[Circadian rhythm disturbance after radiotherapy for brain tumor in infantile period--clinical effect of L-thyroxine and vitamin B12].

We reported here 19-year-old man suffering from circadian sleep-wake (S-W) rhythm disturbance after total tumor resection and whole brain irradiation. This 19-year-old man was diagnosed as having astrocytoma in the right temporal lobe by CT scan and angiography at the age of 6 months. After total tumor resection and whole brain irradiation (60Co 60 Gy), he showed profound psychomotor retardation, endocrinologic dysfunction including hypothyroidism and growth hormone deficiency, and sleep-wake rhythm disturbance. At the age of 19, brain MRI revealed asymmetrical low intensity in the hypothalami

Sleep and rhythm consequences of a genetically induced loss of serotonin.

BACKGROUND: A genetic deficiency in sepiapterin reductase leads to a combined deficit of serotonin and dopamine. The motor phenotype is characterized by a dopa-responsive fluctuating generalized dystonia-parkinsonism. The non-motor symptoms are poorly recognized. In particular, the effects of brain serotonin deficiency on sleep have not been thoroughly studied. OBJECTIVE: We examine the sleep, sleep-wake rhythms, CSF neurotransmitters, and melatonin profile in a patient with sepiapterin reductase deficiency. PATIENT: The patient was a 28-year-old man with fluctuating generalized dystonia-par

Chronobiological aspects of narcolepsy.

The circadian, circasemidian and ultradian features of sleep-wake patterns in narcolepsy are reviewed with respect to the hypotheses that chronobiological disturbances might either be specific to the condition or contribute to its symptomatic profile. The position is taken that, while there are characteristic temporal features associated with the disorder, the chronobiological system is essentially intact, although quantitatively altered. It appears that the fundamental physiopathogenesis involves state boundary control mechanisms, which operate in state transitions, rather than chronobiologic

Sleep Disturbances Following Traumatic Brain Injury.

Sleep disorders commonly complicate the course following traumatic brain injury (TBI). Insomnia, excessive daytime somnolence and alteration of the sleep-wake schedule are common disturbances that affect the course of recovery and prognosis in TBI survivors. Few studies, however, have looked at the diagnosis and management of these disturbances in TBI. Early treatment of sleep disorders must be considered an integral part of the rehabilitation process. Recognition and management of comorbid medical or surgical diseases, assessment and treatment of associated psychiatric disorders, and awarenes

A cross-syndrome cohort comparison of sleep disturbance in children with Smith-Magenis syndrome, Angelman syndrome, autism spectrum disorder and tuberous sclerosis complex.

BACKGROUND: Sleep disturbance is common in children with neurodevelopmental disorders, with high rates identified in children with Smith-Magenis syndrome (SMS), Angelman syndrome (AS), autism spectrum disorder (ASD) and tuberous sclerosis complex (TSC). Phenotypic sleep profiles for these groups may implicate different pathways to sleep disturbance. At present, cross-group comparisons that might elucidate putative phenotypic sleep characteristics are limited by measurement differences between studies. In this study, a standardised questionnaire was administered across groups affording comparis

Sleep time and SOREMPs features support idiopathic hypersomnia and narcolepsy type 2 as a single disease entity.

STUDY OBJECTIVES: Idiopathic hypersomnia (IH) and narcolepsy type 2 (NT2) are rare hypersomnolence disorders with unknown pathophysiology. Recent evidence suggests they may belong to the same spectrum. This study compared objective sleep duration and REM sleep features, including sleep-onset REM periods (SOREMPs), during a 32-h continuous polysomnography (PSG) in patients with confirmed NT2 or IH. METHODS: Consecutive drug-free patients with sleepiness complaints underwent baseline PSG and multiple sleep latency test (MSLT) and were included if their mean sleep latency was ≤8 min, with a clea

Actigraphy-derived circadian rhythms, sleep-wake patterns, and physical activity across clinical stages and pathophysiological subgroups in young people presenting for mental health care.

Staging models for youth mental health aim to locate clinical presentations on a spectrum from at-risk states to persistent disorder and predict future illness trajectories. Our previous publications on trans-diagnostic staging proposed three pathophysiological subgroups of major mood or psychotic disorders in youth ('hyperarousal-anxious depression', 'circadian-bipolar spectrum', 'neurodevelopmental-psychosis'). This study aims to investigate differences in objective measures of 24hr sleep-wake patterns, circadian rhythms, and physical activity across clinical stages and pathophysiological su

A New Perspective on the Treatment of Alzheimer's Disease and Sleep Deprivation-Related Consequences: Can Curcumin Help?

Sleep disturbances, as well as sleep-wake rhythm disorders, are characteristic symptoms of Alzheimer's disease (AD) that may head the other clinical signs of this neurodegenerative disease. Age-related structural and physiological changes in the brain lead to changes in sleep patterns. Conditions such as AD affect the cerebral cortex, basal forebrain, locus coeruleus, and the hypothalamus, thus changing the sleep-wake cycle. Sleep disorders likewise adversely affect the course of the disease. Since the sleep quality is important for the proper functioning of the memory, impaired sleep is assoc

Sleep disorders in Prader-Willi syndrome, evidence from animal models and humans.

Prader-Willi Syndrome (PWS) is a complex genetic disorder with multiple cognitive, behavioral and endocrine dysfunctions. Sleep alterations and sleep disorders such as Sleep-disordered breathing and Central disorders of hypersomnolence are frequently recognized (either isolated or in comorbidity). The aim of the review is to highlight the pathophysiology and the clinical features of sleep disorders in PWS, providing the basis for early diagnosis and management. We reviewed the genetic features of the syndrome and the possible relationship with sleep alterations in animal models, and we describ

Managing sleep issues in Parkinson's disease: an up-to-date review.

INTRODUCTION: In Parkinson's disease (PD), sleep-wake problems are disease-related symptoms that occur throughout the day and have a negative impact on patients' quality of life to an extent that is equal to or greater than that of typical motor symptoms. AREAS COVERED: Insomnia due to fragmented sleep and excessive daytime sleepiness (EDS) worsen as PD progresses. Nighttime wearing-off and early morning-off should be considered first when fragmented sleep is reported in PD patients. If the main complaint of patients with insomnia is difficulty falling asleep, restless legs syndrome should be

Sleep characteristics in child and adolescent offspring of parents with bipolar disorder: a case control study.

BACKGROUND: Impairment of sleep and circadian rhythm is a typical feature of bipolar disorder (BD). We carried out an exploratory cross-sectional case-control study to extend the knowledge of sleep characteristics in offspring at risk for BD. METHODS: We investigated 42 offspring of bipolar parents (OB) (mean age 12.5 ± 3.2) and 42 sex and age matched comparison offspring of healthy parents. We administered the Pediatric Sleep Questionnaire, the Morningness/Eveningness Questionnaire and The General Behavior Inventory Sleep Subscale (GBISS) to assess circadian preference, and to identify sleep

Domains included in multidimensional sleep health composite scores: A scoping review.

While the SATED/Ru-SATED sleep health framework is well-recognized, there is no consensus for how many or which domains should be included in the construct of a multidimensional sleep health composite score. Therefore, the purpose of this scoping review is to determine what domains are included in multidimensional sleep health composite scores and how those domains are assessed. The Preferred Reporting Items for Systematic Reviews and Meta-Analyses extension for scoping reviews (PRISMA-ScR) and the Joanna Briggs Institute's (JBI) updated methodology for scoping reviews were used. Sleep Disorders: The Status and Clinical Significance.

This article presents an overview of sleep as a vital physiologic state that supports homeostasis, cognitive performance, and overall health. It reviews the epidemiology of sleep, emphasizing standard sleep patterns and the population burden of sleep disorders. The article then summarizes the evolution of sleep medicine, from descriptive clinical observation to experimental and electrophysiological research that established the neural basis of sleep. It concludes with an overview of contemporary management of sleep-related breathing disorders and the role of interdisciplinary sleep care.

Sleep Fragmentation in TcMAC21 Mouse Model of Down Syndrome.

BACKGROUND / OBJECTIVE: Down syndrome (DS) is the most common chromosomal disorder worldwide, and approximately ¾ of individuals with DS demonstrate multifactorial sleep disturbances, including sleep apnea. As the effects of chromosome 21 triplication are complex, mouse models may provide valuable insights into the causal mechanisms of disordered sleep in DS. Although the recently developed transchromosomic TcMAC21 mouse model offers the closest genetic similarity to human DS, its sleep-wake architecture is unexplored. We hypothesized that TcMAC21 mice would exhibit sleep disruption similar to

Just a scary dream? A brief review of sleep terrors, nightmares, and rapid eye movement sleep behavior disorder.

The clinical spectrum of sleep disorders in children is broad, ranging from primary snoring and obstructive sleep apnea (OSA) syndrome to complex sleep-related behaviors and movement disorders. Although snoring and OSA typically receive significant attention and discussion, other biologically based sleep disorders are as common, if not more common, in children. A general pediatrician is frequently presented with the complaint of sleep talking, sleep walking, or abnormal movements during sleep. Even more alarming is the presentation of the child suddenly and explosively screaming during sleep.

Advanced sleep phase syndrome: Role of genetics and aging.

Advanced sleep phase (ASP) is seldom brought to medical attention because many individuals easily adapt to their early chronotype, especially if it emerges before the age of 30 and is present in a first-degree relative. In this case, the disorder is considered familial (FASP) and is mostly discovered coincidentally in the presence of other sleep disorders, mainly obstructive sleep apnea syndrome (OSAS). The prevalence of FASP is currently estimated to be between 0.21% and 0.5%. Autosomal dominant mutations in circadian clock genes like PER2, CK1, PER3, CRY2, TIMELESS, and DEC2 have been linked
