Role of somatostatin and apoptosis in breathing control in sudden perinatal and infant unexplained death.

OBJECTIVES: To obtain basic information about the expression of somatostatin in the human central nervous system and, in particular, to evaluate its possible involvement in unexplained perinatal and in sudden infant death syndrome. MATERIAL: Sixty-seven brainstems from subjects aged from 30 gestational weeks to 12 postnatal months, dying of both known and unknown causes, were selected for this study. The unexplained deaths included 17 sudden intrauterine deaths, 5 sudden neonatal deaths and 28 sudden infant deaths. METHOD: All brainstems were fixed in 10% phosphate-buffered formalin, process…

Role of virus-induced myocardial affections in sudden infant death syndrome: a prospective postmortem study.

The cause of sudden infant death syndrome (SIDS) is an unresolved problem of high relevance. Previous studies indicate a role of infections. In our prospective study, we investigated the frequency of virus-induced myocardial affections in SIDS. Postmortem samples from SIDS victims and control subjects were investigated prospectively. Pediatric cases of unnatural death served as controls. Samples were studied for enteroviruses, adenoviruses, parvovirus B19, and Epstein-Barr virus applying PCR. Immunohistochemical investigations for inflammatory cells, the necrosis marker C5b-9((m)) complement c…

Parallel incidences of sudden infant death syndrome and infantile hypertrophic pyloric stenosis: a common cause?

OBJECTIVE: To determine whether there was a correlation between the incidence of infantile hypertrophic pyloric stenosis (IHPS) and the incidence of sudden infant death syndrome (SIDS) during the period 1970 to 1997 and to discuss different causative factors that could be influencing the changing trend in incidence. METHODS: We compared the incidence of IHPS in the Stockholm Health Care Region with the incidence of SIDS in Sweden each year between 1970 and 1997. First, the relation was assessed by calculation of a correlation coefficient; second, the relative linear decrease was estimated for…

The apparent impact of gestational magnesium (Mg) deficiency on the sudden infant death syndrome (SIDS).

Reports of studies in humans as well as data from several animal models show the importance of maternal dietary magnesium for the growth, development and survival of offspring. Published findings in mothers of victims of sudden infant death syndrome (SIDS) and in the SIDS victims are compared with characteristics of magnesium deficiency in humans and animals. Observations concerning the level of magnesium in traditional diets of selected ethnic groups with the highest or lowest rates of SIDS appear to confirm the importance of magnesium in protecting the offspring from sudden death. Ethnic gro…

Interactions between maternal smoking and other prenatal risk factors for sudden infant death syndrome (SIDS).

In numerous investigations, maternal smoking increases the risk of sudden infant death syndrome (SIDS). In the present study we investigated whether prenatal risk factors for SIDS modify the effect of maternal smoking on SIDS mortality. We analysed data from a population-based cohort study (222 cases, 260,604 infants at risk) within the Westphalian Perinatal Inquiry in Germany between 1990 and 1994. In the stratified analysis, smoking was classified into non-smoking, moderate (1-10 cigarettes/d) and heavy smoking (> 10 cigarettes/d). Multiplicative interactions between smoking and other prenat…

Loss of neonatal hypoxia tolerance after prenatal nicotine exposure: implications for sudden infant death syndrome.

Maternal cigarette smoking has a high correlation with sudden Infant Death Syndrome, a condition in which cardiorespiratory failure occurs during an hypoxic episode, as in sleep apnea. Pregnant rats were given nicotine infusions of 2 or 6 mg/kg/day throughout gestation, regimens that produce plasma nicotine levels spanning the range in smokers. The day after birth, animals in the high dose group displayed excessive mortality during hypoxic challenge. These animals were found to be deficient in an essential response component, namely adrenomedullary catecholamine release that is required to mai…

Defects in mitochondrial fatty acid oxidation: clinical presentations and their role in sudden infant death.

Defects in mitochondrial beta-oxidation may result in severe metabolic crisis after metabolic stress. The combination of hypoketotic hypogly … iarrhoea or vomiting. Collectively, such disorders are relatively common and their association with sudden infant death leads to a considerable demand for laboratory investigation. Currently the laboratory methods available are not capable of dealing with this demand in a rational and cost-effective manner and it is necessary to have careful clinically- and pathologi …

Pancreatic islet abnormalities in sudden infant death syndrome. Qualitative and quantitative analyses of 15 cases.

The pancreata of 15 autopsy cases of sudden infant death syndrome (SIDS) and those of 14 age-matched controls were examined qualitatively and quantitatively to re-evaluate the relationship between pancreatic islet abnormalities and sudden death in infancy. Histopathologically, a diffuse or focal form of nesidioblastosis and septal islets were frequently observed in the pancreata of both groups. Endocrine cell dysplasia was found only in 2 infants who had died of SIDS. Quantitatively, there was little difference of islet cell composition between the SIDS cases and the controls. A relatively hig…

[Changes in the cardiovascular chemo-baroreceptors as a possible basis for sudden infant death (SIDS)].

A male sixty days old infant, dying suddenly, has been diagnosed as sudden infant death syndrome (SIDS). Autopsy visualized plurifocal haematic suffusions, typical for an asphyxial status. Abnormalities were not found in the conduction system, but aorticopulmonary paraganglia hyperplasia with mild inflammatory characters has been noted. The role of aorticopulmonary paraganglia, as baro-chemoreceptors, in the cardiovascular regulation, has been anatomical substrate for explaining a sudden infant death, according to a multifactorial theory of SIDS.

[The differential diagnosis of sudden infant death or child abuse--a case of fatal hyperthermia].

During summer-like outdoor temperature, a seven-month old female infant was put into a sleeping bag, wrapped up with a blanket and tied up by her mother and a friend of hers because of unrest and continuous screaming. Prior to that, between 0.30 a.m. and 0.45 a.m., occurred the rectal measurement of temperature: 37.5 degrees C. The infant died one to two hours later. At 10.00 a.m., about 8 hours after death, in the criminal investigations of the police measurements of temperature were made: rectal 38.1 degrees C; in the pharyngeal cavity 29.5 degrees C; surrounding temperature 22.2 degrees C.

A hypothesis concerning Clostridium perfringens type A enterotoxin (CPE) and sudden infant death syndrome (SIDS).

This study identifies the presence of Clostridium perfringens type A enterotoxin (CPE) in some gastrointestinal and serum samples from babies who had died of the Sudden Infant Death Syndrome (SIDS) and other causes. On occasion antibodies to this toxin were identified in sera. CPE is parasympathomimetic in its action. In the adult food poisoning model it is produced when the organism sporulates in vivo. This leads to speculation as to whether this toxin may play an ante-mortem role in the dying process of infants, either in the agonal stages or as a causative factor in SIDS.

[Anatomo-pathologic study of the temporal bone in our 4 cases of sudden infant death].

A new etiopathogenic theory is proposed by the authors about four cases of sudden infant death syndrome. A new histological method on the temporal bone was used applied in these four cases. Inner ear and meningeal injuries were observed. From this observation they proposed the possibilities of brain-stem injury through neurogenic or meningeal pathways. These injuries could be responsible for apnea syndrome and loss of alertness.

Near-miss sudden infant death syndrome in eight infants with sleep apnea-related cardiac arrhythmias.

Eight full-term infants between 3 and 8 weeks of age, who had had 24 to 48 hours of vague problems involving a congested upper airway and/or irritability, had a life-threatening respiratory episode at home, called a near-miss for sudden infant death syndrome event. Polygraphically monitored mixed apneas occurred in clusters during non-rapid eye movement (NREM) sleep with significant cardiac arrhythmias, predominantly sinus arrest. The cardiorespiratory problems continued during sleep in the following 8 to 12 weeks but did not recur after that time, and there were no long-term sequelae when the…

Inconsistency in Maternal Report of Infant Sleep Characteristics: Results from a National Survey and Implications for Sudden Unexpected Infant Death.

OBJECTIVES: To evaluate infant sleep practices and risk factors for sudden unexpected infant Death (SUID) and and to explore the relationship of sleep practices to breastfeeding duration. STUDY DESIGN: Using national data from the Pregnancy Risk Assessment Monitoring System, 2016-2021, we evaluated infant sleep practices and tobacco use among 181,075 mothers of infants born at term. We compared how often mothers reported infants sleeping "alone in their own crib," with their reports of "usual" sleep surface using descriptive statistics. Because of disparities in SUID by race and ethnicity, we

Trends in postneonatal aspiration deaths and reclassification of sudden infant death syndrome: impact of the "Back to Sleep" program.

OBJECTIVE: The introduction of the "Back to Sleep" campaign for the prevention of sudden infant death syndrome (SIDS) brought with it concern that there might be an increase in the incidence of aspiration-related deaths. The objective of this analysis was to describe the trends in postneonatal mortality and proportionate mortality ratios for the United States for the years 1991 to 1996 for aspiration-related deaths and other causes to which a SIDS death could conceivably be reclassified. METHODS: Linked birth and infant death vital statistic files for the United States were used for the years…

First Brazilian birth after Preimplantation Genetic Testing for Severe Methylmalonic Acidemia: preventing sudden infant death.

Methylmalonic acidemia (MMA) is a rare autosomal recessive metabolic disorder caused by pathogenic variants in the MMUT gene, often associated with severe neonatal manifestations and sudden unexpected death in infancy (SUDI). Preimplantation Genetic Testing for Monogenic Diseases (PGT-M) offers a valuable reproductive option for at-risk couples seeking to prevent transmission of such conditions. This represents the first Brazilian case of a healthy birth following combined PGT-M and PGT-A for MMA known. After the neonatal loss of their first child due to mut0-type MMA, a couple underwent genet
