Issuing sickness certificates: a difficult task for physicians: a qualitative analysis of written statements in a Swedish survey.

BACKGROUND: There is scientific evidence, but limited, that physicians experience sickness certification tasks as problematic. More knowledge is needed about physicians' own opinions on this matter. AIM: To further describe the experiences of physicians regarding their task of sickness certification. METHODS: Qualitative analyses of 622 written responses to an open-ended question on physicians' own opinions about sickness certification given in a comprehensive questionnaire that comprised 83 close-ended items about this matter. The questionnaire was sent to 7665 physicians in Sweden and the

Proximate and ultimate causes of pregnancy sickness.

Evolutionary biologists have long been fascinated by pregnancy sickness, the heritable, stereotyped syndrome in early pregnancy that usually consists of benign nausea and vomiting and in around 1% of cases progresses to the pathological extreme hyperemesis gravidarum. Identification of the placental hormone GDF15 as a principal causal factor justifies reassessment of its proximate and ultimate causes. This Review synthesizes knowledge of pregnancy sickness at the four levels of analysis of classical ethology-mechanism, development, phylogeny, and adaptive function. Emerging insight into GDF15'

What physicians want to learn about sickness certification: analyses of questionnaire data from 4019 physicians.

BACKGROUND: Sickness absence is a problem in many Western countries. Physicians have an essential role in sickness certification of patients, which is often recommended in health care but may have side effects. Despite the potentially harming impact of sickness absence, physicians have very limited training in insurance medicine, and there is little research on sickness certification practices. Our aim was to ascertain what knowledge and skills physicians in different clinical settings feel they need in order to improve their competence in sickness certification. METHODS: The data for analysi

Health-related quality of life in patients undergoing anterior cervical discectomy fusion.

Outcomes are an integral part of surgical decision making among patients, families, physicians, and nurses. The purpose of this descriptive, longitudinal study was to investigate and compare health-related quality of life as an outcome measure in patients undergoing anterior cervical discectomy and fusion (ACDF). Data were collected preoperatively, and at 6 weeks and 3 and 6 months postoperatively. Potential participants were identified by staff members at two neurosurgical practices in central New Jersey. Findings revealed improved health over time with significant change in 6 of the 9 subsca

A history of sleeping sickness in Kenya.

Gambian sleeping sickness entered what is now Kenya from Uganda in about 1901 and quickly spread along the Kenyan shores and islands of Lake Victoria, reaching Tanzania in 1902. By 1910 the disease had spread 25 miles inland along the Kuja and Migori rivers and their tributaries. Sleeping sickness waxed and waned in these areas despite attempts to control tsetse fly populations by various methods. It was not until 1950, when the use of insecticides (DDT) applied by backpack sprayer proved successful against Glossina fuscipes at Kibigori, that eradication of G. fuscipes and Gambian sleeping sic

A dietary switch promotes sensory neuron-dependent cancer-associated cachexia.

Sickness behaviors are common in cancer-associated cachexia and affect up to half of lung cancer patients. We demonstrate that among the most common cancer mutations, loss of liver kinase B1 (Lkb1) promotes the development of cachexia in preclinical models of lung cancer. In an effort to improve caloric intake with an obesogenic high-fat diet, we paradoxically observed worsened cachexia-associated sickness. We found that local production of prostaglandin E2 (PGE2), rather than circulating factors, promotes sickness and that genetic, dietary, and pharmacological inhibition of tumor-derived PGE2

Pre-Columbian Andean sickness ideology and the social experience of leishmaniasis: A contextualized analysis of bioarchaeological and paleopathological data from San Pedro de Atacama, Chile.

Sociocultural concepts associated with sickness can profoundly influence social processes and individual experiences of disease. Here, we consider the role of sociocultural beliefs concerning sickness in the construction of individuals' social identities in the pre-Columbian Andes. Paleopathological analyses reveal evidence of mucocutaneous leishmaniasis, a facially disfiguring infectious disease endemic to tropical lowland rainforests, in the skeletal remains of six females buried at Coyo Oriental and Tchecar Túmulo Sur, two Middle Horizon (AD 500-1000) cemeteries in the highland desert of Sa

Obstetricians' attitudes and opinions on sickness absence and benefits during pregnancy.

BACKGROUND: In Sweden, sick leave is taken more frequently by pregnant women than by nonpregnant women. This led us to ask if the taking of sick leave during pregnancy could possibly be explained by attitudes to sickness absence held among obstetricians working in antenatal care. METHODS: All obstetricians (n = 45) engaged in public antenatal care and at work in May 2001 in seven hospitals in South Eastern Sweden were asked to anonymously respond to questions/statements concerning their work; 87% participated. The results were presented as percent (the median value) on a visual analog scale.

Subjective visual vertical in first time seafarers experiencing sea sickness.

OBJECTIVES: Sea sickness, a form of motion sickness, is a common condition among sailors on various sailing vessels, primarily due to their roll and pitch movements. While the exact neurobiological mechanisms remain unclear, the most widely accepted explanation is the sensory conflict theory, which identifies two main sources of conflict: discrepancies between visual and vestibular inputs and conflicts within the vestibular system itself. This study aims to evaluate otolith-mediated verticality perception in first-time seafarers on board a naval frigate ship using the Subjective Visual Vertica

Functional brain network analysis of VR motion sickness.

This study aims to explore the neural impact of virtual reality (VR) motion sickness on individuals from the perspective of brain functional networks, analyze the relationship between changes in Electroencephalogram (EEG) frequency bands and VR motion sickness, and provide a new perspective for evaluating the neural excitation mechanism of VR motion sickness. Twenty-six subjects were grouped based on their sensitivity to VR by a subjective evaluation scale. EEG signals were recorded before and after VR experiment. Functional brain networks were constructed and the changes in perspective of net

Acute mountain sickness is related to nocturnal hypoxemia but not to hypoventilation.

The purpose of the study was to investigate determinants of acute mountain sickness after rapid ascent to high altitude. A total of 21 climbers were studied ascending from <1,200 m to Capanna Regina Margherita, a hut in the Alps at 4,559 m, within <24 h. During their overnight stay at 4,559 m, breathing patterns and ventilation were recorded by calibrated respiratory inductive plethysmography along with pulse oximetry. In the following morning, acute mountain sickness was assessed. Altogether, 11 mountaineers developed pronounced symptoms of acute mountain sickness (Lake Louise score > or =5)

Age-appropriate interventions to address illness perceptions in paediatric oncology: Findings from an expert Delphi study.

BACKGROUND: Paediatric cancer patients face psychosocial challenges, with illness perceptions (IP) playing a central role in coping and health‑related behaviour. While evaluated interventions targeting IP are available in adult oncology, comparable age‑appropriate approaches are lacking in paediatric oncology. This study presents an expert‑derived pool of puppet‑based play therapy interventions aimed at fostering adaptive IP in children aged four to ten years, appraised with regard to suitability and practical implementation. METHODS: As a first analytic step informing a two‑round Delphi proc

Illness by suggestion: expectancy, modeling, and gender in the production of psychosomatic symptoms.

BACKGROUND: Expectancy and modeling have been cited as factors in mass psychogenic illness (MPI), which reportedly affects more women than men. PURPOSE: The purpose of the study is to assess the effects of expectancy and modeling in a controlled laboratory analogue of MPI. METHODS: Students were randomly assigned to inhale or not inhale an inert placebo described as a suspected environmental toxin that had been linked to four symptoms typical of reported instances of MPI. Half of the students observed a female confederate inhale the substance and subsequently display the specified symptoms.

Predictors of parenting behavior trajectories among families of young adolescents with and without spina bifida.

OBJECTIVE: To evaluate the utility of familial and parental variables in predicting trajectories of parenting behaviors among families of young adolescents with and without spina bifida (SB). METHOD: Sixty-eight families with a child with SB and a demographically matched comparison group (CG) of 68 families of an able-bodied child participated. Observational and questionnaire assessments of parenting behavior were collected via home visits at three time points, as were reports of parent and family functioning. RESULTS: Family conflict was negatively associated with adaptive parenting behavio

An integrated cognitive model of panic disorder: the role of positive and negative cognitions.

One reason for the neglect of the role of positive factors in cognitive-behavioural therapy (CBT) may relate to a failure to develop cognitive models that integrate positive and negative cognitions. Bandura [Psychol. Rev. 84 (1977) 191; Anxiety Res. 1 (1988) 77] proposed that self-efficacy beliefs mediate a range of emotional and behavioural outcomes. However, in panic disorder, cognitively based research to date has largely focused on catastrophic misinterpretation of bodily sensations. Although a number of studies support each of the predictions associated with the account of panic disorder

["... but it is for many also a large step: from the brink of death to the celebration of life...". Routine care with new drug treatment possibilities for HIV positive and AIDS patients from the viewpoint of patients].

New combination therapy provides a stabilization of health and a longer life expectancy for people living with HIV/AIDS who are able to deal with their drugs and the strict daily medical regime inflicted. This of course produces high expectations. Due to the success of the combination therapy, the psychosocial problems associated with it have been pushed to the background. How do people living with HIV/AIDS evaluate the changes that occurred since the introduction of the new combination therapy? Which changes affect their daily routine most? Is there a need for change in nursing care? In order

Psychological features in persons at risk for familial Alzheimer's disease.

Persons at risk for inherited neurodegenerative diseases may experience symptoms of anxiety and depression because of concern over the possibility of developing the disease in the future. The purpose of this study was to assess psychological and emotional symptoms in persons at the age of risk for developing early-onset familial Alzheimer's disease (FAD). Their responses on a psychiatric rating scale (SCL-90-R) were compared with four groups: patients with mild FAD; head injury patients; patients with clinically diagnosed depression; and healthy control subjects. Mean scores of the at-risk FAD
