Pulmonary and systemic host response to Streptococcus pneumoniae and Klebsiella pneumoniae bacteremia in normal and immunosuppressed mice.

Mortality related to bacteremic pneumonia remains high, and the role of sepsis in inflammation, pulmonary injury, and death remains unclear, mostly in leukopenic states. In the present study, the microbiology, histopathology, and host response to Streptococcus pneumoniae and Klebsiella pneumoniae infection were determined in an experimental model of bacteremia in immunocompetent and leukopenic mice. Leukocyte depletion by cyclophosphamide did not impair the early clearance of pneumococci from blood but facilitated growth in lungs. By contrast, klebsiellae rapidly grew in blood of leukopenic mi…

A Novel Diagnostic Scoring System to Differentiate between Legionella pneumophila Pneumonia and Streptococcus pneumoniae Pneumonia.

Objective We investigated a novel diagnostic scoring system to differentiate Legionella pneumophila pneumonia from Streptococcus pneumoniae pneumonia. Methods We retrospectively reviewed the clinical data of 62 patients with L. pneumophila pneumonia (L-group) and 70 patients with S. pneumoniae pneumonia (S-group). Results The serum sodium (Na) levels tended to be lower according to the severity [age, dehydration, respiratory failure, orientation disturbance, low blood pressure (A-DROP)] score in the L-group. On a multivariate analysis, we found that four factors were independent predictive mar…

Investigation on the correlation factors of positive Streptococcus pneumoniae antibody and IgG antibody level of Streptococcus pneumoniae in the elderly over 60 years old in Shenzhen.

BACKGROUND: Pneumococcal Polysaccharide Vaccine (PPV-23), designed to protect against the most common serotype of Streptococcus pneumoniae, is intended to protect the elderly and other high-risk groups. However, the immunogenicity of all 23 pneumococcal polysaccharide vaccines in older adults has not been thoroughly studied. OBJECTIVE: The purpose of this study is to look into the factors that influence the effect of the pneumonia vaccine on the elderly over 60 years old in Shenzhen, as well as their IgG antibody level against Streptococcus pneumoniae. METHODS: To determine the immune effect…

Streptococcus pneumoniae, an unusual cause of early-onset neonatal sepsis and necrotizing pneumonia.

Vertically transmitted sepsis due to Streptococcus pneumoniae has a low incidence, and vaginal colonization among pregnant women is exceptional. Necrotizing pneumonia is uncommon in immunocompetent term neonates, and the prognosis is uncertain. At present, systematic screening does not seem warranted in pregnant women. Therefore, aggressive treatment of neonates remains the best treatment.

[Epidemiology of meningitis, pneumonia and acute otitis due to Streptococcus pneumoniae in pediatric in Italy].

Streptococcus pneumoniae is one of the most important bacterial pathogens in pediatrics, both from a quantitative and from a qualitative point of view. It can cause severe diseases such as occult bacteremia, sepsis, meningitis and it is also involved in the determination of a relevant part of very common illnesses such as pneumonia, acute otitis media and rhinosinusitis. However, most of the data regarding the epidemiology of pneumococcal infections in pediatrics have been collected in the U.S.A. and in other industrialized Countries different from Italy. In Italy only studies concerning menin…

Community-Acquired Necrotizing Pneumonia with Bronchopleural Fistulas Due to Streptococcus pneumoniae in an Adult.

BACKGROUND: Necrotizing pneumonia with subsequent bronchopleural fistula attributable to Streptococcus pneumoniae infection has not been reported in adults. CASE PRESENTATION: We here firstly report a case of 23-year-old female with severe community-acquired multiple lung abscesses due to Streptococcus pneumoniae in the left lower lung, which merged and ruptured, further leading to the development of bronchopleural fistulas. After adequate anti-infective treatment and repairing the fistulas under thoracoscopy, the pulmonary infection was under control, and the fistulas were closed. A follow-u…

Bacteremic pneumonia caused by a single clone of Streptococcus pneumoniae with different optochin susceptibilities.

Two isolates of Streptococcus pneumoniae having different optochin susceptibilities were recovered from a blood sample of a 2-year-old boy with community-acquired pneumonia. The two isolates were documented to belong to a single clone on the basis of the isolates' identical serotype (23F), antibiograms by the E-test, random amplified polymorphic DNA patterns generated by arbitrarily primed PCR, pulsed-field gel electrophoresis, and restriction fragment length polymorphism of the penicillin-binding protein genes pbp2b and pbp2x.

Treatment of experimental pneumonia due to penicillin-resistant Streptococcus pneumoniae in immunocompetent rats.

A model of pneumonia due to Streptococcus pneumoniae resistant to penicillin was developed in immunocompetent Wistar rats and was used to evaluate the efficacies of different doses of penicillin, cefotaxime, cefpirome, and vancomycin. Adult Wistar rats were challenged by intratracheal inoculation with 3 x 10(9) CFU of one strain of S. pneumoniae resistant to penicillin (MICs of penicillin, cefotaxime, cefpirome, and vancomycin, 2, 1, 0.5, and 0.5 microg/ml, respectively) suspended in brain heart broth supplemented with 0.7% agar. The rats experienced a fatal pneumonia, dying within 5 days and …

Complement and opsonins in alveolar secretions and serum of rats with pneumonia due to Streptococcus pneumoniae.

Complement appears to have an important role in the early defense against Streptococcus pneumoniae, but the role of complement as a defense mechanism within the lung is not well defined. Complement and heat-labile opsonins in pneumococcal pneumonia were studied by analysis of bronchoalveolar lavage fluid (BALF) of rats inoculated intratracheally with type 3 S. pneumoniae. BALF and serum were obtained at 0, 6, and 24 hr after infection. Leukocyte counts in BALF and histologic studies revealed an acute inflammatory response in the lung at 6 hr; this inflammation progressed for 24 hr. Levels of C…

Novel Immunoprotective Proteins of Streptococcus pneumoniae Identified by Opsonophagocytosis Killing Screen.

The success of polysaccharide conjugate vaccines represents a major advance in the prevention of pneumococcal disease, but the power of these vaccines is limited by partial spectrum of coverage and high cost. Vaccines using immunoprotective proteins are a promising alternative type of pneumococcal vaccines. In this study, we constructed a library of antisera against conserved pneumococcal proteins predicted to be associated with cell surface or virulence using a combination of bioinformatic prediction and immunization of rabbits with recombinant proteins. Screening of the library by an opsonop

Pyomyositis caused by Streptococcus pneumoniae after allogeneic hematopoietic stem cell transplantation.

Pyomyositis is classified into two main types: tropical and non-tropical. Non-tropical pyomyositis occurs among various immunocompromised patients, and Staphylococcus aureus has been reported as the most common pathogen. Pyomyositis caused by Streptococcus pneumoniae is uncommon, and has not been previously reported after allogeneic hematopoietic stem cell transplantation (allo-HSCT). Here, we report a unique case with pyomyositis caused by S. pneumoniae in the bilateral erector spinae muscles 34 months after allo-HSCT. The patient had an initial clinical response following the administration …

[An Early Neonatal Sepsis Case Due to Streptococcus pneumoniae Serotype 7F].

Streptococcus pneumoniae infections which are relatively unusual in the neonates, are associated with substantial morbidity and mortality. Although S.pneumoniae has more than 90 serotypes, invasive disease is more common with certain serotypes. Pneumococcal polysaccharide and conjugate vaccines are developed against the serotypes that cause invasive disease. Until now invasive pneumococcal diseases have been defined due to various serotypes in the neonatal period. Here we present a neonate with an invasive pneumococcal disease due to S.pneumoniae serotype 7F which has not yet been reported in …

A bivalent vaccine to protect against Streptococcus pneumoniae and Salmonella typhi.

Pneumococcal and Salmonella typhi infections are two major diseases for children in developing countries. For typhoid fever, licensed Vi polysaccharide vaccines are ineffective in children <2-year old. While investigational Vi conjugate vaccines have been shown effective in clinical trials, they are currently only available to restricted areas. Pneumococcal capsular polysaccharide conjugate vaccines are highly effective in children, but suffer from some limitations including cost and limited serotype coverage. We have previously shown that a fusion conjugate vaccine, consisting of pneumococcal

Capsular serotype and antibiotic resistance of Streptococcus pneumoniae isolates in Malaysia.

BACKGROUND: Streptococcus pneumoniae is a major causative agent of severe infections, including sepsis, pneumonia, meningitis, and otitis media, that has since become a major public health concern. In this study, the serotypes distribution of pneumococcal isolates was investigated to predict the efficacy of the 7-valent pneumococcal conjugate vaccine (PCV7) among the Malaysian populations. METHODOLOGY/PRINCIPAL FINDINGS: A total of 151 clinical isolates were serotyped using multiplex PCR assays. Out of them, there were 21.2% penicillin-resistant, 29.1% penicillin-intermediate, and 49.7% penic…
