What to do if A + B doesn't work.

The majority of newborn resuscitations require very little beyond simple airway management and assisted ventilation. If cardiovascular collapse is serious enough to warrant additional support, resuscitation algorithms recommend moving to chest compressions and then on to medications and possibly volume replacement if vital signs remain marginal or absent. The evidence base upon which this part of the neonatal resuscitation algorithm is structured is sparse. Chest compressions and medications are rare interventions that do not lend themselves easily to clinical trials. Slowly but surely, howeve

Interventions Involving the Use of Covered Coronary Artery Stents for Pseudoaneurysms of Blalock-Taussig Shunts.

One of the most rare and dangerous complications of a modified Blalock-Taussig shunt involves the formation of a pseudoaneurysm. A pseudoaneurysm may rupture or may result in other adverse events including compression of mediastinal structures or collapse of the underlying lung parenchyma. Shunt-related pseudoaneurysm may be associated with shunt occlusion, bacteremia, or pulmonary hemorrhage. We describe two cases complicated by large pseudoaneurysm formation, following systemic-to-pulmonary artery shunt operations. The patients in question had recurrent pulmonary hemorrhage. Both underwent s

Meatoplasty in Marfan syndrome.

Marfan syndrome (MS) is a rare connective tissue disorder, uncommon in otological practice. Ear surgery in MS has not been previously reported. An identical complication of meatoplasty developed in two patients with MS after modified radical mastoidectomy and meatoplasty for chronic otitis media. Both cases presented post-operatively with an external auditory canal narrowing when the walls of the initial meatoplasty in both cases collapsed. It is proposed that this occurred because of the inherent structural abnormality of the cartilage in Marfan syndrome. A simple technique of revision meatop

Genetic factors affecting hearing development.

Both genetic background and single gene mutations may affect the development of the auditory system. A classification system is presented for those single gene mutations causing hearing impairment. The new feature of this classification is the inclusion of a category for hereditary deafness of central origin. The other categories involve peripheral abnormalities and are: morphogenetic defects, in which the overall structure of the labyrinth is deformed; neuroepithelial degeneration, in which the primary defect appears to occur in the organ of Corti; and cochleo-saccular degeneration, where the

EPS8 dampens the growth dynamics and prolongs the lifetime of actin-based protrusions.

Actin-based membrane protrusions such as filopodia, microvilli, and stereocilia support a range of cell functions, from nutrient absorption to mechanosensation. In each case, membrane deformation is supported by a core bundle of actin filaments, organized in a unipolar 'barbed-end out' manner. Although their structures and proteomes are well characterized, mechanisms governing the growth and stability of these protrusions remain less clear. Factors that localize to the distal tips of these structures are of particular interest, as they are well positioned to control actin assembly at filament

Axonal Regeneration, Growth Cone, and the FIGN Gene Family: A Comprehensive Review.

Mature mammalian central nervous system (CNS) neurons exhibit limited regenerative capacity, resulting in severe and often irreversible functional deficits following injuries such as spinal cord injury or stroke. Both inhibitory extrinsic environments and intrinsic neuronal constraints govern this regenerative failure. Growth cones-cytoskeleton-rich structures at axonal tips-are critical for axon extension and pathfinding during neural repair, with their structural integrity and motility heavily dependent on microtubule (MT) stability and dynamics. Pharmacological stabilization of MTs has been

Interactions Determining Stereoselectivity in Two-Dimensional Systems─The Case of 22-Hydroxycholesterol Epimers.

Oxidized derivatives of cholesterol play an important role in the functioning of biomembranes. Unlike other biomolecules, which are physiologically active in only one enantiomeric form, some oxysterols exist endogenously as two stereoisomers that exhibit strictly different biological effects. In this paper, we focused our attention on 22-hydroxycholesterol (22-OH) epimers, 22(R)-OH and 22(S)-OH, and examined their properties in Langmuir monolayers spread at the air/water interface, using classical surface manometry complemented with Brewster angle microscopy (BAM) images of the film texture. Thin-Section CT Features of Idiopathic Pulmonary Fibrosis Correlated with Micro-CT and Histologic Analysis.

Purpose To elucidate the underlying lung changes responsible for the computed tomographic (CT) features of idiopathic pulmonary fibrosis (IPF) and to gain insight into the way IPF proceeds through the lungs and progresses over time. Materials and Methods Micro-CT studies of tissue cores obtained from explant lungs were examined and were correlated 1:1 with a CT study obtained immediately before transplantation. Samples for histologic analysis were obtained from selected cores. Results In areas with no or minimal abnormalities on CT images, small areas of increased attenuation located in or nea

Examining polyglutamine peptide length: a connection between collapsed conformations and increased aggregation.

Abnormally expanded polyglutamine domains in proteins are associated with several neurodegenerative diseases, of which the best known is Huntington's. Expansion of the polyglutamine domain facilitates aggregation of the affected protein, and several studies directly link aggregation to neurotoxicity. The age of onset of disease is inversely correlated with the length of the polyglutamine domain; this correlation motivates an examination of the role of the length of the domain on aggregation. In this investigation, peptides containing 8 to 24 glutamines were synthesized, and their conformationa

Nanometer-Scale Surface Structure and Sterilization Characteristics of Na/Ag Ion-Exchanged Glass for Anti-Biofouling Application.

Most objects and structures exposed to a marine environment for more than a month undergo biofouling of the surface. Biofouling is a phenome … e glass implanted with Ag ions is immersed in seawater, the surface dissolves and the glass-network-structure collapses, resulting in considerable Ag ion elution, which enhances bactericidal action in seawater. For example, when a specimen immersed in seawater for 30 days was inoculated with the strain for 24 hours, the number of strains decreased from 2.1 × 106 cel …

Defect Engineering via Li2CeO3 Oxygen-Vacancy Coating: Dual Optimization of Structural Integrity and Lattice Oxygen Stability in High-Nickel Cathodes.

High-nickel layered oxide LiNixCoyMn1-x-yO2 (NCM, x ≥ 0.8) materials are considered optimal cathodes for lithium-ion power batteries owing to their high energy density, commendable cycling performance, and cost-effectiveness. However, structural collapse and interface instability during cycling result in diminished cycling stability, significantly hindering their commercial viability. This study reports a strategy to improve the stability of the cathode structure and suppress surface degradation of high-nickel NCMs by introducing LCO (Li2CeO3) coatings. The LCO coating provides a stable surfac

The explosion of the present and the encapsulation of time: transference phenomena in the analysis of a psychotic patient.

The paper deals with some basic problems concerning the experience of time and space in the psychoanalytic treatment of psychotic patients. Whereas borderline patients tend to distort the experience of time and space under emotional pressure, the concepts of time and space seem to dissolve in acute psychotic states of mind. Sometimes this manifests itself in an explosion of the present, where the past is ubiquitous and the future is perceived as the end of all times. The case of a 48 year-old patient with the external diagnosis of 'paranoid-hallucinatory schizophrenia' is presented to illustra

Reduction Structured Rhinoplasty.

Reduction structured rhinoplasty is a rhinoplasty technique that combines elements from two rhinoplasty philosophies: simple reductive rhinoplasty and structural rhinoplasty. The goal is to achieve decreased nasal size while supporting a life-long, lasting nasal shape by employing the use of structural grafts.Reducing nasal size without replacing disrupted nasal support structures has historically led to poor outcomes.Structural grafts include spreader grafts, onlay tip grafts, columellar strut grafts, caudal septal extension grafts, and more. They are utilized to prevent internal nasal valve

When Ultra-Processed Foods are Eaten: The Missing Dimension in Exposure Measurement for Obesity Research.

PURPOSE OF REVIEW: Almost every study linking ultra-processed foods (UPFs) to obesity measures exposure as the proportion of energy or weight derived from Nova group 4 foods, averaged across days. This scalar is commonly treated as though it were the exposure itself, yet it embodies an untested assumption: that the timing and context of intake carry no information beyond total amount. This review examines what is lost when occasion-level intake is collapsed to a daily average and how that loss may affect epidemiological inference. RECENT FINDINGS: Controlled circadian-misalignment protocols a

Stentoplasty-assisted Anterior Column Reconstruction with Navigation-guided Spinopelvic Fixation in an L5 Pathological Fracture Secondary to Vertebral Hemangioma - A Case Report.

INTRODUCTION: Pathological fracture due to vertebral hemangioma at the lumbosacral junction is exceedingly rare and poses unique challenges due to the region's high biomechanical demands and complex anatomy. While vertebral hemangiomas are typically asymptomatic or involve the thoracic spine, symptomatic presentation with neurological deficit and vertebral collapse at L5 is distinctly uncommon. This case report is among the few documented instances highlighting a concept-driven, biomechanically sound treatment strategy in such a scenario, contributing valuable insight to the orthopedic and spi

Contrast and pattern adaptation in visual cortex share a common gain control mechanism.

Neuronal populations in primary visual cortex adapt both to stimulus contrast and to the probability of occurrence of visual patterns. Previous work showed that the magnitude of the population response follows a separable power-law function of contrast and stimulus probability, suggesting the existence of a shared gain mechanism. Here we ask whether a similar equivalence extends beyond response magnitude to the full distribution of activity across neurons within a trial. Across a wide range of adaptation states, we find that population responses are highly sparse and well described by a zero-i

Brain histopathological changes caused by SARS-CoV-2 infection.

Coronavirus disease 2019 (COVID-19), caused by the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), affected over 775 million people in 232 countries and regions between 2020 and 2024, causing over seven million deaths and becoming the largest pandemic of the 21st century. Initially considered a respiratory virus with various clinical forms (from asymptomatic to severe clinical forms with severe respiratory failure), it was later shown that the disease also affects other organs and systems, including the nervous system. Therefore, in this study, we aimed at highlighting the histop

A mechanistic study identifying improved technology critical metal delamination from printed circuit boards at lower power sonications in a deep eutectic solvent.

Deep eutectic solvents (DESs) are an emerging class of ionic liquids that offer a solution to reclaiming technology critical metals (TCMs) from electronic waste, with potential for improved life cycle analysis. The high viscosities typical of DESs, however, impose mass transport limitations such that passive TCM removal generally requires immersion over extended durations, in some cases in the order of hours. It is postulated that, through the targeted application of power ultrasound, delamination of key structures in electronic components immersed in DESs can be significantly accelerated, the
