This article reviews pectus excavatum, carinatum, and arcuatum. Topics covered include etiology, epidemiology, associated syndromes, physiologic impact, workup, indications for treatment, surgical and nonsurgical therapy, results, complications, and emerging therapies. Pectus excavatum is an inward deformation of the sternum and/or anterior chest wall. Pectus carinatum is ether an outward protrusion or tilt of the sternum with potential psychological impact, but no demonstrated physiologic impact. Nonoperative compression bracing is successful in carinatum patients with chest wall flexibility …

Cleft sternum is a rare congenital anomaly due to failure of fusion of the sternal bone. The etiology of this defect is not well understood. The anomaly is more common in females and may be associated with Cantrell's pentalogy. It can be partial or complete. An 8-year-old female child presented with complete defect in the sternum which was confirmed with a chest radiograph and computed tomography. Only 24 cases of isolated complete cleft sternum have been reported so far. The treatment ranges from primary closure of the defect to the use of various autologous and prosthetic grafts. Our patient…

We performed surgical reconstruction on 1655 cases of deformed thoracic cage, we later operated again on 11 of these to repair postoperatively re-deformed anterior chest walls. Based on these experiences, we have concluded as follows. 1: Postoperative recurrence of funnel chest deforming is mainly due to insufficient resection of costal cartilages. In particular transection of the sternum at low levels during sternal turn-over procedure results in postoperative recurrence of depression in the upper anterior chest wall. 2: In young children who have undergone sternal turn-over procedure, the fi…

Sternal closure can lead to hemodynamic and respiratory instability after some complex pediatric cardiac surgeries. In such cases, delayed sternal closure is applied to facilitate postoperative recovery. During open sternum several instruments have been defined to prevent the sternum from compressing the heart; however, most of them have limitations. We, therefore, have attempted to use a new instrument to keep the sternum open in neonates and infants. This instrument has been used as an adult eye speculum which is made of a flexible metal wire. It is delicate, ready to use, easily placed, and…

Tc-macroaggregated albumin (MAA) uptake in the vertebrae has been reported in central vein occlusion, although its sternal uptake is rarely seen. We present a case in which Tc-MAA SPECT/CT showed spotty uptake in the sternum. Contrast-enhanced CT revealed marked narrowing of the left subclavian vein at the thoracic outlet with a developed collateral vein running, in the left anterior chest subcutaneous tissue, between the sternum and left axilla. In this case, IV injection of Tc-MAA from the left forearm probably led to bone marrow uptake in the sternum due to retrograde venous flow through th…

With the increase of patients with adult congenital heart disease, the number of high-risk multiple redo sternotomies is increasing. Calcified conduit embedded in the sternum or large vessels attached to the sternum presents an especially challenging case. This video tutorial presents a simple safe redo sternotomy technique using an ultrasonic bone scalpel in such high-risk patients. The ultrasonic bone scalpel has a narrow cutting blade that oscillates longitudinally. Because the movements of the blade have a much smaller amplitude and faster speed than those of the usual saw, making a precis…

A 46-year-old man with Marfan syndrome was admitted for repair of annuloaortic etasia and funnel chest. Before median sternotomy, seven transverse skin incisions were made for resection of deformed ribs. The convex portions at the costochondral junctions of the right 4 approximately 7th and left 5 approximately 7th ribs were removed. Thereafter, the conventional median sternotomy was safely performed. Aortic root was replaced. After weaning from the cardiopulmonary bypass, the redundant distal end of the sternum was resected, fractured sites of the concave sternum were straightened and secured…

The distance from the suprasternal notch to this level over the sternum was calculated. The structures below the intermammary line, the lower half and the lower third of the sternum, and the optimal level were determined. The level with the largest cross-sectional area of the ventricle was approximately the lower fourth …

Chondromanubrial (arcuate) pectus carinatum is the rarest protrusion deformity of the chest. Its surgical correction was first described by Ravitch in 1952. We have recently encountered five patients with this deformity who have provided additional insight into the anatomy and optimal repair of this condition. It is notable for a short nonsegmented sternum with marked posterior angulation at the site of the normal chondromanubrial junction. It is optimally corrected by subperichondrial resection of the second to the seventh costal cartilages with a broad wedge-shaped osteotomy through the ante…

Paediatricians know of the condition of pectus excavatum in the growing child and may be acquainted with other anomalies of the sternum, such as forked and perforate xiphisterna and perforations higher up the sternum. The sternal malformation depicted in this case report, although not unusual in appearance, has a most rare origin.

A 46-year-old man had pain between the shoulders and at the sternum. Two and a half months earlier he had fallen when cycling. It appeared the pain was caused by fractures of thoracic vertebrae V and VI and a dislocation of the manubrium and body of the sternum.

Traumatic bilateral sternoclavicular joint dislocation is very rare injury. In shoulder girdle injuries, anterior dislocation of the sternoclavicular joint accounts for 3 % and posterior sternoclavicular joint dislocation is lesser. Previous reported cases about bilateral sternoclavicular joint dislocation were result from proximal clavicle fracture with intact connection between sternum and ribs. But, the sternoclavicular joint dislocation secondary to fracture and angulation of the sternum with intact relationship between ribs and clavicle has not been reported. Authors experienced patient w…

We report the positron emission tomography/computed tomography appearance of solitary sternal metastasis from primary ovarian carcinoma in a 65-year-old woman who underwent surgery followed by chemoradiation treatment 3 years earlier. Restaging positron emission tomography/computed tomography study demonstrates an intensely active destructive lesion involving the entire sternum (maximum standard uptake value: 4.7) with a photopenic soft tissue mass component in the center of the sternum protruding anteriorly suggestive of necrosis. This sternal lesion was subsequently biopsied and pathology re…

Pectus carinatum (PC) is a chest deformity caused by a disproportionate growth of the costal cartilages compared to the bony thoracic skeleton, pulling the sternum towards, which leads to its protrusion. There has been a growing interest on using the 'reversed Nuss' technique as a minimally invasive procedure for PC surgical correction. A corrective bar is introduced between the skin and the thoracic cage and positioned on top of the sternum highest protrusion area for continuous pressure. Then, it is fixed to the ribs and kept implanted for about 2-3 years. The purpose of this work was to (a)…

Sternal injuries are caused by two mechanisms: direct and indirect forces on the sternum. An indirect sternum fracture is usually caused by a forcible anteflexion of the spine. When a patient with back bruising complains of anterior chest pain, a sternal fracture should be considered in the differential diagnosis.

INTRODUCTION: To analyze the anatomy of the sternum and manubrium with a focus on the proximity of vital posterior structures to provide surgeons with guidelines for screw length and to increase awareness of locations for the safest hardware placement.
MATERIALS AND METHODS: A retrospective chart review was conducted at a level one trauma center in the southeastern United States. Patients with a chest CT in 2022 without evidence of abnormalities or prior cardiac or thoracic surgery, chest trauma, or sternotomy were included in the study. The primary study measures were dimensions of the manub…

Large-cell neuroendocrine carcinoma of the thymus is a rare cancer that is more aggressive than other thymic epithelial tumors with poorer prognosis. We report one such case in a 44-year-old male patient with invasion into the upper 2/3rd of the sternum but no endocrinopathy. Complete enbloc resection of the tumor with adherent pericardium, upper 2/3rd of the sternum and mediastinal lymphadenectomy was performed. The chest wall was reconstructed with polypropylene mesh and bilateral pectoralis major muscle advancement flap cover. A search of PubMed database with user query words 'neuroendocrin…

A congenital deformity of the chest is described in which all of the ribs and costal cartilages on the right side are deeply depressed, the apex of the depression being much farther lateral than is usual in pectus excavatum. The sternum is not depressed or scaphoid but is usually rotated to the right, sometimes markedly. The right breast is smaller than the left breast. Four of the patients were female and one was male. A single unsatisfactory experience demonstrated that rigid internal fixation is required. In the other four patients, triple section of the costochondral elements and appropria…

Pectus carinatum is a chest deformity characterized by a protrusion of the sternum and ribs (usually 3-7 ribs). The treatment of these patients varies in relation to age. In younger patients we prefer to use a custom-made brace, surgery is the elective treatment for older patients. The minimally-invasive technique (Abramson procedure) is used rarely and for mild defects, whereas open surgery is still preferred by many surgeons to repair major deformities. In our institution we use a modified Ravitch approach trough a vertical incision, which is performed on top of the most prominent part of th…
