Scintigraphic evaluation of bile dynamics before and after endoscopic sphincterotomy.

Endoscopic sphincterotomy has been used for the treatment of many biliary disorders, including extraction of obstructing common bile duct stones. Manometric studies of the sphincter of Oddi and the common bile duct have shown a marked decrease in biliary tract pressures after sphincterotomy. A prospective study of 12 patients with intact gallbladders using hepatobiliary imaging before and after sphincterotomy was performed to assess changes in bile dynamics. Nonfilling of the gallbladder occurred in 9 of the 12 patients after sphincterotomy, indicating significant alteration of bile flow. Such…

[Tomato skin in the common bile duct after endoscopic sphincterotomy].

Lithotomy and endoscopic sphincterotomy were carried out in a 83-year-old woman because of choledocholithiasis. One year later calculi and tomato skins were found in the common bile duct. Significance is attributed to the tomato skins in the development of calculi. The attention is drawn to the observation that operative interventions performed through the Vater's papilla, spontaneous or iatrogenic choledochoduodenal fistulas create favourable conditions for foreign material to get into the choledochus and this may cause lithogenesis.

Successful treatment of postoperative external biliary fistulas by endoscopic sphincterotomy.

External biliary fistulas occur as a complication of biliary tract surgery. Until recently, excision of the fistula was the primary therapeu … the remaining patient after an intraoperative biopsy of a carcinoma of the gallbladder. Endoscopic sphincterotomy is a reliable and effective therapeutic alternative in patients with external biliary fistulas.

Antegrade transcholecystic sphincterotomy: canine study of a new interventional technique.

Operative cholecystostomy and drainage tubes were placed in 5 dogs. Five weeks later, a steerable catheter was manipulated through the cholecystostomy and cystic duct into the common duct. An endoscopic diathermy catheter was placed through it into the duodenum and positioned at the papilla of Vater under fluoroscopic control for sphincterotomy. Results demonstrate that antegrade transcholecystic sphincterotomy is a feasible technique and can be performed safely. Antegrade sphincterotomy was then applied in the first human patient.

A preliminary report: urgent duodenoscopic sphincterotomy for acute gallstone pancreatitis.

Eleven patients wtih gallstone-related acute pancreatitis underwent urgent duodenoscopy and diathermy sphincterotomy; six had stones impacted at the papilla of Vater, and the remainder had stones in the bile duct. One patient had a gallstone in the pancreatic duct. Endoscopic removal of the duct stones resulted in a prompt, persistent clinical and biochemical improvement, except in one patient who had been referred from another hospital with a pseudocyst 3 weeks after the onset of illness. Pancreatography results were remarkably normal in the remaining patients. Use of emergency biliary decomp…

Endoscopic sphincterotomy in high-risk patients with biliary tract disease.

Endoscopic sphincterotomy was performed in 53 patients with biliary tract disease who were considered to be at high risk from conventional surgical procedures. Forty-five patients had choledocholithiasis, while eight had miscellaneous disorders including carcinoma, stenosis of the papilla, and postoperative biliary fistulae. Forty-three of 45 patients with choledocholithiasis have been free of symptoms since the procedure, and most have had repeat retrograde cholangiography confirming the absence of bile duct-stones. Endoscopic sphincterotomy is a safe and effective procedure in selected high-…

Non-operative removal of bile duct stones by duodenoscopic sphincterotomy.

Surgical exploration of the common bile duct carries considerable risks in elderly and frail patients. Peroral, fibreoptic duodenoscopy allows access to the papilla of Vater in sedated patients and the opportunity to perform a diathermy sphincterotomy for removal of bile duct stones. We have attempted sphincterotomy in 134 patients, most of whom had previously undergone cholecystectomy and had some contraindication to a further operation. Sphincterotomy was technically successful in 129 patients (96 per cent), and all duct stones were removed in 119 patients (92.5 per cent of successful sphinc…

Successful removal of retained bile duct stones by endoscopic sphincterotomy.

Two cases are reported in which endoscopic sphincterotomy was successful in allowing passage of stones retained in the common bile duct after cholecystectomy, performed in one case three weeks and in the other 14 years before.

Endoscopic Sphincterotomy Plus Large Balloon Dilation Versus Endoscopic Sphincterotomy Alone for Patients with Common Bile Duct Stones: A GRADE Evaluation of Systematic Review and Meta-Analysis.

INTRODUCTION: Endoscopic sphincterotomy (ES) and endoscopic papillary balloon dilation (EPBD) are established treatments for common bile duct (CBD) stones, each with limitations. Endoscopic sphincterotomy with large-balloon dilation (ESBD) combines both techniques, but its efficacy versus ES alone remains controversial. We conducted a systematic review and meta-analysis of randomized controlled trials (RCTs) comparing ESBD with ES alone for the management of CBD stones. METHODS: We searched PubMed, Scopus, Embase, Ovid, Web of Science, and Cochrane Library for RCTs comparing ESBD versus ES al…

Comparative study of conventional lateral internal sphincterotomy, V-Y anoplasty, and tailored lateral internal sphincterotomy with V-Y anoplasty in the treatment of chronic anal fissure.

BACKGROUND: Lateral internal sphincterotomy has been proven highly effective in curing anal fissure but with a high incidence of postoperative incontinence. OBJECTIVE: We compared conventional lateral internal sphincterotomy, V-Y advancement flap, and combined tailored lateral internal sphincterotomy with V-Y advancement flap in treating anal fissure. PATIENTS: Consecutive patients treated for anal fissure at our colorectal unit were evaluated for inclusion. Participants were randomly allocated to receive conventional sphincterotomy (GI), V-Y advancement flap (GII), or combined tailored late…

Endoscopic cystostomy and biliary sphincterotomy for choledochoceles: A pediatric case series.

Management of choledochoceles (type III choledochal cysts) in children varies. We highlight the potential role of endoscopic management of choledochoceles with cystostomy and biliary sphincterotomy through a series of three successfully treated pediatric patients aged 12-13 at our tertiary center. Patients presented with symptoms including abdominal pain and pancreatitis. Choledochoceles were identified by preprocedure imaging on magnetic resonance imaging, with sizes ranging between 7 and 15 mm in maximal diameter. Histology revealed two choledochoceles lined by biliary epithelium and one by …

Safety and success of precut biliary sphincterotomy: Is it linked to experience or expertise?

AIM: To determine the rates of success and complications of precut biliary sphincterotomy (PBS) based on prior experience and to compare the complication rates between PBS and standard endoscopic sphincterotomy (ES). METHODS: A retrospective evaluation of prospectively collected non-randomized data at an academic tertiary referral center. The study included all patients in an eight-year period who underwent PBS and ES by a single endoscopist who had no formal training in PBS. The main outcome measures of the study were success and complications of PBS with a comparison to complications of ES.…

An endoscopic sphincterotomy of the minor papilla in the management of symptomatic pancreas divisum.

BACKGROUND/AIMS: The efficacy of endoscopic treatment in pancreas divisum remains controversial. This study evaluated the results of an endoscopic sphincterotomy of the minor papilla and temporary transpapillary pancreatic stenting in patients with pancreas divisum. METHODOLOGY: Pancreas divisum was diagnosed in four patients between 1994 and 2004. All patients demonstrated episodes of recurrent upper abdominal and back pain were with a median follow-up period of 14.5 months. One patient was treated by a sphincterotomy of the minor papilla alone, while three others also underwent transpapilla…
