Splenic rupture following minor blunt abdominal trauma may occasionally be accompanied by unexpected perisplenic findings that complicate diagnosis and management. We report the case of an 18-year-old previously healthy male who presented with progressive abdominal pain 10 days after being struck in the left upper abdomen by a soccer ball during a recreational football match. Initial abdominal ultrasonography demonstrated extensive splenic injury and moderate hemoperitoneum but did not identify a focal infectious collection. Emergency diagnostic laparoscopy revealed an American Association for…

A young woman presented with abdominal pain and jaundice secondary to Epstein-Barr virus-induced hepatitis and splenomegaly. She collapsed and CT imaging showed a large haemoperitoneum secondary to atraumatic splenic rupture. Splenic artery embolisation was attempted without success, and subsequently she underwent an open splenectomy. A collaborative multidisciplinary approach and adequate hospital set-up allowed for an initial attempt at non-operative intervention with the aim of splenic preservation, with rapid conversion to open surgery when necessary. Total blood loss of 7.5 litres was rep…

A 62-year-old woman presented with jaundice and fatigue. A blood examination revealed markedly elevated serum transaminase and bilirubin levels and negative hepatitis virus markers. A radiographic examination revealed no bile duct abnormalities. Serum transaminase levels decreased and normalized without immunosuppression after transcatheter arterial embolization and splenectomy for spontaneous splenic rupture, respectively. The pathological findings of the liver biopsy specimens were consistent with those of acute-onset autoimmune hepatitis (AIH), despite normal serum IgG levels. The clinical …

INTRODUCTION AND IMPORTANCE: Spontaneous splenic rupture (SSR) is a rare phenomenon where the spleen ruptures without associated trauma. SSR can lead to an intra-abdominal haemorrhage and an acute abdomen that can be life threatening.
CASE PRESENTATION: In this article, we present the case of an 81-year-old woman with chronic lymphocytic leukaemia who presented to the emergency department with severe abdominal pain.
CLINICAL DISCUSSION: In order to stabilize the patient, while awaiting elective surgery, we managed the rupture with splenic embolization and we reviewed the literature related t…

Angiosarcoma is an aggressive malignant endothelial cell tumor of vascular or lymphatic origin, with rapid progression and high mortality. It usually occurs in middle-aged and elderly people, mainly in the skin or subcutaneous or mammary gland. The incidence of angiosarcoma is low,and it is difficult to be timely detected.Due to the lack of effective therapy at present, the mortality rate of this disease is high. This paper reports a case of hepatic and splenic epithelioid angiosarcoma in a young male with sudden splenic rupture as the initial symptom, aiming to deepen clinicians' understanding of th…

We report a rare case of splenic tuberculosis (TB) in a male patient with a competent immune system who had no previous record of pulmonary TB. A 56-year-old male patient came to our outpatient department complaining of upper abdominal pain with a few episodes of vomiting for three days. He had alcoholism, smoked for 15 years, and had no past history of diabetes mellitus, hypertension, TB, or HIV. An abdominal ultrasound and CT scan at admission showed pancreatitis with a splenic abscess. After five days of admission, the patient's vitals deteriorated, and he had severe abdominal pain. CT scan

Emergencies of splenic origin in pregnancy involving rupture of the splenic artery or hemorrhage are rare events that can present suddenly and unexpectedly. We report two cases of young women, both in the third trimester of gestation, who suffered sudden malaise while at home and were admitted to the ER. On arrival, both were in severe hemorrhagic shock due to gross hemoperitoneum and they died in the operating theatre during emergency surgery. To better clarify the causes and sequence of these mortalities, forensic autopsy was requested in each case. In the first (a 26-year-old primigravida i

Objective: Splenic ectopic pregnancy is an exceptionally rare and life-threatening form of abdominal ectopic pregnancy, often presenting significant diagnostic and management challenges. We report a case of ruptured splenic ectopic pregnancy initially suspected to be a tubal miscarriage. Case Report: A 36-year-old woman presented with mild left-sided pelvic pain, no vaginal bleeding and a positive pregnancy test. This was an unplanned pregnancy, and she was not sure of her LMP. This presentation in early pregnancy requires ruling out an ectopic pregnancy through clinical assessment and laborat

An 89-year-old female presented to the emergency department (ED) with hypotension and altered mental status. The patient had no external signs of trauma or hemorrhage and no abdominal tenderness on examination. The patient remained hypotensive after initial fluid resuscitation, and laboratory testing revealed a significant anemia. Point-of-care ultrasound (POCUS) was used to perform a rapid ultrasound in shock (RUSH) exam in an attempt to uncover the etiology of undifferentiated hypotension. The exam displayed free fluid in the right upper quadrant and the left upper quadrant exam demonstrated

Splenic rupture in a neonate is a rare but potentially fatal condition that may trigger evaluation for child abuse. It is a diagnosis of exclusion that has been reported in the surgical literature but may be underrecognized by pediatric radiologists. We report a case of a newborn with an unremarkable prenatal, delivery, and nursery course who presented with anemia, abdominal distension, and lethargy. Abdominal ultrasound with Doppler and computed tomography (CT) of the head, cervical spine, chest, abdomen, and pelvis without contrast showed findings of splenic rupture and anoxic brain injury. …

Angiosarcoma, a rare endothelial cell tumor, accounts for < 2% of all sarcomas, typically affecting elderly patients. We present the case of a 62-year-old male with a history of hypertension and ischemic heart disease, who initially presented with severe lower back pain. Subsequent imaging revealed a ruptured splenic lesion and multiple liver lesions, leading to a diagnosis of stage 4 angiosarcoma. Post-splenectomy, the patient exhibited pathological vertebral fractures and elevated liver function tests, consistent with metastatic disease. Despite aggressive surgical intervention and supportiv

Amyloidosis is a term describing the extracellular deposit of fibrils composed of subunits of several different normal serum proteins in various tissues. Amyloid light chain (AL) amyloidosis contains fibrils that are composed of fragments of monoclonal light chains. Many different disorders and conditions can lead to spontaneous splenic rupture, including AL amyloidosis. We present a case of a 64-year-old woman with spontaneous splenic rupture and hemorrhage. A final diagnosis of systemic amyloidosis secondary to plasma cell myeloma was made with infiltrative cardiomyopathy and possible diasto…

This case report describes fatal exsanguination due to splenic artery hemorrhage into a pancreatic pseudocyst with cystogastrostomy in a 46-year-old woman. The decedent had a complicated medical history including necrotizing pancreatitis, giant pseudocyst formation after cystogastrostomy procedure, and coiling of a hemorrhagic splenic artery. While hospitalized, she underwent embolization of a ruptured splenic artery pseudoaneurysm. Weeks later, she went into hemorrhagic shock and was ultimately pronounced at the hospital. Doctors suspected an upper gastrointestinal (GI) bleed as the cause of

Splenic rupture due to any cause is a life-threatening complication and commonly attributed to trauma. Atraumatic splenic rupture is very rarely reported, and the incidence is currently unknown. Anticoagulants and dual anti-platelet medication can increase the chances of a splenic rupture. Surgical removal of the spleen may be warranted to prevent a life-threatening bleeding. Early identification and intervention are required for most patients as only a few qualify for medical management.

The traumatic diaphragmatic hernia is considered a rare clinic condition and represents a diagnostic challenge. Its presentation is mainly a surgical emergency, leading to scarce time for complementary image studies. We present a case of a 21-year-old male who suffered a penetrating wound in the left hemitorax from whom is suspected to have a traumatic visceral herniation into the thoracic cavity by chest X-ray, confirming a splenic herniation with intrathoracic hemorrhage, as well as the proposal of a management algorithm for this defiant cases.

Plasmodium vivax, which was previously considered 'benign', has come out with unusual severe symptoms. We, here present an unusual case of Plasmodium vivax malaria presenting with spontaneous splenic rupture with hemoperitoneum, renal and pulmonary dysfunction. Following conservative management, there was an uneventful recovery, and the patient was discharged in a good general condition.
