Sella turcica anatomy by three-dimensional computed tomography for an endonasal transsphenoidal approach to pituitary adenoma.

BACKGROUND: The aim of this study was to identify the anatomic landmarks of sella turcica on the surface of the head, and to investigate the feasibility of studying sella turcica anatomy individually by three-dimensional computed tomography (3D-CT) before an endonasal transsphenoidal operation. PATIENTS AND METHODS: The three-dimensional anatomic structures of the heads of 49 patients were studied by 3D-CT using image reconstruction with surgical clip image registration. The tip of the nose and the apex of the ear helix were used as surface markers on the head, and the locations of the spheno

[Anatomical study on the sphenoidal sinus and petrous apex cells].

Morphological relation between the sphenoidal sinus and the petrous apex cell was studied using HRCT images obtained by 339 adults and 43 infants. The adults were classified into normal group (without chronic otitis media or its surgery) 237 cases, and otitis group (chronic otitis media or its post. op.), 102 cases. The posterior extent of the sphenoidal sinus was evaluated in reference to the carotid canal, and the petrous apex cells were evaluated by their mode of distribution in the apex. In the normal adults, no evident relation was seen between the posterior extent and the mode of distrib

Primary Ewing Sarcoma in the Sphenoidal Sinus: A Case Report.

Ewing's sarcoma (EWS) is a rare malignant bone tumor that primarily affects children and young adults. While it typically affects long bones, it can occur in the head and neck region, including the paranasal sinuses in rare cases. We present a challenging case of a 45-year-old female diagnosed with an EWS of the sphenoidal and the right nasal fossa. A subtotal removal of the mass was performed on the patient followed by combined adjuvant radiotherapy and chemotherapy. Due to its rarity, the diagnosis and management of EWS in the paranasal sinuses are challenging.

Orbitocranial fibrous dysplasia. A case report.

A 14-year-old girl had right proptosis, facial asymmetry, and gradual increasing poor vision. X-ray findings showed sclerosis in the sphenoidal bone and the base of the skull. Partial excision of dysplastic bone, decompression of the optic canal, orbit, and superior orbital fissure, and reconstruction of the upper orbital wall were performed one year later. Histopathologic examinations disclosed fibrous dysplasia of sclerotic form. There has been no progression of the disease since the surgery, and good cosmetic and visual results had been achieved.

The surgical management of cranio-orbital neurofibromatosis.

Experience with the surgical management of cranio-orbital neurofibromatosis in 14 patients is reported (age range, 6-40 years). The skeletal abnormality of the orbit that occurs in a small proportion (less than 1%) of patients with neurofibromatosis is, in essence, the absence of the membranous portions of the sphenoid and the adjacent bone forming the boundaries between the cranium and the orbit. The goal of surgery is tumor resection and reconstruction of the posterior bony defect by bone graft. Two additional procedures are also described that better enhance aesthetically-the mask lift and

Ocular complications in craniofacial fibrous dysplasia.

Three cases of craniofacial fibrous dysplasia are reported, all presenting to the ophthalmologist first. They all had ocular complications due either to direct involvement of the orbital bones or to secondary complications. Sphenoidal mucocele is added to the other complications previously reported, and accordingly a classification of the various ways that fibrous dysplasia can affect the ocular structures is proposed.

Painful ophthalmoplegia secondary to a mucocele involving the sella turcica, superior orbital fissure, and sphenoid sinus.

A case of painful ophthalmoplegia associated with an extensive lesion involving the sella turcica, superior orbital fissure, and sphenoid sinus in a 57-year-old man is reported. Even though nasal and ocular symptoms and signs represent the usual features of sphenoidal mucoceles, extension to the intracranial cavity as seen in this lesion is rare. Surgical exploration via a sublabial, transseptal approach revealed a mucocele of the sphenoid sinus. This case exhibited extensive and aggressive behavior simulating a malignant neoplasm.

Neuroimaging findings of extensive sphenoethmoidal dysplasia in NF1.

Whereas isolated sphenoid wing dysplasia (SWD) is a well-known clinical feature in neurofibromatosis 1 (NF1), extensive cranial defects involving multiple bones have been rarely reported in this disorder. In this report, we describe the clinical course of a 20-year-old male with NF1 and an extensive cranial bone dysplasia. The large sphenoethmoidal defect was associated with transethmoidal and orbital cephalocele as well as inferolateral herniation of the frontal lobe. In spite of the large defect, the individual did not have any symptoms or complications resulting from the osteopathy. We revi

Management of Aggressive Recurrent Meningioma Using a Combined Transfacial-Pterional Approach.

Skull base meningiomas, although histologically benign, can exhibit locally aggressive behavior, particularly when arising from the sphenoid wing. Their potential to infiltrate bone and extend extracranially into the orbit, infratemporal fossa, and facial soft tissues significantly increases surgical complexity and recurrence risk. Complete resection in these cases requires wide anatomical exposure and multidisciplinary planning to preserve critical neurovascular structures while achieving oncologic control. We present the case of a woman with a recurrent sphenoid wing meningioma extending int

Reversible visual loss caused by fibrous dysplasia.

Fibrous dysplasia is a developmental anomaly of bone, often affecting the facial bones. We treated a patient who had fibrous dysplasia involving the right maxillary and sphenoid bones. The patient had a sudden loss of visual acuity to R.E.: counting fingers. Visual fields demonstrated a central scotoma, and retrobulbar neuritis was diagnosed. When vision failed to improve spontaneously, however, radiologic studies were performed. These showed compromise of the optic canal by bony proliferation and an apparent mucocele at the orbital apex. Surgical exploration disclosed fibrous dysplastic bone …

Isolated sphenoid sinusitis or mucocele: a potential complication of endonasal transsphenoidal surgery.

Rhinosinusitis/mucocele are rare complications of transsphenoidal surgery. We present a retrospective analysis of 323 patients who received transsphenoidal surgery for pituitary adenoma. Twenty of the 323 patients (6.2%) developed rhinosinusitis/mucocele after transsphenoidal surgery as shown by MRI. All 20 patients with rhinosinusitis/mucocele occurred in the group who received the small sphenoidotomy approach and simple postoperative nasal care. Medical management was successful in 13 of 20 cases (65%). The remaining seven patients received endoscopic sphenoidotomy. On re-operation, purulent

Extracranial skull base chondrosarcoma.

Chondrosarcoma of the skull base is a rare neoplasm. The most common presenting symptoms are hoarseness, dysphagia, and diplopia, which is associated with palsies of cranial nerves X, IX, and VI. The temporal bone is the most common site of tumor origin, followed by the sphenoid bone. These tumors must be differentiated from chordoma, chondroid chordoma, osteogenic sarcoma, enchondroma, and meningioma. Diagnosis is made by patient history, radiologic imaging, and biopsy. The treatment is surgical excision followed by radiation therapy. Five-year survival rates for grades I, II, and III are 90,…

Confounding features of ectopic craniopharyngioma: A differential for tumors showing diffusion restriction.

Craniopharyngiomas comprise approximately 1-3% of all intracranial tumors and arise from squamous epithelial rests along remnants of Rathke's cleft. They have bimodal age distribution. The ectopic sites reported are the nasopharynx, sphenoid bone, pineal region, cerebellopontine angle and third ventricle. We report a case of ectopic craniopharyngioma in the Foramen of Monro with restricted diffusion, which have is an unusual feature, not commonly reported and is a potential pitfall.

Tuberculous osteomyelitis and optic neuritis.

This case report describes a rare instance of bilateral optic papillitis associated with histologically and bacteriologically proven tubercular osteomyelitis of the right sphenoid and temporal bone of the orbit in a 30-year-old woman. This case also highlights the usefulness of computerized axial tomography of the orbits, skull, and paranasal sinuses to evaluate optic neuritis and its pathophysiology in the event of no obvious cause or contributing systemic disease.

[Radiologic diagnosis of the orbit and anterior cranial fossa].

Basing on frequent diseases associated with typical changes of the orbita and the anterior cranial fossa - osteomas, fibrous dysplasia, meningiomas of the sphenoid bone, neurofibromatosis, mucoceles and pyoceles, dermoid tumours, malignant tumours and transmitted inflammatory processes - the x-ray diagnostic procedure is explained. Plain x-ray films and computed tomography are the most important methods, whereas examinations employing film tomography and angiography are now restricted to a few selected cases only.

OBJECTIVE: This study was conducted to develop a technique for endoscopic assistance during deep lateral orbital bone decompression. MATERI … g dura mater (DM) was observed. In 19 cases, reaching the cortical layer of the greater wing of the sphenoid bone adjacent to the DM was indicated by the appearance of thin cortical fragments in the projection of the middle cranial fossa oriented at an acute angle to the bone defect; in 5 of these cases, cortical fragments occupied almost the entire orbital sur …

Brain Perfusion Changes in a Patient with Facial Trauma.

A 69-year-old male was admitted to our hospital because of left facial trauma with bone fractures, including the maxillary sinus, zygomatic arch, and ethmoid and sphenoid bones. Brain computed tomography was unremarkable but regional cerebral blood flow with hexamethyl-propylene-amine oxime single-photon emission computed tomography (SPECT) showed hypoperfusion of the left hemisphere, which was reversible since a repeat SPECT 4 months later was substantially improved. Brain perfusion SPECT may provide information on cerebrovascular status in some cases of facial injury.
