regions, scrotum, intergluteal fold and also the scalp. There was no associated hair growth anomaly. There was extensive pitting of the entire face. Dowling-Degos disease appears to result from a genetically determined imbalance of epithelial-mesenchymal regulatory mechanisms causing increased epithelial proliferation, as well as a disturbance of the maturation and differentiation of the proliferating keratinocytes.

Scalp avulsions and review of successful replantation.

Microvascular surgery has greatly altered the handling of scalp avulsions. Review of the injury mechanism and of successful replantations has yielded some basic axioms concerning treatment of this tragic injury. Generally, larger avulsions contain vessels that are easily identified and anastomosed, and therefore have a better prognosis. Long periods of ischemia do not appear to threaten graft viability. The superficial temporal artery is the most reliable artery for scalp replantation; accompanying veins can normally be used for venous drainage, though vein grafts are often needed due to vesse…

Aggressive basal cell carcinoma of the scalp.

Three examples of an unusually aggressive form of basal cell carcinoma in the skin of the scalp were treated surgically. The extent of each tumor was verified with gross and microscopic examination. The tumors invaded fat, muscle, fascia, and bone. Dural involvement was seen in one case. Light microscopy showed a marked variation in growth patterns and cytologic types from one portion of the tumor to another. Characteristics included a diffusely infiltrating pattern with a fibrotic stromal response, rare intraepidermal tumor nests, formation of occasional duct-like structures, poor cellular co…
