Epidemiology of spinal cord injury in the context of closed head injury.

DESIGN: Retrospective Study. BACKGROUND: The incidence of traumatic brain injury (TBI) in patients with spinal cord injury (SCI) is well documented. However, the incidence of SCI specifically in patients with closed head injury (CHI) is understudied. SCI and CHI are underdiagnosed, and this can be complicated history. Facilities are often focused on patients' primary issues, and with polytrauma patients' cognitive impairment, SCI can be missed. Thus, this study aimed to determine the incidence of SCI in patients with CHI. METHODS AND MATERIALS: This retrospective study used Pennsylvania Trau

Role of melatonin in traumatic brain injury and spinal cord injury.

Brain and spinal cord are implicated in incidences of two of the most severe injuries of central nervous system (CNS). Traumatic brain injury (TBI) is a devastating neurological deficit involving primary and secondary injury cascades. The primary and secondary mechanisms include complex consequences of activation of proinflammatory cytokines, cerebral edema, upregulation of NF-κ β, disruption of blood-brain barrier (BBB), and oxidative stress. Spinal cord injury (SCI) includes primary and secondary injury cascades. Primary injury leads to secondary injury in which generation of free radicals a

Back injury prevention program on a spinal cord injury unit.

A back injury prevention program was developed for the Spinal Cord Injury (SCI) nursing staff at the San Diego Veterans Affairs Medical Center (SDVAMC). This preventive program has the following components: 1. All nursing staff are sent to a one hour "back school" in order to learn and practice proper body mechanics. This program is held in the therapy clinic and conducted by a kinesiotherapist. In addition, each nurse must be certified in transfer techniques by our clinical nurse specialist, before performing transfers on patients. 2. All nursing staff participate in 5-10 minutes of stretchin

Anatomic location of spinal cord injury. Relationship to the cause of injury.

In a population based sample, 550 of 619 people with fatal and nonfatal spinal cord injuries were classified according to the cause of injury and according to the anatomic location of the highest spinal cord injury. Motor vehicle occupants accounted for 42% of the people with spinal cord injuries. Sixty percent of those injured had a cervical cord injury, 25% a thoracic cord injury, and 15% a lumbosacral cord injury. The specific anatomic locations most commonly injured were at C1 (12% of all spinal cord injuries), C6 (9%), and L2 (5%). The anatomic site of injury was significantly (P less tha…

Maladaptive spinal plasticity opposes spinal learning and recovery in spinal cord injury.

Synaptic plasticity within the spinal cord has great potential to facilitate recovery of function after spinal cord injury (SCI). Spinal plasticity can be induced in an activity-dependent manner even without input from the brain after complete SCI. A mechanistic basis for these effects is provided by research demonstrating that spinal synapses have many of the same plasticity mechanisms that are known to underlie learning and memory in the brain. In addition, the lumbar spinal cord can sustain several forms of learning and memory, including limb-position training. However, not all spinal plast

Etanercept in Spinal Cord Injury: A Systematic Review.

Background/Objectives: Traumatic spinal cord injury (SCI) frequently results in permanent motor and sensory deficits. Tumor necrosis factor-α is rapidly upregulated after SCI and contributes to secondary injury cascades, including microglial activation, cytokine amplification, and blood-spinal cord barrier disruption. Etanercept, a TNF-α inhibitor, has been investigated in modulating post-SCI neuroinflammation. This systematic review synthesizes preclinical evidence evaluating the therapeutic role of etanercept in SCI. Methods: A systematic review was conducted in accordance with PRISMA 2020 g

Injury and therapy in a human spinal cord organoid.

Damage to the spinal cord can lead to irreversible paralysis and loss of sensory function, but translation of preclinical therapies remains elusive. We recently showed that bioactive supramolecular assemblies of peptide amphiphiles can reverse paralysis in an acute mouse model following severe spinal cord injury (SCI). Here we report the development of two human spinal cord organoid injury models to simulate SCI in vitro, a laceration of the organoid with a scalpel and a compressive contusion commonly used in preclinical models, both resulting in immediate neuronal death and the formation of g

Therapeutic potential of ultrasound for spinal cord injury.

Spinal cord injury is a debilitating condition that often results in permanent impairments in motor, sensory, and autonomic functions. The potential for treating spinal cord injury with ultrasound is increasingly being recognized. Ultrasound addresses several key aspects of spinal cord injury, including promoting nerve healing by creating a regenerative environment, attenuating inflammation, and managing secondary complications. While preclinical studies show encouraging results, further translational efforts are necessary to adapt these therapies for clinical use. Insights gained from transcr

The emerging role of cuproptosis in spinal cord injury.

Copper is a vital trace element integral to numerous biological processes, including iron metabolism, neurotransmitter synthesis, mitochondrial respiration, oxidative stress regulation, and energy production. However, disturbances in copper metabolism can result in pathological conditions, including cuproptosis-a newly recognized form of programmed cell death (PCD) marked by copper accumulation and the disruption of copper-dependent metabolic pathways. Cuproptosis has been associated with various diseases, such as cancer, metabolic disorders and neurodegenerative disorders. In the context of s

Spinal cord injury: T cells to the rescue?

Gao, Kim, and colleagues recently reported that clonal populations of CD4+ T cells could be detected in mice that underwent spinal cord injury (SCI). A subset of clones mediated enhanced motor recovery and suppressed inflammation. Further studies may point towards novel cell therapies for SCI, for which care is presently supportive only.

Interaction of microglia with the microenvironment in spinal cord injury.

This article discusses the peculiarities of microglia behaviour and their interaction with other cells of the central nervous system (CNS) during neural tissue injury with a focus on spinal cord injury (SCI). Taking into account the plasticity of microglia, the influence of the microenvironment should be taken into account to establish the mechanisms determining the polarization pathways of these cells. Determination of the system of microglia interactions with other CNS cells during injury will reveal the patterns of post-traumatic microglia responses, in particular, determining both pro-infl

Rehabilitation Considerations for Women with Spinal Cord Injury.

Although men are more commonly affected than women with spinal cord injuries (SCIs), women comprise a growing portion of the population of individuals with SCIs. Guidelines for primary care and SCI issues are generally non-sex specific, and there are differences in the medical and rehabilitation needs of women compared with men. Consideration of these differences can optimize function and health for women with SCI and improve quality of life.

Mechanisms of nitric oxide in spinal cord injury.

Spinal cord injury (SCI) is a primary lesion of the spinal cord that results from external forces or diseases, accompanied by a cascade of secondary events. Nitric oxide, an endogenous gas that functions as a signaling molecule in the human body, plays a crucial role in vasodilation of smooth muscles, regulation of blood flow and pressure, and inflammatory response. This article provides a comprehensive overview of the involvement of nitric oxide in SCI and highlights recent advances in basic research on pharmacological agents that inhibit nitric oxide elevation after SCI, offering valuable in
