A dynamic theory for hydrocephalus revealing benign intraspinal tumours: tumoural obstruction of the spinal subarachnoid space reduces total CSF compartment compliance.

Hydrocephalus may rarely be the revealing symptom of intraspinal tumours. This form of presentation is more common in malignant spinal cord tumours due to subarachnoid dissemination and meningeal tumoural infiltration up to the intracranial basal cisterns. In contrast, in benign intraspinal tumours the mechanism of hydrocephalus has been a subject of controversy and various explanations have been proposed. We discuss different pathophysiological hypotheses and propose a mechanical explanation based on current knowledge of cerebrospinal fluid (CSF) dynamics. We hypothesize that hydrocephalus in

Non-dysraphic intramedullary spinal cord lipoma. A case report.

Non-dysraphic spinal cord lipomas are rare benign lesions, accounting for approximately 1% of all spinal cord tumors. Patients usually present with long histories of disability followed by rapid progression of their symptoms. Presenting symptoms include spinal pain, dysesthesic sensory changes, gait difficulties, weakness, and incontinence. Magnetic resonance imaging is the examination of choice. The T1- and T2-weighted images show increased and decreased signal intensity, respectively. The goal of intervention is decompression and generous debulking following by duraplasty. Early diagnosis of

Intradural spinal lipoma of the conus medullaris without spinal dysraphism.

A 42-year-old man suffering from progressive left radicular sensory motor loss (L4 level) underwent neurosurgical repair. Neuroimaging (RMI) had led to the diagnosis of schwannoma of the filum terminale with lipomatous component. Histological examination visualized a true mature lipoma associated with numerous bundles of more or less dystrophic nerve fibers. This histological benign tumor raised the problem of the genesis of intradural lipomas of spinal cord.

Intramedullary spinal cord hemorrhage (hematomyelia).

Intramedullary spinal cord hemorrhage (hematomyelia) is an uncommon cause of myelopathy and can present in an acute, subacute, stepwise, or chronic fashion. Spinal vascular malformations such as intramedullary cavernomas and intradural arteriovenous malformations are the most common cause of atraumatic intramedullary spinal cord hemorrhage based on the existing literature. Additional considerations include warfarin or heparin anticoagulation, hereditary or acquired bleeding disorders, primary spinal cord tumors, spinal cord metastases, Gowers' intrasyringal hemorrhage, or a delayed complicatio

Comprehensive guide to the diagnosis, management, and treatment of metastatic spinal cord compression syndrome.

Spinal metastases are the most common tumors found in the neuraxis. They correspond to 90 % of all tumors at this level. The thoracic region is the most frequently affected area, followed by the lumbar region, with the cervical region being the least frequently affected place. Back pain is the most common symptom, appearing in up to 95 % of patients who present with spinal cord compression syndrome. The evaluation of patients with spinal cord compression syndrome is complex and requires an entire multidisciplinary team for diagnosis, management, and treatment. In the medical part, treatment is

Sporadic Hemangioblastoma Arising from the Infundibulum.

Hemangioblastomas are rare vascular tumors most often found in the posterior fossa and cervical spinal cord and commonly associated with von Hippel-Lindau Disease. We report a case of sporadic hemangioblastoma in a patient without von Hippel-Lindau Disease. Imaging characteristics included a solid, suprasellar mass that was homogeneously enhancing. These findings most resembled a pituicytoma or choroid glioma because of the close association with the infundibulum and the homogeneous avid enhancement. Microscopically, the neoplasm was seen to be composed of vascular channels associated with foa

Spinal Myeloid Sarcoma "Chloroma" Presenting as Cervical Radiculopathy: Case Report.

Study Design Case report. Objective Myeloid sarcoma (also known as chloroma) is a rare, extramedullary tumor composed of immature granulocytic cells. It may occur early in the course of acute or chronic leukemia or myeloproliferative disorders. Spinal cord invasion by myeloid sarcoma is rare. The authors report a rare case of spinal myeloid sarcoma presenting as cervical radiculopathy. Methods A previously healthy 43-year-old man presented with progressive neck, right shoulder, and arm pain. Cervical magnetic resonance imaging (MRI) revealed a very large enhancing extradural soft tissue mass e

MR imaging of congenital anomalies of the pediatric spine.

Congenital anomalies of the pediatric spine are ideally evaluated with MR imaging. The wide spectrum of pathology in this category of disease is dramatically displaced for both diagnosis and treatment planning for the neurosurgeon. An understanding of embryology and knowledge of the clinical presentation combined with the MR imaging findings allows the radiologist to play a key role in the evaluation of these complex anomalies.

Perioperative outcomes and survival after surgery for intramedullary spinal cord tumors: a single-institution series of 302 patients.

OBJECTIVE: Intramedullary spinal cord tumors (IMSCTs) are rare neoplasms whose treatment is often technically challenging. Given the low volume seen at most centers, perioperative outcomes have been reported infrequently. Here, the authors present the largest single-institution series of IMSCTs, focusing on the clinical presentation, histological makeup, perioperative outcomes, and long-term survival of surgically treated patients. METHODS: A cohort of patients operated on for primary IMSCTs at a comprehensive cancer center between June 2002 and May 2020 was retrospectively identified. Data o

Updates in the Management of Metastatic Spine Disease.

The number of cancer diagnoses continues to increase each year in the United States, and given the propensity for bone metastases from solid organ malignancies, orthopaedic spine surgeons will inevitably encounter patients with metastatic spine disease and need to have a framework for approaching the evaluation and treatment of these complex patients. Many patients seeking care for spinal metastases already have a history of disseminated malignancy, but metastatic spine disease itself will be the presenting symptom of cancer in approximately 20% of patients. Because the first presentation of c

Epidemiology of spinal cord paralysis in Fiji: 1985-1994.

This descriptive analytical ten year (1985-1994) retrospective study assessed the pattern of spinal cord paralysis (SCP) in the Fiji Islands utilising medical rehabilitation hospital data. Fiji Islands is an archipelago of 300 islands in the south western Pacific with a multi-ethnic population of over three quarters of a million. Rehabilitation of all SCP is provided at the Medical Rehabilitation Unit (MRU). Data was collected from medical records of new SCP (n = 140) admitted to MRU and analysed with Epi Info 5 assessing associations between cause and other variables. The incidence of new SCP

Disseminated histiocytic sarcoma in 2 FeLV-positive cats in Brazil.

Histiocytic sarcomas (HSs) are uncommon malignant neoplasms in cats, typically affecting older animals and not previously associated with feline leukemia virus (FeLV). Here, we describe 2 cases of disseminated HS in young FeLV-positive domestic shorthair cats that clinically and grossly mimicked lymphoma. Case 1, a 1-y-old female cat, was presented with hemiplegia and dysuria and was euthanized because of the clinical suspicion of spinal lymphoma. Case 2, a 4-y-old male cat, had hyporexia, ocular abnormalities, and bilateral renal enlargement and was euthanized because of the clinical suspicio

Mini-Open Lateral Corpectomy for Thoracolumbar Junction Lesions.

BACKGROUND: Neoplastic, traumatic, infectious, and degenerative pathologies affecting the thoracolumbar junction pose a unique challenge to spine surgeons. Posterior or anterior approaches have traditionally been utilized to treat these lesions. Although minimally invasive surgeries through a lateral approach to the thoracic or lumbar spine have gained popularity, lateral access to the thoracolumbar junction remains technically challenging due to the overlying diaphragm positioned at the interface of the peritoneum and pleura. OBJECTIVE: To describe a mini-open lateral retropleural retroperit

Brain tissue adaptability to slow-growing tumors: case report of clivus meningioma.

A 46-year-old female patient with a large slow-growing craniocervical junction tumor is presented. Her complaints began 6 months before with sensory and painful sensations, sphincter impairment, and motor events (spastic tetraparesis, more pronounced on the left extremities). Magnetic resonance of the head revealed a rounded tumor of 2.5 cm in diameter, by its characteristics corresponding to meningioma, at the level of C1 vertebra and craniocervical junction, with the base at spinal canal anterior wall, occupying most of the craniocervical junction, compressing spinal cord and medulla oblonga

Staggering gait in medical history.

Drunkenness and senility were recognized early as the basis of a staggering gait. To these were added venereal excesses, hence syphilis. Medical and scientific concerns began to be focused on "locomotor ataxia" in the 19th century with the systematic development of neuroanatomy and physiology. Rolando and Flourens were followed by Romberg and Todd, and later Friedreich, who all gave the spinal cord temporal precedence as a culprit over the cerebellum--and there were some forerunners. New spinal sensory pathways were delineated by Goll, Flechsig, and Gowers. In France, we must specially credit
