BACKGROUND: Skin and soft tissue infections (SSTIs) are a leading cause for hospitalizations in the United States. Few studies have addressed the appropriateness of antibiotic therapy in the management of SSTIs without complicating factors. We aimed to determine the appropriateness of antibiotic treatment duration for hospitalized adult patients with uncomplicated SSTIs.
METHODS: This was a retrospective analysis performed at two academic medical centers in Pittsburgh, Pennsylvania on patients aged 18 years and older with primary ICD-9 code for SSTIs admitted August 1st, 2014-March 31st, 2015…

BACKGROUND: Uncomplicated skin and soft tissue infections are among the most frequent indications for outpatient antibiotics. A detailed understanding of current prescribing practices is necessary to optimize antibiotic use for these conditions.
METHODS: This was a retrospective cohort study of children and adults treated in the ambulatory care setting for uncomplicated cellulitis, wound infection, or cutaneous abscess between March 1, 2010 and February 28, 2011. We assessed the frequency of avoidable antibiotic exposure, defined as the use of antibiotics with broad gram-negative activity, co…

BACKGROUND: Necrotizing soft tissue infections often are characterized by fulminant presentation and lethal outcomes. Besides critical care support and antibiotic therapy, aggressive surgical treatment is important for the therapy of necrotizing fasciitis. The aim of this study was to develop a procalcitonin (PCT) ratio indicating successful surgical intervention.
METHODS: The study group consisted of 38 patients treated with clinical signs of sepsis caused by a necrotizing soft tissue infection. All patients received radical surgical treatment, and serum levels of PCT and C-reactive protein …

Peripheral intravenous (i.v.) catheter-related soft tissue infections begin with local skin and soft tissue inflammation, which can progress to cellulitis or even tissue necrosis requiring aggressive surgical treatment. We conducted a matched case-control study to investigate risk factors for peripheral i.v. catheter-related soft tissue infections in hospitalised patients. We retrospectively identified 46 cases that occurred during 2006-2008 in two teaching hospitals. Each case was randomly matched with four control subjects from the same ward and on the same day that the soft tissue infection…

Tissue infections or skin, skin structure, and deep seated soft tissue infections are general terms for infections of the entire skin layer including the subcutaneous and muscle tissue layers and their respective fascia structures. Infections of the different mediastinal fascias (mediastinitis) and retroperitoneal fascia infections also belong to this category. Due to the variability of their clinical presentation, skin and soft tissue infections can be classified according to different features. The following aspects can be used for classification: - anatomical structures, - pathogens, - nece…

Patients with severe infections of the potential spaces of the head and neck are commonly managed in the ICU. These infections may present with devastating complications such as airway obstruction, jugular septic thrombophlebitis, lung abscess, upper airway abscess rupture with asphyxiation, mediastinitis, pericarditis, and septic shock. A thorough understanding of the anatomy and microbiology of these infections is essential for proper management of these patients. Retropharyngeal, danger, prevertebral, lateral pharyngeal, and submandibular space infections and their site-specific clinical ma

Necrotizing soft-tissue infections (NSTI) are often life-threatening illnesses that may be best treated at specialty care facilities such as burn centers. However, little is known about current treatment patterns nationwide. The purpose of this study was to describe the referral patterns for treatment of NSTI using a multistate discharge database and to investigate the differences in patients with NSTIs treated at burn centers and nonburn centers. The National Inpatient Sample is an all-payer inpatient database from 37 states containing data from 14 million hospital stays each year. We identif

OBJECTIVES: Hyperbaric oxygen (HBO) therapy is often combined with antibiotic therapy for infections such as gas gangrene and osteomyelitis. Although numerous investigations have been undertaken to assess the effect of adjunctive HBO therapy on the treatment of infections, the bout of treatment has not been referred in the previous investigations. The purpose of this retrospective study was to evaluate the efficacy of HBO therapy on the bout of treatment for soft tissue infections.
PATIENTS AND METHODS: In the period between 1994 and 2001, we treated 23 patients with soft tissue infections. N…

After detection of a bacterial infection of the joint, an absolute indication for intervention is given. Systemic antibiotic drug therapy is indicated and drainage of the joint has to be performed immediately. The following therapeutic algorithm regimen is a proven remedy in treating pyoarthrosis of the knee joint: During the initial period, the infection can be controlled by arthroscopic irrigation and systemic antibiotic therapy. Depending on the progredient findings or if the symptoms persist, curing the joint by open synovectomy is the next step of treatment. If open joint revision includi

A case of acute appendicitis complicated with necrotizing soft tissue infections (NSTIs) in an extremely elderly woman (98 years-old) is reported. She was admitted to our hospital with a history of increasing pain localized in the right lower abdomen. Abdominal ultrasonography revealed appendicolithiasis and periappendiceal fluid. An appendectomy and closure of the perforated cecum was performed. On the 5th post-operative day, the patient developed skin erythemas and crepitation in the right lower quadrant. An exploration and drainage of the recent operative incision was performed under the di…

Cellulitis, infected wounds, abscesses, and other bacterial infections of skin and soft tissue are commonly seen in the practice of emergency medicine. Informal observations in a teaching hospital suggest that newer agents having a broader antibiotic spectrum are frequently used for empiric outpatient treatment of mild to moderate soft tissue infections, so information from a prospective study was analyzed to determine whether this practice is appropriate. Healthy adult volunteers were enrolled in a study of the aerobic bacteriology and response to treatment of skin and soft tissue infections.…

Six soft tissue infections (three epiglottitis, one cellulitis, one pneumonia, and one arthritis) with ampicillin-resistant Haemophilus influenzae were treated initially with high doses of ampicillin (200 to 400 mg/kg/day intravenously) alone and had good clinical responses. All had documented bacteremia with H. influenzae. One child was treated only with ampicillin; treatment in the remainder was changed to oral therapy with other antibiotics to facilitate discharge. There was no recurrence of disease. Disc diffusion studies done on clinical isolates of both resistant and sensitive organisms …

Soft tissue and osteo-articular infections are rarely seen in patients with HIV infection and other immunodeficiency states. When present in HIV-infected patients, they tend to occur in the presence of low CD4(+)cell counts, intravascular indwelling catheters, extra-articular infection and trauma, and in intravenous drug users and haemophiliacs. A wide spectrum of clinical manifestations is seen, ranging from cellulitis and soft tissue abscesses to septic arthritis and pyomyositis. In general, the clinical picture and response to therapy is similar to that of patients without HIV infection. Ca

Necrotizing soft-tissue infections (NSTIs) are acute, rapidly progressive infections of soft tissues, often associated with signs of severe systemic toxicity and progressive multi-organ failure. This rapid, often fatal progression of local disease mandates timely diagnosis. Delays in diagnosis and treatment may have dire consequences. Effective treatment includes appropriate debridement associated with directed antibiotic therapy and hemodynamic support. Early surgical debridement with complete removal of necrotic tissue, including potential amputation, is essential to decrease mortality and o

BACKGROUND: Perirectal abscess is encountered commonly in surgical practice, and simple incision and drainage usually is sufficient. However, life-threatening sepsis may occur. In such instances, the high morbidity and mortality rates from the severe systemic inflammatory response syndrome (SIRS) associated with rapidly evolving necrotizing and gas-forming soft tissue infections are of major concern.
METHODS: Case report and review of pertinent literature.
RESULTS: A rare case of extensive necrotizing fasciitis of the torso arising from a perirectal abscess, in which the principles of radica…

BACKGROUND: Necrotising soft tissue infection is a rare and severe infectious condition affecting the skin and underlying soft tissues. Since the COVID-19 pandemic, an increase has been reported internationally in cases caused by Streptococcus pyogenes (Group A Streptococcus, GAS). We have investigated the incidence, aetiology and resistance patterns of necrotising soft tissue infections in the head and neck region at the Department of Otorhinolaryngology, Oslo University Hospital, Rikshospitalet for the period 2017-2023.
MATERIAL AND METHOD: Retrospective review of medical records of patient…

This report describes a multiparous woman in her mid-40s who underwent an abdominal myomectomy for abnormal uterine bleeding due to fibroids and desire for fertility preservation which resulted in the rare consequence of uterine loss due to severe infectious complications postoperatively. Despite receiving prophylactic clindamycin at the index surgery, her postoperative course was complicated by an abdominal wall abscess as well as necrotising soft tissue infection involving the uterus. Management included parenteral antibiotics and a supracervical hysterectomy with bilateral salpingo-oophorec
