PURPOSE: To evaluate the influence of smoking on comorbidity, treatment, visual and general outcome in patients with scleritis.
METHODS: The smoking habits of 103 patients with a diagnosis of episcleritis or scleritis were evaluated. These patients were treated by one ruling protocol at the Leiden University Medical Center between 1997 and 2000. Medical records of each patient were evaluated in detail. Data on possible factors concerning smoking were collected by postal questionnaire.
RESULTS: Of all 103 patients diagnosed with either episcleritis or scleritis, 41 (39.8%) were smoking during…

CASE REPORT: A 48-year old woman diagnosed with rheumatoid arthritis 10 years previously, developed severe anterior scleritis in the right eye (RE) refractory to conventional treatment. The patient was treated with 3 intravenous doses of Infliximab achieving complete remission of the scleral inflammation within four weeks.
DISCUSSION: The use of Infliximab, a blocker of tumour necrosis factor (TNFalpha) could be an alternative approach to the management of refractory scleritis, but further long-term investigations would be necessary to evaluate their safety and efficacy.

PURPOSE: To report a case of nodular scleritis and panuveitis associated with erythema elevatum diutinum, a rare immunocomplex-mediated skin disease.
DESIGN: Observational case report.
METHODS: A 22-year-old woman who was diagnosed with erythema elevatum diutinum developed nodular scleritis and panuveitis of the right eye. She had experienced peripheral ulcerative keratitis with corneal perforation.
RESULTS: All other known causes of nodular scleritis and panuveitis were investigated and ruled out.
CONCLUSIONS: Erythema elevatum diutinum should be considered as an underlying systemic disea…

PURPOSE: To report a case of congenital erythropoietic porphyria that presented as acute scleritis over a bilateral scleromalacia perforans in the interpalpebral fissure.
METHODS: An 18-year-old man presented with painful red eye, a history of photophobia, and passing highly colored urine since childhood. Dermatological and biochemical evaluations were done.
RESULTS: The patient had normal vision in both eyes with bilateral scleromalacia perforans. The right eye showed painful, nodular scleritis. Dermatological examination revealed multiple, vesciculobullous cutaneous lesions with atrophy an…

Approximately 50% of patients with scleritis are known to be associated with systemic connective tissue diseases or vasculitic diseases such as rheumatoid arthritis (RA), Wegener's granulomatosis, relapsing polychondritis, and systemic lupus erythematosus. The patients with scleritis in association with Behcet's disease (BD) have been rarely described in the literature. We report a 46-year-old female patient with BD who developed the nodular scleritis.

UNLABELLED: Numerous systemic diseases can cause scleritis or episcleritis. Frequently, symptoms and complications compromising vision can only be managed with systemic immunosuppressants. There are no clear guidelines on the indications for systemic immunosuppressants in patients with episcleritis and scleritis.
PATIENTS AND METHODS: The aim of the present retrospective study was to investigate how many patients with episcleritis or scleritis have an associated systemic disease and at what stage it is diagnosed. Secondly, the proportion of patients who present with episcleritis or scleritis …

We report a case with unilateral recurrent anterior scleritis which developed sclerochoroidal calcification at the posterior pole without any overt sign of posterior scleritis during the follow-up period of nine years. We believe that subtle posterior scleral inflammation might lead to sclerochoroidal calcification which might be dystrophic in nature.

PURPOSE: To determine the origin of scleritis, uveitis, and glaucoma in a boy with fever, sore throat, joint pains, and malaise.
METHODS: The patient underwent extensive hematologic, serologic, rheumatologic, and radiologic examinations.
RESULTS: The patient recovered after extensive medical therapy. He met the Jones criteria for the diagnosis of acute rheumatic fever and had an increased result for antistreptolysin O antibody titer.
CONCLUSIONS: Antistreptococcal antibody testing is helpful in diagnosing rheumatic fever as a cause of scleritis, uveitis, and glaucoma.

Posterior scleritis is a rare disease, the clinical signs of which may vary. There is no consensus on the appropriate method of treatment for this disease. Some have advocated treatment with nonsteroidal antiinflammatory drugs, whereas others have reported variable success with potentially more toxic therapy. Patients with posterior scleritis constitute approximately 1% of the uveitis clinic population at the Casey Eye Institute in Portland, Oregon. The cases of 6 patients with posterior scleritis, the diagnosis of which was elusive, are reported. Posterior scleritis was generally confirmed by…

Isolated scleritis (without keratitis) associated with infections is uncommon, and correct diagnosis and appropriate therapy for it are often delayed. Six patients with infection-associated scleritis were seen at our institution between May 1983 and May 1990 (these patients represented 4.6% of all patients with scleritis [six of 130 patients] in that period). Three of these cases were associated with systemic infections. One was associated with syphilis, one was associated with tuberculosis, and one was associated with toxocariasis. Three cases resulted from local infections. One was associate…

Scleritis, episcleritis and Mooren's ulcer form a poorly understood spectrum of inflammatory syndromes involving the ocular surface. Their pathogenesis is thought to be a vasculitis and may be related to immune complex deposition, as these syndromes are often associated with systemic diseases which are assumed to be vasculitic in aetiology. Scleritis and Mooren's ulcer are amongst the most severe external inflammatory diseases, often causing extensive damage to the sclera and cornea. They require aggressive investigation and management, often including systemic anti-inflammatory therapy. Syste…

We used autogenous periosteum to reinforce scleral defects in two patients with necrotizing scleritis. This technique produced good anatomical results and maintained useful vision in each case. Autogenous periosteum offers advantages over other tissues as a tectonic graft for necrotizing scleritis, since it is easy to harvest and handle, it is strong and vascular, and is not subject to immunologic rejection.

A 29-year-old male patient with IgA nephropathy developed scleritis and myasthenia gravis during the follow-up period. No lymphocytic infiltration or atrophy in muscle fibers was observed in muscle biopsy specimens. However, an immunofluorescent study on the same specimens showed deposits of immunoglobulin A in muscular vessels. The development of scleritis and myasthenia gravis in a patient with IgA nephropathy suggests a common immunologic basis for these disorders.

Large cells containing prominent, refractile PAS-positive inclusions were found in cytological smears prepared from scrapings of the conjunctiva of an elderly diabetic patient with idiopathic scleritis progressing to scleromalacia perforans. Electron-microscopic study of these inclusions revealed them to be intracellular accumulations of dense, flocculent mucoid material within vacuoles whose interior surface was frequently decorated with microvilli. Similar inclusions have been reported in a number of neoplasms in nonocular tissue. The significance of these findings with regard to the patient…
