Clinical implications of epidural fat in the spinal canal. A scanning electron microscopic study.

BACKGROUND AND OBJECTIVES: This review of articles summarizes recent developments in relation to fat located in the epidural space and also in dural sleeves of spinal nerve roots in order to improve our understanding of the clinical effects of the epidural blockade. METHOD: Medline search was carried cross-matching of the following words: "epidural fat", "epidural space", "adipose tissue" and "fat cells" from 1966 to 2008 in which articles referring to different pathologies that alter the epidural fat were also reviewed. Techniques used by different authors included the use of samples from di

Roentgenological study of the sagittal diameter of the cervical spinal canal in normal adult Japanese.

The sagittal diameter of the cervical spinal canal on roentgenograms in normal adult Japanese aged 15 years or over, 505 males and 492 females, was investigated to define the normal distribution and lower limit. Lateral roentgenograms of cervical spinal canals were taken at a constant focus-film distance of 1.5 m. The mean +/- SD magnification coefficient was 1.17 +/- 0.02. The mean +/- SD sagittal diameters of the cervical spinal canals at each vertebral level were: C-1, 21.0 +/- 2.2 mm; C-2, 18.0 +/- 1.7 mm; C-3, 15.8 +/- 1.5 mm; C-4, 15.2 +/- 1.5 mm; C-5, 15.3 +/- 1.5 mm; C-6, 15.7 +/- 1.5

Epidural compression in the spinal canal caused by excessive adipose tissue. A case report.

Excessive deposition of fat in the epidural space is rare. In a recent review of the literature, there were 15 reported cases, 14 were associated with steroid use and one was attributed to hypothyroidism. This report reviews treatment of a patient with symptoms of cauda equina compression caused by epidural adipose tissue. He had no history of steroid use or obvious evidence of an underlying endocrinopathy.

Treatment of cervical spondylotic myelopathy by enlargement of the spinal canal anteriorly, followed by arthrodesis.

Thirty-seven patients who had enlargement of the spinal canal anteriorly and stabilization of the spine for cervical spondylotic myelopathy were followed for an average of forty-nine months (range, twenty-eight to seventy months). Myelography and computed tomographic myelography were performed preoperatively on all patients to determine the location and features of the areas of decompression. The canal was enlarged by discectomy; by subtotal corpectomy and removal of the anteromedial parts of the pedicles; or by removal of osteophytes or of the posterior longitudinal ligament, or both. Partial

[The effect of x-ray contrast media on magnetic resonance tomography of the spinal canal].

The effects of x-ray contrast media on signal intensities were studied. Relaxation times of myelographic contrast media were measured. Whereas relaxation times of Amipaque were longer, compared to CSF, the relaxation times of Duroliopaque, Omnipaque und Solutrast were significantly shorter than that of CSF. Therefore, the qualitative and quantitative effects of different contrast media on signal intensities varied. Most notable was the increase of signal intensity on T1-weighted images due to oily contrast media.

Analysis of the efficacy of UBE in the treatment of tophus deposition within the lumbar spinal canal.

RATIONALE: Lumbar intraspinal gouty tophus deposition is clinically rare, with nonspecific clinical/imaging features leading to frequent misdiagnosis. Current treatments mainly rely on open surgery, and systematic reports on Unilateral Biportal Endoscopy (UBE) application are scarce, necessitating exploration of UBE's efficacy. PATIENT CONCERNS: Seven patients (October 2023-November 2024) presented with persistent low back pain, accompanied by lower limb pain and/or numbness. Conservative treatment (urate-lowering drugs, anti-inflammatory drugs, and physical therapy) for ≥3 months was ineffec

Solitary sacral osteochondroma growing into the spinal canal: Case report and review of the literature.

Osteochondroma is one of the most common benign bone tumors, mainly involving the bone ends of long bones, and involving the spine is rare. It often involves the competing, followed by the thoracic and lumbar spine, and rarely involves the sacrum. We report the imaging findings of a solitary osteochondroma of the sacrum. The patient was a 37-year-old woman who presented clinically with progressive low back pain associated with left buttock pain and discomfort. CT and MRI showed that the lesion originated from the left lamina of S1 and grew anteriorly and superiorly, resulting in compressive re

Spinal canal invasion as a predictor of neurological deficit in traumatic vertebral burst fractures.

BACKGROUND: This study correlated the extent of spinal canal compression from retropulsed traumatic burst cervical, thoracic, and lumbar spine fractures with the severity of neurological dysfunction. METHODS: One hundred and sixty-nine patients with cervical, thoracic, or lumbar sub-axial traumatic burst fractures were seen in an emergency department from 2019 to 2021; 79.3% were men, averaging 37 years of age. The lumbar spine was most frequently involved (42%), followed by the thoracic (36.1%) and cervical (21.9%) levels. The extent of spinal canal compression was quantitated utilizing Hash

Synovial sarcoma of the spinal canal and paraspinal muscle and retroperitoneum: a case with extensive calcification.

INTRODUCTION: Synovial sarcoma (SS) is a rare mesenchymal malignant tumor. SS of the spine or retroperitoneum is an extremely rare site. Approximately 30% cases show focal calcifications on radiographs and computed tomography (CT) images, while extensive calcification rarely occurs. We presented a case of SS involving the spinal canal and paraspinal muscle and retroperitoneum, which showed extensive calcification on CT. CLINICAL PRESENTATION: The present report describes the case of a 13-year-old girl suffering from a tumor in the spinal canal and paraspinal muscle and retroperitoneum with ex

Safety of Thermosensor Insertion in the Midline of the Spinal Canal Anterior to the Dura: A Cadaveric Study.

OBJECTIVE: To evaluate the safety of the insertion of a blunt-tip thermosensor inside the anterior epidural space using the trans-osseous route in the dorsal spine and the double oblique trans-foraminal approach in the lumbar spine. MATERIALS AND METHODS: A total of 10 attempts were made on a 91 years old human specimen. Thermosensors were inserted under fluoroscopic guidance in the anterior part of the spinal canal using various oblique angulations. Surgical dissection was then performed to identify the position of the thermosensor and look for any injury to the dural sac or the spinal cord/

Effect of Sagittal Spinal Balance on the Outcome of Decompression Surgery for Lumbar Canal Stenosis.

OBJECTIVE: Although sagittal spinal balance is known to affect the outcome of spinal deformity surgery, its effect on simple decompression surgery is not well understood. MATERIALS AND METHODS: Patients who underwent unilateral laminotomy for bilateral decompression for lumbar canal stenosis were prospectively enrolled in the study. Before surgery and 6 months after surgery, the following sagittal-alignment parameters were measured: lumbar lordosis (LL), pelvic tilt (PT), pelvic incidence minus lumbar lordosis (PI-LL), and sagittal vertical axis (SVA). At the same time, short-form 36 (SF-36)

Is the atlas size associated with the pathophysiology of symptomatic spinal canal stenosis at the C1 level?

Narrow cervical spinal canal is an important risk factor for the development of cervical myelopathy. Patients with this disease often present with congenital narrowness of the cervical spinal canal. While there are studies on patients with subaxial spinal canal stenosis (SAS), few examined the coexistence of congenital narrow spinal canal in patients with cervical myelopathy at the C1 level. We investigated the characteristics of patients with C1 stenosis (C1S) with special reference to the size of the atlas. Thirteen patients (8 men, 5 women, mean age 76 years) with C1S were retrospectively a
