Sleep in the critically ill patient.

Critically ill patients are known to suffer from severely fragmented sleep with a predominance of stage I sleep and a paucity of slow wave and REM sleep. The causes of this sleep disruption include the intensive care unit (ICU) environment, medical illness, psychological stress, and many of the medications and other treatments used to help those who are critically ill. The clinical importance of this type of sleep disruption in critically ill patients, however, is not known. This article reviews the literature on sleep disruption in the ICU, the effects of sepsis on sleep, the effects of commo

How we fall asleep: regional and temporal differences in electroencephalographic synchronization at sleep onset.

OBJECTIVES: We hypothesized that the brain shows specific and predictable patterns of spatial and temporal differences during sleep onset (SO) reflecting a temporal uncoupling of electrical activity between different cortical regions and a dissociated wakelike and sleeplike electrocortical activity in different cortical areas. METHODS: We analyzed full-scalp electroencephalographic (EEG) recordings of 40 healthy subjects to investigate spatial and temporal changes of EEG activity across the wake-sleep transition. We quantified EEG sleep recordings by a fast Fourier transform (FFT) algorithm a

Sleep in the harp seal (Pagophilus groenlandica). Peculiarities of sleep in pups during the first month of their lives.

Two white-coated (ten day-old) and two grey-spotted (one month-old) pups of the harp seal were implanted with electrodes for recording the electrocorticograms of the two hemispheres, electrocardiogram, electrooculogram and electromyogram of the neck muscles. In all individuals ECoG slow waves always developed synchronously in both hemispheres. When white and grey pups were recorded on the snow, the amount of active wakefulness was, respectively, 21.1% (s.e. 11.8%) and 29.9% (s.e. 0.2%) of total recording time, quiet wakefulness = 27.1% (s.e. 4.4%) and 38.2% (s.e. 6.6%), drowsiness = 7.7% (s.e.

Memory consolidation in human sleep depends on inhibition of glucocorticoid release.

Early sleep dominated by slow-wave sleep has been found to be particularly relevant for declarative memory formation via hippocampo-neocortical networks. Concurrently, early nocturnal sleep is characterized by an inhibition of glucocorticoid release from the adrenals. Here, we show in healthy humans that this inhibition serves to support declarative memory consolidation during sleep. Elevating plasma glucocorticoid concentration during early sleep by administration of cortisol impaired consolidation of paired associate words, but not of non-declarative memory of visuomotor skills. Since glucoc

Influence of the cholinergic agonist SDZ 210-086 on sleep in healthy subjects.

The administration of 1.0 mg SDZ 210-086, an orally acting muscarinic agonist, shortened rapid-eye movement (REM) latency, increased REM percent of sleep period time and the total duration of REM sleep, and decreased slow-wave sleep in 12 healthy male subjects. The administration of 0.5 mg SDZ 210-086 had no statistically significant effect on sleep variables. Although the tonic components of REM sleep (REM duration, the REM percent of sleep period time) were increased, REM density percent (total) was significantly decreased due to the prolongation of total REM duration (in minutes) and a para

Somnogenic muramyl peptides.

Sleep-promoting materials isolated from human urine and rabbit brain are muramyl peptides (MPs). The most active component of the urinary material is N-acetylglucosaminyl-1,6-anhydro-N-acetylmuramyl-Ala-Glu-diaminopimel yl-Ala; 1 pmol, infused into a lateral cerebral ventricle of rabbits, induced excess slow-wave sleep (SWS) for several hours. MP-induced sleep is normal in that it is similar to the deep sleep that follows sleep deprivation. Other biological actions of MPs (e.g., pyrogenicity and immunomodulatory activity) could be dissociated, but only in part, from somnogenic actions. Interle

[The sleep-wake cycle during Trypanosoma brucei rhodesiense human African trypanosomiasis in 2 French parachutists].

Polysomnography was performed in two military personnel who contracted human African trypanosomiasis (HAT) in Rwanda. They were evacuated to France and gramined during the fourth, sixth and eleventh months of evolution. Patient A presented a meningoencephalitis with Trypanosoma brucei rhodesiense (T. b. r.) in the cerebrospinal fluid. Between the second and third treatments with melarsoprol, the patient presented an acute encephalitic attack with pyramidal and cerebellar symptoms, which were improved by corticotherapy and disappeared progressively in 6 months. Patient B presented a major infla

Sleep and mechanical ventilation.

Critically ill patients have severe sleep disruption and typically encounter loss of circadian sleep pattern, steep fragmentation, increasing proportions of transitional stages of sleep, and loss of slow wave and rapid eye movement sleep. Mechanical ventilation is associated with these same sleep abnormalities, but what is attributable to the intensive care unit environment versus mechanical ventilation itself may be difficult to discern. Recent studies have shown that the ventilator mode and inappropriate settings can contribute to sleep fragmentation, and it is important to avoid overventila

REM sleep deprivation increases early morning pineal melatonin in castrated rats.

Recently nighttime melatonin levels have been shown to be attenuated in depressive patients or patients with dementia of the Alzheimer type. On the other hand, depression can be transiently relieved by deprivation of rapid eye movement sleep. Since exogenous melatonin administration increases rapid eye movement sleep and slow wave sleep in the rat, could rapid eye movement sleep deprivation then inversely influence endogenous melatonin production? We found indices that in castrated Wistar rats 4 days of rapid eye movement sleep deprivation by the cuff pedestal method elevates the pineal conten

Exploring clinical applications and long-term effectiveness of benzodiazepines: An integrated perspective on mechanisms, imaging, and personalized medicine.

Benzodiazepines have been long-established treatments for various conditions, including anxiety disorders and insomnia. Recent FDA warnings emphasize the risks of misuse and dependence associated with benzodiazepines. This article highlights their benefits and potential drawbacks from various perspectives. It achieves this by explaining how benzodiazepines work in terms of neuroendocrinology, immunomodulation, sleep, anxiety, cognition, and addiction, ultimately improving their clinical effectiveness. Benzodiazepines play a regulatory role in the HPA axis and impact various systems, including

Qualitative and quantitative analyses of polysomnographic measurements in foals.

Veterinary and human medicine are still seeking a conclusive explanation of the function of sleep, including the change in sleep behaviour over the course of an individual's lifetime. In human medicine, sleep disorders and abnormalities in the electroencephalogram are used for prognostic statements, therapeutic means and diagnoses. To facilitate such use in foal medicine, we monitored 10 foals polysomnographically for 48 h. Via 10 attached cup electrodes, brain waves were recorded by electroencephalography, eye movements by electrooculography and muscle activity by electromyography. Wireless p

Cortical activity and sleep in the rat lateral hypothalamic syndrome.

Continuous EEG recordings were performed in lateral hypothalamic (LH) damaged rats from day 1 post-lesion when totally aphagic and adipsic until complete recovery of their feeding and drinking. During the early post-operative stage (days 1--3 post-lesion), LH rats showed a complete disorganization of their basal EEG which was characterized by two superimposed activities, a constant rapid low voltage activity modulated by high voltage low frequency waves. Except in the case of intense illumination, several arousing stimuli did not affect this activity. By day 4 post-lesion, the two overlapping

Common slow modulation of respiration, arterial blood pressure and cortical activity during sleep onset while napping.

Using healthy subjects, concomitant 30- to 60-s modulations of respiration, arterial blood pressure and EEG activity were found in 21 experiments about napping. Although mean arterial pressure (MAP) modulations above and below 1/30 Hz increased, in respiratory amplitude (RA) only the lower frequency components augmented significantly. This slow modulation of RA was found to be asymmetrical in time, the duration of RA decreasing parts in the modulation waves being 42% longer than the duration of RA increasing parts. The concomitance of the slow modulations in the different organ systems is acco

Nutritional Status and Sleep Quality Are Associated with Atrial Fibrillation in Patients with Obstructive Sleep Apnea: Results from Tokyo Sleep Heart Study.

The prevalence of obstructive sleep apnea (OSA) in patients with atrial fibrillation (AF) has been observed to be much higher than in control participants without AF. Limited data exist regarding the prevalence of AF in patients with OSA. The clinical characteristics, nutritional status, and sleep parameters associated with AF in patients with OSA remain unclear. In this study, we aimed to determine the prevalence and factors associated with AF in patients with OSA from a large Japanese sleep cohort (Tokyo Sleep Heart Study). This was a single-center explorative cross-sectional study. Between

The relationship between anxiety, depression, daytime sleepiness in the REM-related mild OSAS and the NREM-related mild OSAS.

INTRODUCTION: Obstructive sleep apnea syndrome (OSAS) is a common form of sleep-related respiratory disease characterized by recurrent blockages in the upper airway. Rapid eye movement (REM)-related OSAS is a condition in which apneas and hypopneas are more common during REM sleep. We investigated whether there was any difference between REM-related mild OSAS group and NREM-related mild OSAS group in terms of anxiety, depression, and daytime sleepiness. METHODS: A total of 166 patients with mild OSAS (72 patients with REM-related and 94 NREM-related OSAS) participated in the study. Hospital A

Electrophysiology of a slow (0.5-4 Hz) intrinsic oscillation of cat thalamocortical neurones in vivo.

1. Electrophysiologically identified thalamocortical neurones have been intra- and extracellularly recorded in acutely prepared cats, under different anaesthetic conditions. 2. A slow (0.5-4 Hz) membrane potential oscillation was observed in thalamocortical cells recorded in motor, sensory, associational and intralaminar thalamic nuclei. The oscillation consisted of rhythmic low-threshold spikes alternating with after-hyperpolarizations. 3. About 80% of the neurones with intact cortical connections were set into the slow oscillatory mode by bringing their membrane potential to between -68 and

[EEG studies in rats to determine the threshold levels of chlorpromazine with respect to the state of vigilance].

Taking into account the actual vigilance status of the animals (awake, drowsiness, slow-wave-sleep) we were able to show EEG changes at lower chlorpromazine levels (0.42 mumole/kg b.w., i.p.) in contrast to 1.4 mumole/kg b.w., i.p.) in the model "without considering the vigilance status". These changes were especially pronounced during the awake state.

A review of sleepwalking (somnambulism): the enigma of neurophysiology and polysomnography with differential diagnosis of complex partial seizures.

The goal of this report is to review all aspects of sleepwalking (SW), also known as somnambulism. Various factors seem to initiate SW, especially drugs, stress, and sleep deprivation. As an etiology, heredity is important, but other conditions include thyrotoxicosis, stress, and herpes simplex encephalitis. Psychological characteristics of sleepwalkers often include aggression, anxiety, panic disorder, and hysteria. Polysomnographic characteristics emphasize abnormal deep sleep associated with arousal and slow wave sleep fragmentation. In the differential diagnosis, the EEG is important to pr

Effects of intraventricular implantation of crystalline estradiol benzoate on the sleep-wakefulness circadian rhythm of ovariectomized rats.

The effect of estrogen on the sleep-wakefulness circadian rhythm was examined in ovariectomized rats. Implantation of crystalline estradiol benzoate (EB) into the third cerebral ventricle reduced the slow wave sleep (SWS) and paradoxical sleep (PS) appearances within the dark period. This result was similar to that obtained by a systemic administration of 20 micrograms of EB. The inhibition of sleep during the nighttime was more remarkable in PS than in SWS. The reduction of SWS was due to a shorter duration of SWS episodes and the reduction of PS to a fewer number of PS episodes. These result
