Effects of reductions in US foreign assistance on HIV, tuberculosis, family planning, and maternal and child health: a modelling study.

BACKGROUND: The USA has traditionally been the largest donor to health programmes in low-income and middle-income countries (LMICs). In January 2025, almost all such funding was stopped and prospects for its resumption are uncertain. The suddenness of the funding cuts makes it difficult for national health programmes in LMICs to adapt. We aimed to estimate the impact of these cuts on deaths and other outcomes (new infections, number of family planning users, and unplanned pregnancies) for four health areas that have been a focus of a substantial amount of US foreign assistance: HIV, tuberculos

Efforts to Increase Implementation of Evidence-Based Clinical Practices to Improve Adolescent-Friendly Reproductive Health Services.

PURPOSE: The purposes of this study were to describe changes in implementation of evidence-based clinical practices among health center partners as part of a multicomponent, community-wide teen pregnancy prevention initiative; to better understand the barriers to and facilitators of implementation of the evidence-based clinical practices; and to describe the technical assistance and training provided to the health center partners and key lessons learned. METHODS: Health center data from the second and third years (2012 and 2013) of the teen pregnancy prevention community-wide initiative were

Financial Neutrality for Living Organ Donors: Reasoning, Rationale, Definitions, and Implementation Strategies.

In the United States, live organ donation can be a costly and burdensome undertaking for donors. While most donation-related medical expenses are covered, many donors still face lost wages, travel expenses, incidentals, and potential for future insurability problems. Despite widespread consensus that live donors (LD) should not be responsible for the costs associated with donation, little has changed to alleviate financial burdens for LDs in the last decade. To achieve this goal, the transplant community must actively pursue strategies and policies to eliminate unreimbursed out-of-pocket costs

Health Management Planning for Landscape Fire Smoke Events: A Literature Review and Suggested Human-Centred Framework for Resource Development.

Poor air quality arising from landscape fire smoke (LFS) events presents a substantial health burden to at-risk individuals and communities. However, there are few dedicated health management resources available for priority populations, despite increasing climate-related landscape fire risks. We conducted a literature review to analyse health management plans and strategies available for at-risk individuals in Australia, Canada and the United States. Our review identified four resources, of which only two were dedicated LFS health action plans, revealing a critical gap in preparedness. Given

Associations of Person-Centred Planning and Service Coordination With Health Outcomes of Adults With Developmental Disabilities.

BACKGROUND: People with intellectual and developmental disabilities (IDD) experience significant health and healthcare disparities. Home and community-based services (HCBS) are crucial for the health and well-being of adults with IDD, and national regulations mandate person-centred planning (PCP) for all HCBS service administration. Despite these requirements, limited evidence exists on how PCP impacts health outcomes. This study examines the association of PCP and health outcomes, unmet healthcare needs and preventative care access. METHODS: This study uses 2021-2023 National Core Indicators

Development and use of mobile containment units for the evaluation and treatment of potential Ebola virus disease patients in a United States hospital.

Ebola virus disease (EVD) has been the subject of recent attention due to the current outbreak in West Africa, as well as the appearance of a number of cases within the United States. The presence of EVD patients in the United States required health care systems to prepare for the identification and management of both patients under investigation and patients with confirmed EVD infection. This article discusses the development and use of a mobile containment unit in an extended treatment area as a novel approach to isolation and screening of potential EVD patients.

The importance of establishing a national health security preparedness index.

Natural disasters, infectious disease epidemics, terrorism, and major events like the nuclear incident at Fukushima all pose major potential challenges to public health and security. Events such as the anthrax letters of 2001, Hurricanes Katrina, Irene, and Sandy, severe acute respiratory syndrome (SARS) and West Nile virus outbreaks, and the 2009 H1N1 influenza pandemic have demonstrated that public health, emergency management, and national security efforts are interconnected. These and other events have increased the national resolve and the resources committed to improving the national hea

The state of US trauma systems: public perceptions versus reality--implications for US response to terrorism and mass casualty events.

BACKGROUND: Injury has long been identified as the number one killer of Americans under the age of 34, and establishment of regional trauma systems and centers incorporating primary, secondary, and tertiary care and injury-prevention strategies has proved to be a vital element in reducing injury-related sequelae, deaths, and even costs. Despite these facts, trauma system development has not been given priority for funding in many local and state governments and only intermittently at the federal level. Consequently, many of the nation's trauma centers are strapped for funds to provide emergenc

An innovative approach to training hospital-based clinicians for bioterrorist attacks.

The recent attacks of September 11, 2001, and the subsequent dissemination event of anthrax in the United States demonstrated the necessity for hospitals to initiate bioterrorism education for clinicians. Events such as the release of sarin gas into the Tokyo subway by the Aum Shinrikyo cult provided some insight into how quickly emergency medical personnel may be overwhelmed by causalities of unconventional weapons. Educational interventions to prepare hospital-based practitioners for such disasters must fit among the demands of patient care, administrative duties, and continuing education wi

Family planning clinic services in the United States: patterns and trends in the late 1990s.

CONTEXT: Publicly funded family planning clinics are a vital source of contraceptive and reproductive health care for millions of U.S. women. It is important periodically to assess the number and type of clinics and the number of contraceptive clients they serve. METHODS: Service data were requested for agencies and clinics providing publicly funded family planning services in the United States in 1997. The numbers of agencies, clinics and female contraceptive clients were tabulated according to various characteristics and were compared with similar data for 1994. Finally, county data were ta

Targeting strategies: an overview of criteria and outcomes.

Researchers generally agree that Geriatric Evaluation and Management (GEM) Units are effective only when they are targeted at a specific group of frail, elderly patients who are most likely to benefit. Such patients are those who are neither too sick (eg, severely demented or moribund) nor too well. Various strategies for identifying such patients have been employed by investigators with little consensus on the most efficient targeting criteria. Criteria most often use for inclusion in GEM programs are various combinations of patient age, degree of functional impairment, presence of geriatric

Strengthening Bilateral Cooperation Through the Japanese Ministry of Health, Labour, and Welfare Liaison to the US Administration for Strategic Preparedness and Response.

Beginning in 2016, the Japanese Ministry of Health, Labour and Welfare (MHLW) began stationing liaisons within the US Department of Health and Human Services (HHS) Administration for Strategic Preparedness and Response to strengthen cooperation on bilateral health security engagement and collaboration. Notable accomplishments include the establishment of the US-Japan Health Security Committee and collaborations between the US-Japan Disaster Medical Assistance Team on mass event responses, such as the repatriation of American citizens from the Diamond Princess in February 2020 and the COVID-19

Dissemination, adoption, and implementation of acamprosate for treating alcohol use disorders.

OBJECTIVE: Acamprosate has been available in the United States for treating alcohol use disorders (AUDs) for nearly a decade, yet few studies have examined its use within AUD treatment organizations. In addition to describing dissemination and adoption of acamprosate, this study provides novel data regarding organizational processes that underlie its implementation within adopting programs. METHOD: Data were drawn from interviews with leaders of a nationally representative sample of 307 organizations delivering AUD treatment. Quantitative indicators of organizational characteristics, dissemin

The process and benefits of a university and state health agency collaboration for alcohol-free pregnancies in Oregon.

BACKGROUND: A significant number of college women are at risk for alcohol-exposed pregnancies because the ages of the heaviest alcohol consumption is typically 18 through 21 years, and contraception may be used ineffectively or not at all. These risks call for greater prevention efforts. OBJECTIVE: Collaboration between a higher education institution and a government health agency to reduce alcohol-exposed pregnancies in Oregon. METHODS: Health professionals from the Fetal Alcohol Syndrome (FAS) Prevention Program of the Oregon Public Health Division presented current research and explained
