Iophendylate or spillage from epidermoid--a diagnostic dilemma on cranial MR imaging.

MR characteristics of iophendylate in spinal subarachnoid space are well known. Still, it is difficult to differentiate it from spilled contents of epidermoid on cranial MR imaging.

Extended endoscopic endonasal approach to the chiasmatic cistern: An anatomical study of the arachnoid.

This paper studied the arachnoid of the chiasmatic cistern (CC) and the methods for increasing the exposure of the CC from an endoscopic perspective. Eight anatomical specimens with vascular injection were used for endoscopic endonasal dissection. The anatomical characteristics of the CC were studied and documented, and anatomical measurements were collected. The CC is an unpaired five-walled arachnoid cistern located between the optic nerve, optic chiasm, and the diaphragma sellae. The average exposed area of the CC before the anterior intercavernous sinus (AICS) was transected was 66.67 ± 33

Extraparenchymal neurocysticercosis in the United States.

Neurocysticercosis (NCC) is endemic in the developing world but is becoming more common in the United States because of immigration. Although NCC is pleomorphic in its presentation, extraparenchymal NCC may be challenging to diagnose and treat. Extraparenchymal NCC is probably more frequent than previously thought. Neurologists and neuroradiologists in the United States are often unaware of the pretreatment/post-treatment radiographic patterns of extraparenchymal NCC and the potentially poor prognosis if not correctly diagnosed and managed. The review of this condition is important given incre

Radiology of the ambient cistern. Part I: Normal.

The angiographic and high-resolution, thin-section CT appearance of the normal ambient cistern is correlated with anatomic dissections and sections of normal, formalin-fixed cadaver brains. Each of three separate anatomic segments of the ambient cistern--the supratentorial segment, the tentorial edge and the subtentorial segment--has a characteristic CT appearance that can be correlated with specific vascular structures on routine angiography.

Superficial siderosis - mechanism of disease: an alternative hypothesis.

Superficial siderosis is an increasingly common diagnosis due to advances in imaging. Resulting from the presence of blood in the subarachnoid space, it leads to progressively debilitating ataxia, deafness and myelopathy. Magnetic resonance imaging has revolutionized diagnosis and management of this condition. In one-third of patients with this diagnosis, no subarachnoid bleed can be identified. A lack of sensitivity in our ability to detect subarachnoid bleed is a possible explanation in this group of patients. A novel approach would be to consider defects in the body's defense against intrac…

The "Transdural" Paradox: The Case of a Paraclinoid Internal Carotid Artery Aneurysm With an Intradural Neck and Extradural Dome.

Paraclinoid internal carotid artery (ICA) aneurysms pose a surgical challenge because of their complex relationship with the distal dural ring (DDR), the boundary between the extradural cavernous sinus and the intradural subarachnoid space. The classification of these aneurysms as strictly intradural or extradural is often complicated by the anatomical variability of the DDR, which can form a dural pouch known as the carotid cave. This anatomical paradox can lead to a "transdural" aneurysm, in which the dome is in the extradural or intradural space, which poses a risk for subarachnoid hemorrha…

Urgent treatment of severe subarachnoid hemorrhage caused by ruptured traumatic aneurysm of the cavernous internal carotid artery using coil embolization followed by superficial temporal artery-middle cerebral artery anastomosis: a case report.

BACKGROUND: Traumatic aneurysm of the cavernous internal carotid artery (ICA) with extension into the subarachnoid space is associated with increased risk of fatality especially when it is accompanied by severe subarachnoid hemorrhage (SAH). Only cases of patients who survived the acute stage and who were treated in a delayed setting have been reported. There has been no successfully treated case immediately after an injury. CASE DESCRIPTION: We encountered a 48-year-old man who presented with dense SAH immediately after being involved in a motor vehicle accident. Emergent angiography reveale…

Intracranial migration of silicone oil from an eye with optic pit.

BACKGROUND: The origin of subretinal fluid in eyes with optic pit remains controversial. METHODS: Case report. RESULTS: The authors found that silicone oil, implanted into an eye that developed proliferative vitreoretinopathy after surgery for optic pit-related macular detachment, has migrated into the subarachnoid space. CONCLUSIONS: As this case shows, cerebrospinal fluid may migrate into the submacular space in eyes with optic pits.

Small animal model for investigation of subarachnoid hemorrhage and cerebral vasospasm.

A method for induction of subarachnoid hemorrhage (SAH) in a rat model is described. Resolution of the hemorrhage was documented photographically and microscopically at intervals from 1 hr to 8 days. Photographs indicated that most of the hemorrhage was resorbed within 3 days, an observation confirmed microscopically by the amount of red blood cells in the subarachnoid space. Significant cerebral vasospasm was documented within the first 2 days after the induction of hemorrhage with the basilar artery returning to baseline values at an average of 3 days followed by moderate dilatation at 5 to …

The authors identified a subgroup of patients with cranial dural AVF's in whom the AVF was drained only by leptomeningeal veins, and sought to establish whether simple interruption of the vein draining the blood from the AVF into the subarachnoid s …

Cysticercosis of the central nervous system. II. Spinal cysticercosis.

The compromising of the spinal canal by cysticercus is considered infrequent, varying from 16 to 20% in relation to the brain involvement. In the spinal canal the cysticercus predominantly places in the subarachnoid space. Clinical signs in spinal cysticercosis can be caused by direct compression of the spinal cord/roots by cysticerci and by local or at distance inflammatory reactions (arachnoiditis). Another mechanism of lesion is degeneration of the spinal cord due to pachymeningitis or circulatory insufficiency. The most frequent clinical features are signs of spinal cord and/or cauda equin…

Spontaneous spinal subarachnoid hemorrhage and subdural hematoma. Report of two cases.

Two patients with spontaneous spinal subarachnoid hemorrhage are presented to emphasize the clinical and radiological features of this uncommon illness. Both had severe back pain at the onset. One patient had a subdural hematoma that compressed the conus medullaris and cauda equina, and was drained percutaneously; the other had clots in the subarachnoid space. The cerebrospinal fluid showed a polymorphonuclear pleocytosis that simulated septic meningitis. Complete spinal angiography failed to reveal a cause for the hemorrhages.

The role of complement in inflammation during experimental pneumococcal meningitis.

The mechanism whereby an effective bactericidal inflammatory reaction develops in the subarachnoid space is not clearly defined. While normal cerebrospinal fluid is deficient in complement, immunoglobulin and leukocytes, these serum components appear in cerebrospinal fluid (CSF) during the course of bacterial meningitis. Using a rabbit model of pneumococcal meningitis we examined the role of the alternate complement pathway in three early events important to the defense of the subarachnoid space: leukocyte chemotaxis, phagocyte mediated bacterial killing, and clearance of bacterial components …

Collapse or distention of the perioptic space in children - What does it mean to pediatric radiologists? Comprehensive review of perioptic space evaluation.

The perioptic space comprises the subarachnoid space [SAS] of the optic nerve communicating with the SAS of the central nervous system. Pressure variations in the SAS of the central nervous system can be transmitted to the optic papilla through the perioptic space. Variations in the diameter of the perioptic space serve as an important indicator for select intracranial pathologies in the pediatric population. Though the perioptic space can be evaluated using various imaging modalities, MRI is considered highly effective due to its superior soft tissue resolution. With advancement in MR imaging…
