Creatine kinase alterations after acute malignant hyperthermia episodes and common surgical procedures.

Skeletal muscle may release creatine kinase (CK) during a malignant hyperthermia (MH) episode; however, muscle damaged during surgery may al … K in the range of most surgical procedures, and approximately 50% of MH positive patients not given succinylcholine had peak CK similar to those of most surgical procedures. Dantrolene did not significantly alter peak CK in MH positive patients; however, succinylcholine was associated with significantly higher peak CK. These data suggest that patients who have ha …

Simultaneous posterior and anterior interosseous nerve syndromes.

A patient with simultaneous posterior and anterior interosseous nerve syndromes is presented. During induction of anesthesia, the paralyzed muscles exhibited denervation sensitivity to succinylcholine. Following exploration and repair of a posterior interosseous nerve defect, full function returned in the dorsal muscles as well as the flexor pollicis longus and pronator quadratus muscles.

The risk of malignant hyperthermia in children undergoing muscle biopsy for suspected neuromuscular disorder.

BACKGROUND: It is assumed that those children with known or suspected neuromuscular disorders (NMD) are at increased risk for malignant hype … dergone muscle biopsy for suspected NMD. Of these, only 274 received a volatile anesthetic agent or succinylcholine and were included for study. Records were examined for evidence of MH or rhabdomyolysis. RESULTS: No patient exhibited signs or symptoms suggestive of MH. One patient was found, by muscle biopsy, to have evidence of rhabdomyolysis prior to exposure …

[Clinical assessment of laryngeal mask airway in general anesthesia with spontaneous breathing].

The laryngeal mask airway (LMA) is a new form of oropharyngeal airway that provides an alternative to endotracheal intubation and face mask … urse of the anesthesia. Blood pressure and heart rate were compared on induction in group I and II. Succinylcholine 1 mg/kg, thiopentone 4-6 mg/kg and fentanyl 2 micrograms/kg were used in group I while using the same dosage of succinylcholine and thiopentone without fentanyl for group II. We found no significant differences in vital signs between the two groups.

[20 years' experience using anesthesia in pediatric bronchology].

The authors inform about their experience with anesthesia gained in the last 20 years with 50,000 bronchological examinations in children. Different anesthesia methods are discussed. They conclude that local anesthesia in adults and in adolescents is a possible procedure using fiberscope. In infants and children the most preferred anesthesia is halothane-nitrous oxide-oxygen-anesthesia with relaxation by succinylcholine.

Malignant Hyperthermia: Review of Diagnosis and Treatment during Cardiac Surgery with Cardiopulmonary Bypass.

Malignant hyperthermia is a potentially life-threatening hypermetabolic disorder, often induced by exposure to volatile anesthetics and succinylcholine. There are few reports of malignant hyperthermia during cardiopulmonary bypass. Here the authors review available literature including case reports of malignant hyperthermia and cardiopulmonary bypass and discuss the potential implications of malignant hyperthermia diagnosis and management as it applies to cardiac surgery.

Anesthetic induction trismus, more than a closed-mouth problem.

Trismus, or masseter hypertonia, that results from the use of succinylcholine during induction of anesthesia is a rare and dangerous phenomenon. It presents to the anesthesiologist the immediate problem of airway management but it also must be recognized by the physician as a harbinger of malignant hyperthermia. We report a case of induction trismus and discuss its association with malignant hyperthermia. The pathophysiology, diagnosis and treatment of malignant hyperthermia are reviewed.

Incidence and factors associated with out-of-hospital peri-intubation cardiac arrest: a secondary analysis of the CURASMUR trial.

The Incidence of peri-intubation cardiac arrest (PICA) has been rarely assessed in the out-of-hospital setting. The objectives of this study were to assess the incidence and factors associated with PICA (cardiac arrest occurring within 15 min of intubation) in an out-of-hospital emergency setting, wherein emergency physicians perform standardized airway management using a rapid sequence intubation technique in adult patients. This was a secondary analysis of the "Succinylcholine versus Rocuronium for out-of-hospital emergency intubation" (CURASMUR) trial, which compared the first attempt intub…

Hyperkalemic cardiac arrest during anesthesia in infants and children with occult myopathies.

In 1992, the Malignant Hyperthermia Association of the United States and The North American Malignant Hyperthermia Registry received reports of cardiac arrest in apparently healthy children given succinylcholine. Using data from 1990 to 1993, this study analyzes: (1) etiology of all reported pediatric arrests and (2) whether survival was associated with certain patient or treatment variables. We reviewed retrospectively all reports of pediatric (age < 18 years) arrests occurring within 24 hours of anesthesia. Etiology of arrests and presence of myopathy were determined. Twenty-five patients (9…

Drug-induced hyperthermia and rhabdomyolysis during the perioperative period: report of three patients.

Drug-induced hyperthermia is one condition that anesthesiologists may meet even though it is uncommon, it is life threatening. We report 3 cases of patients at Siriraj Hospital, Mahidol University who developed drug-induced hyperthermia and rhabdomyolysis from different mechanisms. In two of them, the diagnosis was suspected malignant hyperthermia. Rigidity, hyperthermia and tachyarrhythmia developed just after inhalation induction (halothane and sevoflurane) and intubation with succinylcholine. The other case was the result of amphetamine abuse. He also had received both succinylcholine and i…

Malignant hyperthermia: a potentially fatal complication of anesthesia.

Malignant hyperthermia (MH) is an uncommon cause of anesthetic-induced death; it is an inherited disorder that is triggered by the administration of anesthetic drugs. MH is triggered by the administration of volatile anesthetic agents and succinylcholine, a depolarizing muscle relaxant. It is imperative that the perianesthesia staff be familiar with the pharmacokinetics and pharmacodynamic properties of dantrolene, which is the only known agent effective in the treatment and prophylaxis of MH.

Early Neurophysiological Monitoring of Train of Four Assists in the Detection of Pseudocholinesterase Deficiency.

A craniotomy with cortical and subcortical mapping was planned for a 64-year-old male with a large right frontotemporal brain mass. Total intravenous anesthesia was performed, and 200 milligrams of succinylcholine was administered at induction. A train of four prior to head pinning (52 minutes after succinylcholine administration) revealed zero of four twitches in the left hand and foot. The patient did not regain spontaneous breathing despite discontinuation of infusions and the surgeon canceled the case at 108 minutes from induction. The patient was safely extubated at 270 minutes. Pseudocho…

Malignant Hyperthermia in PICU-From Diagnosis to Treatment in the Light of Up-to-Date Knowledge.

Malignant Hyperthermia (MH) is a rare, hereditary, life-threatening disease triggered by volatile anesthetics and succinylcholine. Rarely, MH can occur after non-pharmacological triggers too. MH was detected more often in children and young adults, which makes this topic very important for every pediatric specialist, both anesthesiologists and intensivists. MH crisis is a life-threatening severe hypermetabolic whole-body reaction. Triggers of MH are used in pediatric intensive care unit (PICU) as well, volatile anesthetics in difficult sedation, status asthmaticus or epilepticus, and succinylc…
