Spontaneous Recovery of Active Shoulder External Rotation in Patients with Brachial Plexus Birth Injury.

BACKGROUND: Brachial plexus birth injury (BPBI) occurs as a result of a difficult delivery. External rotation (ER) of the shoulder (ER) is usually one of the last movements that may recover. There is no consensus about the predicting factors for spontaneous recovery or the optimal timing for surgical treatment of ER in BPBI patients. The aim of this retrospective study was to describe spontaneous recovery of active ER and evaluate predicting factors for the recovery. METHODS: The authors screened 562 patients and identified a consecutive cohort of 103 BPBI patients, who had no active ER at th

Effects of bench press technique variations on musculoskeletal shoulder loads and potential injury risk.

While shoulder injuries resulting from the bench press exercise are commonly reported, no biomechanical evidence for lowering injury risk is currently available. Therefore, the aim of the present study was to compare musculoskeletal shoulder loads and potential injury risk during several bench press variations. Ten experienced strength athletes performed 21 technical variations of the barbell bench press, including variations in grip width of 1,1.5 and 2 bi-acromial widths (BAW), shoulder abduction angles of 45°, 70° and 90°, and scapula poses including neutral, retracted, and released conditi

Aortic Injury After Shoulder Reduction in a Patient with Multiple Rib Fractures: A Case Report.

CASE: A 74-year-old woman presented with multiple rib fractures, hemothorax, spleen rupture, and right shoulder dislocation. Shoulder joint reduction was performed using the traction-countertraction method. An injury to the descending aorta, caused by the seventh rib, was confirmed. CONCLUSION: The authors encountered an aortic injury, a rare complication of shoulder joint reduction, in a patient with a posterior rib fracture. When managing patients with posterior fractures of the fourth to ninth ribs on the left side, the reduction method should be carefully selected depending on the status

Staged Reconstruction for Humeral Osteomyelitis after Severe Crush Injury of the Shoulder: A Case Report.

The incidence of humeral osteomyelitis is relatively rare as compared to incidence of lower limb osteomyelitis. Despite having no guideline in the management of humeral osteomyelitis, surgeons have utilised their experience in managing lower limb osteomyelitis to treat humeral osteomyelitis. By adhering to principles including thorough debridement of necrotic bone and soft tissue, staged bony and/or soft tissue reconstruction, and targeted antimicrobial therapy, a good outcome can be achieved in the management of humeral osteomyelitis. We report a case of Cierny-Mader type IV proximal humeral

Humeral Retroversion and Injury Risk After Proximal Humeral Epiphysiolysis (Little Leaguer's Shoulder).

BACKGROUND: The increased humeral retroversion on the dominant side of throwing athletes is thought to result from repetitive throwing motion. Little Leaguer's shoulder-a rotational stress fracture of the proximal humeral epiphyseal plate-may influence the risk of humeral retroversion and injury of the shoulder or elbow joint. PURPOSE: To investigate the effect of Little Leaguer's shoulder on humeral retroversion and the rates of shoulder and elbow injuries. STUDY DESIGN: Cohort study; Level of evidence, 3. METHODS: 10 high school baseball players (average age, 16.6 years; range, 16-18 year

Axillary Artery Injury In Blunt Shoulder Trauma - An Uncommon Association With Proximal Humerus Fracture.

Axillary artery injury along with proximal humerus fracture as a result of blunt trauma is a rare incidence. We present a 65-year-old male with a pulseless cold hand, who had blunt trauma 3 days ago, leading to fracture of proximal end of humerus along with axillary artery injury. Surgical intervention revealed contusion of axillary artery with intravascular thrombosis causing complete occlusion. Internal fixation of fracture along with excision of contused axillary artery was done followed by interposition venous graft. The diagnostic modalities and treatment options for such a case are discu

Preventing brachial plexus injury during shoulder surgery: a real-time cadaveric study.

BACKGROUND: Brachial plexopathy is not uncommon after shoulder surgery. Although thought to be due to stretch neuropathy, its etiology is poorly understood. This study aimed to identify arm positions and maneuvers that may risk causing brachial plexopathy during shoulder arthroplasty. METHODS: Tensions in the cords of the brachial plexuses of 6 human cadaveric upper limbs were measured using load cells while each limb was placed in different arm positions and while they underwent shoulder hemiarthroplasty and revision reverse arthroplasty. Arthroplasty procedures in 4 specimens were performed

Size of greater tuberosity fragment: a risk of iatrogenic injury during shoulder dislocation reduction.

PURPOSE: Shoulder dislocation with greater tuberosity fractures (GTF) is becoming increasingly common, as is the number of cases of iatrogenic humeral neck fractures (IHNF) during reduction. This study investigated the relationship between size of greater tuberosity fragment and occurrence of IHNF in patients with shoulder dislocation and GTF. METHODS: A retrospective study was made to identify all patients presenting with shoulder dislocation with GTF between September 2014 and July 2016. There were 74 patients with an average age of 52.4 years (range 18-84 years) representing 76 cases of sh

Bilateral spontaneous anterior shoulder dislocation: A missed orthopedic injury mistaken as proximal neuropathy.

A number of orthopedic injuries can occur during epileptic seizures. Anterior shoulder dislocation is one such orthopedic injury that is quite rare. The shoulder dislocation may injure the brachial plexus. Besides seizures, the nerves can also be damaged by anticonvulsive therapy. Muscle wasting following a seizure can misguide a clinician to investigate only neural or muscular pathologies. We report here an individual with epilepsy who was referred to us for electrodiagnostic evaluation of proximal muscle wasting related to a suspected proximal neuropathy. He was found to have a normal electr

The risk of suprascapular and axillary nerve injury in reverse total shoulder arthroplasty: An anatomic study.

PURPOSE: Implantation of a reverse total shoulder arthroplasty (rTSA) places the axillary and suprascapular nerves at risk. The aim of this anatomic study was to digitally analyse the location of these nerves in relation to bony landmarks in order to predict their path and thereby help to reduce the risk of neurological complications during the procedure. METHODS: A total of 22 human cadaveric shoulder specimens were used in this study. The axillary and suprascapular nerves were dissected, and radiopaque threads were sutured onto the nerves without mobilizing the nerves from their native path

Shoulder fusion after a self-inflicted gunshot wound: an injury pattern and treatment option.

Gunshot injuries to the shoulder are rare and difficult to manage. We present a case series of seven patients who sustained a severe shoulder injury to the non-dominant side as a result of a self-inflicted gunshot wound. We describe the injury as 'suicide shoulder' caused by upward and outward movement of the gun barrel as the trigger is pulled. All patients were male, with a mean age of 32 years (21 to 48). All were treated at the time of injury with initial repeated debridement, and within four weeks either by hemiarthroplasty (four patients) or arthrodesis (three patients). The hemiarthropl
