Intracranial aerocele in facial injury.

Aspects of intracranial aerocele are described and exemplified by two cases which presented to the Plastic Surgery Unit. Austin Hospital, Melbourne, as radiological findings incidental to facial injury. The first case illustrated the occasional problem of identifying the site of dural tear; a fracture could not be identified on X-ray film or at operation, and this can make subsequent dural repair more difficult. The second case was associated with florid clinical signs of facial fractures which were supported by X-ray findings. Both patients underwent surgery for the primary injury, and receiv

Posterior cruciate ligament rupture and all-epiphyseal repair with suture tape augmentation in a 5-year-old girl: a case report and review of the literature.

INTRODUCTION: Only a few case reports regarding pediatric posterior cruciate ligament (PCL) ruptures without bone avulsion exist in the literature. The present study aims to share our experience in the diagnosis, treatment, and prognosis of a child with a proximal PCL tear. MATERIALS AND METHODS: This article reports a 5-year-old female diagnosed with a proximal PCL tear. The ruptured PCL was repaired with an all-epiphyseal suture tape augmentation (STA) without evidence of growth plate violation. RESULTS: The suture tape was removed under arthroscopy and revealed the PCL was re-attached at

[Ultrasonography in the primary diagnosis and follow up of ovarian tumors].

Preoperative sonographic data in 317 patients who underwent surgery at the University Women's Clinic Heidelberg for an ovarian tumour during a 6 years' observation period, were compared with the results of clinical examination. The prognostic value of both techniques was examined, by relating the findings (as to tumour size, demarcation from the uterus) to the postoperative histological or cytological diagnosis. In the 214 cases of benign and 103 of malignant ovarian tumours a correct diagnosis was provided by ultrasound in 88% and 74% respectively. Thus significantly more often than by clinic

[Surgical therapy of germinal testicle tumors (author's transl)].

Surgical intervention in patients with germinal testicular tumors is performed to obtain the exact tumor histology and to define the stage of disease. This can be achieved by inguinal orchiectomy and retroperitoneal exploration. In cases without metastases, a modified retroperitoneal lymphadenectomy (LA) can reduce operative morbidity. With regard to the juvenile age of the patients, protection of ejaculation is of particular importance. If retroperitoneal lymph node metastases are present radical LA should be performed. In case of infiltration of ureters or kidney pedicles a coincident of nep

A surgeon's nightmare: pyoderma gangrenosum with pathergy effect mimicking necrotising fasciitis.

A 53-year-old woman was admitted for vulval swelling and fever. She was initially diagnosed with vulval cellulitis and given parenteral antibiotics. Within 1 week, she developed necrotic-looking skin lesions extending from her vulva to her buttock. Emergency surgical debridement with diversion colostomy was performed in view of suspected necrotising fasciitis. Shortly after the surgery, she developed necrotic-looking skin lesions at the peripheral venous cannula insertion site, central line insertion site, and around her surgical wounds and stoma. A second surgical debridement was performed an

Sequential compression device may cause peroneal nerve palsy.

This article presents 2 cases of foot drop after joint replacement surgery that presented after sequential compression device application. In both cases, intact peroneal nerve function was documented by the surgeon in the recovery room prior to sequential compression device application. We believe that excessive pressure over the superficial aspect of the peroneal nerve in conjunction with decreased pain stimulus from analgesia may have contributed to these complications. We maintain sequential compression devices are the current mechanical thromboprophylaxis of choice; however, 4 recommendati

Strangulated Small Bowel Within a Posterior Rectus Sheath Hernia in a Patient Without Prior Abdominal Surgery.

BACKGROUND: Posterior rectus sheath hernia is an exceptionally rare interparietal abdominal wall hernia in which abdominal contents herniate through the posterior rectus sheath into the retrorectus space while the anterior rectus sheath remains intact. Clinical diagnosis may be difficult because the hernia remains confined within the abdominal wall, and delayed recognition may result in bowel strangulation. We report a rare case of spontaneous posterior rectus sheath hernia complicated by bowel necrosis requiring staged operative management. CASE PRESENTATION: A 73-year-old woman with no hist

Clinical, radiographic, and arthroscopic outcomes after ulnar shortening osteotomy: a long-term follow-up study.

PURPOSE: Previous studies have investigated the long-term outcomes of ulnar shortening osteotomy (USO) in the treatment of ulnocarpal abutment syndrome (UCA), but none have used arthroscopic assessments. The purpose of this study was to investigate the long-term clinical outcomes of USO with patient-based, arthroscopic, and radiographic assessments. METHODS: We retrospectively reviewed 30 patients with UCA after a minimum follow-up of 5 years, with arthroscopic evaluations at the time of both USO and plate removal. We confirmed the initial diagnosis of UCA by radiography and arthroscopy. Mean

Clinical practice and evidence in endoscopic treatment of bleeding peptic gastroduodenal ulcer.

OBJECTIVE: To investigate treatment practice in non-variceal upper gastrointestinal bleeding (NVUGIB) caused by gastroduodenal ulcer and how it adheres to the best evidence as documented in randomized studies and meta-analyses. MATERIAL AND METHODS: The literature was surveyed to identify appropriate practices, and a structured multiple choice questionnaire developed and mailed to all departments in Denmark treating UGIB. RESULTS: All 42 departments responded. All had therapeutic gastroscopes and equipment necessary for endoscopic haemostasis; 90% of departments had written guidelines. Adjuv

Residual Cholesteatoma After Endoscope-guided Surgery in Children.

OBJECTIVE: Endoscopes can facilitate surgery within tympanomastoid recesses that are not visible with the operating microscope. This study investigates whether use of endoscopes to guide dissection of cholesteatoma leads to lower rates of residual cholesteatoma than using the endoscope only for inspection after microscope-guided dissection. STUDY DESIGN: Comparative cohort study. SETTING: Tertiary pediatric center. PATIENTS: Two hundred thirty-five patients with acquired or congenital cholesteatoma in children <18 years having intact canal wall surgery and follow-up >12 months. INTERVENTIO

Arthroscopic Treatment of Collapsed Subchondral Fatigue Fracture in the Femoral Head of a Young Military Recruit at a 5-Year Follow-Up: A Case Report.

CASE: A 20-year-old military recruit presented with pain in his left hip. He was referred by the military hospital for osteonecrosis of the femoral head in both hip joints. Plain radiographs and magnetic resonance imaging showed a marked collapse in the left femoral head; however, the patient was diagnosed as having subchondral fatigue fractures of the femoral head (SFFFH) after a thorough review of previous sequential images. Arthroscopic debridement and labral repair were performed. At 3 years after surgery, the patient was symptom-free and second-look hip arthroscopy revealed the articular

Primary extramedullary plasmacytoma in the middle ear: differential diagnosis and management.

Primary extramedullary plasmacytoma (PEP) is an uncommon neoplasm of plasma cell origin which afflicts the head and neck mainly. In this study we report a rare case of a 34-year-old man who presented with left ear tinnitus, hearing loss, blocked feeling and headache. Exploratory tympanotomy revealed a mass extending into the attic and the mastoid antrum. Following canal wall-up mastoidectomy, the tumour was carefully removed. Histological examination (including immunoperoxidase staining) and thorough clinical, laboratory and radiological evaluation revealed an exclusively cytoplasmic monoclona

Adhesion prevention in endometriosis: a neglected critical challenge.

Prevention of adhesions, whether de novo or by re-formation, is one of the most important and surprisingly neglected aspect of the treatment of endometriosis. Adhesions may cause infertility, dyspareunia, chronic pelvic pain but also intestinal obstruction and complications at subsequent surgery. They may play a role in the development of some forms of the disease such as ovarian endometriomas and possibly also deep invasive nodules. Three randomized controlled trials have been published documenting some partial success with Interceed, Oxiplex/AP gel or Adept solution in reducing adhesions ext

Open abdomen in acute mesenteric ischemia.

In acute mesenteric ischemia, early diagnosis should optimally be followed by either open or endovascular intestinal revascularization. All too often, diagnosis is delayed and diagnosis and treatment are performed at the same time during explorative laparotomy. The majority of patients will be diagnosed when transmural intestinal infarction has developed and at this time point damage control strategies involving intestinal revascularization, bowel resection, open abdomen and second look may be necessary to salvage the patient. This review outlines the principles of the damage control surgery a

Acute renal insufficiency complicating major cardiovascular surgery.

Thirty-one patients underwent hemodialysis for renal failure as a complication of major cardiovascular surgery at the University of Minnesota (1968-1973). Only eight patients (26%) survived. A review of the literature shows that since the beginning of hemodialysis the mortality of those patients has not improved. Infection was the overwhelming cause of death. The infections were difficult to diagnosis because they were frequently associated with abdominal abscesses that were almost uniformly overlooked. Several possible ways of improving these patients survival are: 1) the use of early operati

Management of mucinous urachal neoplasm presenting as pseudomyxoma peritonei.

BACKGROUND: Mucinous neoplasms of the urachus are rare malignancies so that the physicians' index of suspicion for a timely and accurate diagnosis is low. Also, this disease may present with a wide variety of symptoms and signs. METHODS: Two patients with pseudomyxoma peritonei as the initial presentation of urachal mucinous adenocarcinoma were treated successfully. The medical literature regarding treatment options for this manifestation of the disease was searched. RESULTS: Two patients with large volume of pseudomyxoma peritonei originating in a mucinous urachal neoplasm were treated with

Clinical feasibility of a novel biphasic osteochondral composite for matrix-associated autologous chondrocyte implantation.

OBJECTIVE: Matrix-associated autologous chondrocyte implantation has been used to treat cartilage defects. We developed a biphasic cylindrical osteochondral composite construct for such use, and conducted this study to determine its feasibility for treating osteochondral lesions in human knees. METHOD: Ten patients with symptomatic osteochondral lesions at femoral condyles were treated by replacing pathological tissue with the construct of dl-poly-lactide-co-glycolide, whose lower body was impregnated with β-tricalcium phosphate and served as osseous phase. The construct had a chamber to load

A study on 107 patients with acute mesenteric ischemia over 30 years.

Acute mesenteric ischemia (AMI) is a life threatening cause of acute abdomen. The purpose of this study is to define risk factors that predict the adverse outcome of AMI and to present our experience in the last 30 years. Hospital records and clinical data of 107 patients undergoing surgical intervention for AMI during the last 30 year period were reviewed and clinical outcomes as well as factors influencing mortality were analyzed. Mesenteric arterial thrombosis, arterial embolism and nonocclusive mesenteric ischemia (NOMI) were the cause of AMI in 68 (63.6%), 28 (26%), and 11 patients (10.2%

Incidence Rate and Determinants of Recurrent Cholesteatoma Following Surgical Management: A Systematic Review, Subgroup, and Meta-Regression Analysis.

Background/Objectives: Cholesteatoma is a destructive middle ear pathology that can cause chronic infection, ossicular erosion, and hearing loss. While surgical excision is the standard treatment, recurrence remains a major clinical challenge, and comprehensive data on long-term outcomes are limited. This meta-analysis evaluated cholesteatoma recurrence rates following surgery, identified clinical and surgical predictors of recurrence, and assessed trends across follow-up durations, techniques, and patient demographics. Methods: We searched PubMed, Scopus, Web of Science, CENTRAL, and Google S
