Duodenal leiomyoma: a case report of upper gastrointestinal hemorrhage from a giant duodenal tumor.

INTRODUCTION: Duodenal leiomyoma is a rare tumor, originating in the intestinal wall smooth muscle, making up <0.1% of all gastrointestinal tumors. The majority are discovered incidentally on endoscopy due to the small size of tumors at presentation. Larger duodenal leiomyomas may present with anemia from gastrointestinal hemorrhage. CASE PRESENTATION: A 67-year-old male presented with life-threatening upper gastrointestinal hemorrhage. Magnetic resonance imaging identified an 11.9 × 8.7 cm submucosal mass in the second portion of the duodenum adjacent to the pancreatic head. An urgent upper

Primary Retrobulbar Leiomyosarcoma in a Dog: A Case Report.

A 2-year-old female Mongrel dog weighing 3.12 kg presented with a 2-month history of progressive exophthalmos of the left eye and periorbital swelling. Fine-needle aspiration cytology of the affected tissue revealed atypical cells of suspected malignant mesenchymal tumor origin. Computed tomography and magnetic resonance imaging revealed an ill-demarcated soft tissue mass in the left retrobulbar space extending into the nasal cavity and into the frontal lobes of the brain with destruction of the adjacent cribriform plate and the basisphenoid bone. Histopathological features of the tumor were c

Primary iris leiomyoma.

Intraocular leiomyomas are uncommon and usually occur in the ciliary body. Primary leiomyoma of the iris is both rare and a difficult diagnosis to make, given melanocytic tumors are more common and may be amelanotic. The somewhat controversial diagnosis of iris leiomyoma requires further confirmation by immunohistochemistry and electron microscopy. Herein, we describe a 58-year-old man with a 2-mm round translucent pink lesion of the iris. The tumor was excised by sector iridectomy. Immunohistochemistry showed positivity for both smooth muscle actin and desmin and negativity for S-100, HMB45,

Use of myocardin in the classification of mesenchymal tumors of the uterus.

It can sometimes be difficult to distinguish between the 2 main types of uterine mesenchymal neoplasms; uterine smooth muscle tumors (SMTs) and endometrial stromal sarcomas (ESSs), particularly when the ESSs show smooth muscle differentiation or the SMTs are highly cellular. The aim of this study was to investigate myocardin expression in normal uterus myometrium, in SMTs, and in ESSs and to determine whether myocardin can be used as a useful diagnostic tool in the classification of problematic uterine mesenchymal tumors. Immunohistochemical staining was performed in each group. Besides myocar

Angiomyolipomatous Lesions of the Nasal Cavity (Sinonasal Angioleiomyoma with Adipocytic Differentiation): A Multi-Institutional Immunohistochemical and Molecular Study.

PURPOSE: Mesenchymal neoplasms composed of vascular, smooth muscle, and adipocytic components are uncommon in the nasal cavity. While angioleiomyoma (AL) is a smooth muscle tumor in the Head & Neck WHO classification, it is considered of pericytic origin in the Skin as well as Soft Tissue and Bone classifications. For nasal AL with an adipocytic component, the terms AL with adipocytic differentiation and angiomyolipoma (AML) have been applied, among others. AML is a type of perivascular epithelioid cell tumor (PEComa), most often arising in the kidney, sometimes associated with the tuberous sc

Adenomyomatous polyp of the endometrium: a case report.

BACKGROUND: Adenomyomatous polyps of the endometrium are rare, pedunculated lesions that contain smooth muscle in addition to the usual components of an endometrial polyp. Grossly, adenomyomatous polyps have the same appearance as an ordinary endometrial polyp and are characterized histologically by the intertwining of smooth muscle, endometrial glands and endometrial stroma. CASE: A 32-year-old woman underwent a hysterectomy for refractory menometrorrhagia and was found to have a large, sessile, polypoid mass completely filling the endometrial cavity, consisting of multiple, thick-walled ves

Novel SRF-ICA1L Fusions in Cellular Myoid Neoplasms With Potential For Malignant Behavior.

Pericytic tumors comprise a histologic continuum of neoplasms with perivascular myoid differentiation, which includes glomus tumors, myopericytoma, myofibroma, and angioleiomyoma. Despite their morphologic overlap, recent data suggest a dichotomy in their genetic signatures, including recurrent NOTCH gene fusions in glomus tumors and PDGFRB mutations in myofibromas and myopericytomas. Moreover, SRF-RELA fusions have been described in a subset of cellular variants of myofibroma and myopericytoma showing myogenic differentiation. Triggered by an index case of an unclassified cellular myoid tumor

Uterineintravenous leiomyomatosis with cardiac involvement.

Intravenous leiomyomatosis is a histologically benign smooth muscle tumor that arises either by direct extension of a uterine leiomyoma into the adjacent veins or by vascular intimal smooth muscle proliferation. Herein, we report the case of a 60-year-old female who was noted to have suspected cardiac mass on elective echocardiography done electively for abnormal electrocardiographic findings. Computed tomography and cardiac magnetic resonance indicated the presence of an intravenous leiomyoma originating from the uterus and extending to the inferior vena cava and right atrium. The patient was

Modification of hyaluronan by heavy chains of inter-α-inhibitor in idiopathic pulmonary arterial hypertension.

We previously reported an altered hyaluronan (HA) metabolism in idiopathic pulmonary arterial hypertension (IPAH) lung tissue and cultured smooth muscle cells. Hyaluronan was present in the smooth muscle cell layer surrounding the pulmonary vasculature and in plexigenic lesions. Additionally, cultured pulmonary artery smooth muscle cells produced spontaneous HA "cable" structures, without additional stimuli, that were leukocyte-adhesive. We now present evidence that the HA that accumulates in IPAH plexigenic lesions is a pathological form of HA in which heavy chains (HCs) from the serum-derive

The carcinoid endocardial plaque; an ultrastructural study.

Ultrastructural studies disclosed that the plaque-like endocardial thickenings in three patients with the carcinoid syndrome were composed of smooth muscle cells embedded in a stroma that was rich in acid mucopolysaccharides, collagen, and microfibrils, but devoid of elastic fibers. The smooth muscle cells contained variable numbers of myofilaments and cisterns of rough surfaced endoplasmic reticulum, and their basement membranes were greatly thickened, reduplicated, and arranged in layers. The endocardial plaques appeared histologically and ultrastructurally similar regardless of their locati

Simultaneously occurring disseminated peritoneal leiomyomatosis and multiple extrauterine adenomyomas following hysterectomy.

Disseminated peritoneal leiomyomatosis (DPL), also known as leiomyomatosis peritonealis disseminata, is a rare condition characterized by multiple benign smooth muscle tumors proliferating along the peritoneal surfaces. In previous case reports, these tumors have been noted to involve the ovaries, round ligaments, bladder, bowel, peritoneum, and mesentery. To date, approximately 150 cases of DPL have been described in the literature. Extrauterine adenomyoma is an even rarer entity, involving benign tumors composed of smooth muscle tissue, endometrial glands, and endometrial stroma arising outs

Smooth muscle neoplasms of the vulva masquerading as Bartholin gland duct cysts.

Smooth muscle neoplasms of the vulva can be mistaken for Bartholin duct cysts, which can lead to a delay in diagnosis. We present a case of vulvar leiomyoma and a case of leiomyosarcoma that clinically mimicked Bartholin duct cysts. Identification of leiomyosarcomas in this region is particularly important; due to the risk of recurrence, patients may need radiation and/or chemotherapy in addition to adequate surgical treatment and appropriate follow up. Prior series have shown that risk of recurrence is related to inadequate resection and not to the size or grade of tumor. It is critical that

[Leiomyosarcoma of larynx and hypopharynx].

The laryngeal and hypopharyngeal smooth muscle tumors are very rare. One case of hypopharyngeal leiomyosarcoma and two cases of laryngeal smooth muscle tumors were reported: one angioleiomyoma, one leiomyosarcoma and one carcinoleiomyosarcoma. The patients usually present no characteristic symptoms in early stage and the clinical symptoms are similar to squamous cell carcinoma. It is difficult to make a correct diagnosis before operation. Treatment of first choice consists of primary radical resection including a large part of normal tissue. Early lymph node metastasis is rare. Precautary neck

Lipomatous tumors of the anterior mediastinum with muscle differentiation: a clinicopathological and immunohistochemical study of three cases.

Three cases of primary lipomatous tumors of the anterior mediastinum with prominent muscle differentiation are presented. The patients were two women and one man between the age of 52 and 68 years. All patients presented with progressive shortness of breath. Radiographically, all patients demonstrated anterior mediastinal tumors which were surgically resected. In two cases, the gross findings were those of circumscribed tumors, while one lesion was described as an ill-defined and unencapsulated neoplasm. Histologically, two cases corresponded to a well-differentiated liposarcoma, while the thi

Electron microscopy in the study of myofibroblastic lesions.

Electron microscopy in the diagnosis and academic study of myofibroblastic lesions is discussed. Myofibroblasts from granulation tissue and tumor stroma are regarded as the nearest equivalent to a "normal" myofibroblast population with which to define myofibroblastic differentiation in tumoral and pseudotumoral lesions. Histological features include a plump-spindle-cell morphology, with an ill-defined cytoplasm paler and less fibrillar than in smooth-muscle cells, and matrix collagen. Myofibroblasts stain for alpha-smooth-muscle actin, fibronectin, and vimentin. Desmin is found in some lesiona

Uterine sarcoma with KAT6B/A::KANSL1 fusion: a molecular and clinicopathological study on 9 cases.

Uterine sarcomas with KAT6B/A::KANSL1 fusion represent a new entity characterized by bland morphology, commonly with hybrid features of low-grade endometrial stromal sarcoma (LG-ESS) and tumors with smooth muscle differentiation. In our study, we performed a detailed morphological, immunohistochemical, and molecular analysis of 9 cases of these tumors. Six of those had been originally diagnosed as LG-ESS, one as leiomyoma, one as leiomyosarcoma, and the remaining case as sarcoma with the KAT6B/A::KANSL1 fusion. Seven cases showed overlapping features between endometrial stromal and smooth musc

Pathological features and clinical associations of 58 small incidental angiomyolipomas of the kidney.

Although angiomyolipoma (AML) of the kidney is a well-described and broadly studied neoplasm, little is known about clinical and morphological features of small incidental tumors occurring sporadically. Sections from 58 small AMLs found in 37 patients from 402 consecutive unselected autopsies were studied. A female predominance and a bimodal age distribution (30-50 and >70 years) were observed. No AML was found in a prepubertal patient. Six patients had multiple and bilateral AMLs (2-8 tumors per patient). Overall, the tumors ranged in size from 0.1 to 10 mm (mean=1.5 mm) and were variably com

Renal Leiomyoma in a Female Adult: A Histopathological Case Report.

Renal leiomyomas are rare benign mesenchymal tumors that arise from the smooth muscle cells in the renal capsule, renal pelvis, and the smooth muscles of vessels in the kidney. They are usually found by accident during autopsies or on different imaging modalities made on other occasions. The clinical presentation may include hematuria and abdominal or flank pain, although renal leiomyomas are most frequently asymptomatic. This case concerns a 45-year-old woman with an asymptomatic tumor mass in the left kidney, discovered incidentally on an outpatient CT scan conducted on an unrelated occasion
