A dual injury of the shoulder: acromioclavicular joint dislocation (type IV) coupled with ipsilateral mid-shaft clavicle fracture.

A direct blow to the shoulder, as may be sustained in a road traffic accident (RTA), can result in various combinations of fracture dislocations in the shoulder joint complex. Among these, a rare variety is an acromioclavicular joint (ACJ) dislocation coupled with ipsilateral mid-shaft clavicle fracture. Diverse treatment options have been described in the literature, ranging from non-operative and operative, to hybrid management. Treatment for this complex injury is predominantly dictated by the type of dislocation and displacement of the clavicle fracture, as well as age and demand of the pa

Percutaneous Transluminal Angioplasty for Axillary Artery Dissection After Open Reduction of Shoulder Fracture-Dislocation: A Case Report.

CASE: Injury of the axillary artery after shoulder-joint fracture or dislocation is uncommon. We describe the clinical outcome of an elderly patient with a dissecting aneurysm of the axillary artery following open reduction of a fracture-dislocation of the shoulder. The arterial dissection was successfully managed with percutaneous transluminal balloon angioplasty. CONCLUSION: This case report highlights the need for careful vascular examination of all elderly patients with a fracture or dislocation of the shoulder and the possible use of percutaneous transluminal angioplasty for the manageme

An assessment of patient satisfaction with nonoperative management of clavicular fractures using the disabilities of the arm, shoulder and hand outcome measure.

BACKGROUND: Clavicle fractures historically have been managed without internal fixation. Current literature is raising questions regarding this management as opposed to offering operative fixation in some instances. This study addresses the use of the Disabilities of the Arm, Shoulder and Hand (DASH) outcomes measure to identify those that have the least satisfaction with nonoperative care of the clavicle fracture based upon clavicular deformity and variation in fracture location based upon Allman Classification. METHODS: Patients having suffered clavicle fractures were mailed the DASH Outcom

Reconstruction of a missed posterior locked shoulder fracture-dislocation with bone graft and lesser tuberosity transfer: a case report.

INTRODUCTION: Posterior shoulder fracture-dislocation is a rare emergency condition with poor prognosis when there is a delay in diagnosis and presence of associated injuries. CASE PRESENTATION: We present a case of a neglected four-part fracture-dislocation of the proximal humerus in a 34-year-old Greek woman. Except from the substantially displaced and comminuted tuberosity fractures, an anterolateral defect of approximately 50% of the articular surface was apparent. Open reduction of the humeral head was followed by reconstruction of the proximal humerus with allograft impaction, transfer

Risk factors for greater tuberosity nonunion following reverse total shoulder arthroplasty for proximal humerus fractures: a systematic review and meta-analysis.

BACKGROUND: Greater tuberosity nonunion following reverse total shoulder arthroplasty (rTSA) for proximal humerus fracture is known to be associated with poor outcomes. However, it is unknown which factors place patients at risk for not healing their tuberosity repair after rTSA. The purpose of this review is to identify and characterize the predictors of greater tuberosity nonunion (GTNU) following rTSA for proximal humerus fractures. METHODS: A systematic search was conducted using key terms "tuberosity healing reverse" and "tuberosity nonunion arthroplasty" in databases of English language

Acromion and Clavicle Stress Fractures After Reverse Total Shoulder Arthroplasty Reflect Failure to Address Osteoporosis: A Case Report and Literature Review.

The exponential increase in the rate of reverse total shoulder arthroplasty (RTSA) has been accompanied by a rise in complication rates of this procedure. Of these, peri-prosthetic stress fractures can be particularly problematic due to their potential to cause significant impairment of shoulder function. Despite the association between these stress fractures and osteopenia/osteoporosis, pre-operative bone density assessment is not standard practice for elective RTSA. We report the case of a 68-year-old female patient who, at eleven weeks after elective RTSA (for rotator cuff-tear arthropathy)

The distal acromial tip fusion: a novel procedure to mitigate acromial stress fractures in high-risk patients undergoing reverse shoulder arthroplasty.

Acromion and scapular spine fractures (ASFs) pose significant challenges in the management of patients undergoing reverse total shoulder arthroplasty and there is a pressing need for effective preventive measures. Current predictive factors for ASF include female sex, osteoporosis, older age, and anatomical considerations such as glenoid medialization and acromial morphology. However, surgical options to mitigate ASF risk are limited. In this study, we propose a novel technique: incorporation of distal acromial tip fusion (DATF) during primary reverse total shoulder arthroplasty procedures. DA

Satisfactory Short-Term Outcomes of Reverse Shoulder Arthroplasty for Complex Three- and Four-Part Fractures of the Humeral Head in Octogenarians.

BACKGROUND: Proximal humeral fractures with severe comminution and poor bone quality are among the most common injuries in the elderly population. Reverse shoulder arthroplasty (RSA) has been widely used to manage complex three- and four-part humeral head fractures. The purpose of the present study was to report the result of this technique in the demanding population of octogenarians. MATERIALS AND METHODS: Twenty-six patients above the age of 80 years were included in the study and followed for a minimum of one-year follow-up. To assess the functional outcomes the postoperative range of mot

Outcomes of acute vs. delayed reverse shoulder arthroplasty for proximal humerus fractures in the elderly: a systematic review and meta-analysis.

HYPOTHESIS: Reverse shoulder arthroplasty (RSA) has become an increasingly popular treatment option for proximal humerus fractures in the elderly. There is however contradictory evidence on the impact of timing of RSA on patient outcomes. It remains unclear if poor results after initial nonsurgical or surgical management can be improved with delayed RSA. The aim of this systematic review and meta-analysis is to compare the outcomes of acute RSA and delayed RSA for the treatment of proximal humerus fractures in the elderly. MATERIALS AND METHODS: A systematic search was performed on 4 database

Comparison of 30-day complications between reverse shoulder arthroplasty and open reduction internal fixation for the treatment of proximal humerus fractures.

PURPOSE: While the use of open reduction internal fixation (ORIF) has remained stable over the last decade, there has been a significant increase in the use of reverse total shoulder arthroplasty (RTSA) for proximal humerus fractures (PHFs). This study sought to compare the complication profiles of RTSA to ORIF in a large, validated, retrospective cohort. METHODS: Patients who underwent surgical treatment for PHFs with RTSA or ORIF were identified in a national database (NSQIP) using CPT and ICD codes. Demographics and comorbidities were identified for each cohort of patients. Thirty-day comp

Shoulder hemiarthroplasty for proximal humeral fractures: analysis of clinical and radiographic outcomes at midterm follow-up: a series of 51 patients.

Shoulder hemiarthroplasty has historically been the gold standard treatment for proximal humeral fractures not suitable to open reduction and internal fixation. In the last decades, reverse shoulder arthroplasty has gained more and more importance. Aim of this study is to retrospectively analyze subjective and objective outcomes of a group of consecutive patients. Fifty-one patients were evaluated with a mean follow-up of 18 months (range 12-60). For all the cases, the same surgical technique was performed in all cases. Reverse shoulder arthroplasty in 3 and 4 part proximal humeral fractures in patients aged more than 65 years: Results and complications.

INTRODUCTION: The treatment of 3and 4 part proximal humeral fractures in elderly patients is still controversial. The frequent co-existence of poor quality bone and rotator cuff abnormalities in patients with multiple clinical conditions and with difficulties for physical rehabilitation leads to disappointing clinical results, even when the radiological images are acceptable. OBJECTIVE: To evaluate the clinical, radiological, and functional results in patients over 65 years old with complex proximal humerus fractures treated with reverse shoulder arthroplasty. Trunk rotation and hand-elbow-shoulder axis disruption increase the risk of humeral shaft fractures during arm wrestling: a video analysis study.

BACKGROUND: Arm wrestling is a globally popular sport that can lead to serious orthopaedic injuries, particularly humeral shaft fractures. Despite their importance, the mechanisms and positional risk factors underlying these injuries have not yet been clearly defined. This study aimed to investigate whether specific body positions are associated with an increased risk of humeral shaft fractures. METHODS: This study was designed as a retrospective video-based observational comparative study. Seventy-four publicly available videos of arm wrestling matches resulting in humeral shaft fractures we

Acute open reduction and internal fixation versus nonoperative treatment of scapular spine fractures after reverse shoulder arthroplasty: a propensity score-matched study.

BACKGROUND: Scapular spine fractures occur in 1%-11% of cases after reverse shoulder arthroplasty (RSA). While nonoperative management has been recommended, poor clinical outcomes have been reported for Levy type II-III fractures. The literature on surgical management mostly includes open reduction and internal fixation (ORIF) for established nonunions with mixed results. Currently, little evidence exists on acute ORIF of displaced scapular spine fractures after RSA. Therefore, the purpose of this propensity score (PS)-matched study was to compare acute ORIF (<6 weeks) with nonoperative manage

Acromial and scapular spine fractures following reverse total shoulder arthroplasty: treatment dilemma and radiographic prognosticators for enhanced clinical outcomes: a multicenter study.

BACKGROUND: The management of acromial and scapular spine fractures (ASFs) after reverse total shoulder arthroplasty (rTSA) remains challenging, often prompting surgeons to opt nonoperative treatment. The reported outcomes vary due to multiple influencing factors. This study aimed to analyze prognostic factors affecting outcomes of nonoperatively treated ASFs following rTSA. METHODS: Among 2,837 cases, 78 patients (2.7%) with ASFs following rTSA from 8 regional hospitals were retrospectively reviewed, including 10 surgical and 68 nonoperatively managed cases. The study focused on the 68 nonop

Acute plate fixation of displaced midshaft clavicular fractures is not associated with earlier return of normal shoulder function when union is achieved.

AIMS: It is unclear whether acute plate fixation facilitates earlier return of normal shoulder function following a displaced mid-shaft clavicular fracture compared with nonoperative management when union occurs. The primary aim of this study was to establish whether acute plate fixation was associated with a greater return of normal shoulder function when compared with nonoperative management in patients who unite their fractures. The secondary aim was to investigate whether there were identifiable predictors associated with return of normal shoulder function in patients who achieve union wit
