Early subtypes and progressions of progressive supranuclear palsy: a data-driven brain bank study.

BACKGROUND: Progressive supranuclear palsy (PSP) is typically characterized by vertical supranuclear gaze palsy and early falls, referred to as Richardson's syndrome (PSP-RS). Other presentations include postural instability (PSP-PI), Parkinsonism (PSP-P), speech/language disorder (PSP-SL), frontal presentation (PSP-F), ocular motor dysfunction (PSP-OM), and corticobasal syndrome (PSP-CBS). However, differences across the early presentations and their subsequent clinical courses remain to be elucidated. OBJECTIVE: This study aimed to characterize early PSP subtypes and their subsequent progre

Non-fluent progressive aphasia, depression, and OCD in a woman with progressive supranuclear palsy: neuroanatomical and neuropathological correlations.

This paper details the case of a 64-year-old woman who presented to the psychiatry service with worsening mood in the context of a diagnosis of obsessive-compulsive disorder (OCD). On further examination she was found to have clinical findings consistent with frontotemporal lobar degeneration of the non-fluent progressive aphasia subtype. At post-mortem she was found to have progressive supranuclear palsy. We argue, in retrospect, that her OCD was likely prodromal to the development of her dementia. This case highlights the fact that frontotemporal lobar degeneration/progressive supranuclear p

Longitudinal monitoring of tau aggregation in progressive supranuclear palsy with [18F]PI-2620 PET.

INTRODUCTION: Progressive supranuclear palsy (PSP), a 4-repeat tauopathy, can be visualized using [18F]PI-2620 tau positron emission tomography (PET). However, the value of sequential [18F]PI-2620 imaging for tracking tau accumulation during the disease course has not yet been investigated. METHODS: Twenty-three PSP patients underwent two [18F]PI-2620 PET scans (interval: 21.4 ± 4.3 months) and were compared to cross-sectional data from 25 healthy controls. Regional volume of distribution ratio values were analyzed for longitudinal tau changes, clinical correlations, and network-based propaga

Deep brain stimulation in progressive supranuclear palsy: a dead-end story? A narrative review.

Progressive supranuclear palsy (PSP) is a rare, debilitating neurodegenerative disorder that significantly impairs both motor and cognitive functions. Current pharmacological treatments offer only transient symptomatic relief, driving interest in the past in alternative therapeutic strategies such as deep brain stimulation. Deep brain stimulation (DBS), known for its success in treating motor symptoms of Parkinson's disease, has been explored as a possible symptomatic treatment for PSP, considering the pedunculopontine nucleus (PPN), involved in motor control and postural stability, as a promi

Tau positron emission tomography in the Parkinson and Richardson subtypes of progressive supranuclear palsy.

Progressive supranuclear palsy (PSP) is an atypical form of parkinsonism affected by deficiencies regarding efficient methods of examination, thereby hindering the establishment of an early and certain diagnosis prior to post-mortem examination. Most studies are based on magnetic resonance imaging analyses; however, there is growing interest in the evaluation of radiotracers in positron emission tomography (PET). In this narrative review, we attempt to elaborate on the significance of tau radiotracers and other PET biomarkers in the context of examination of two major subtypes of PSP: Richards

Inflammation PET and plasma neurofilament light predict survival in people with progressive supranuclear palsy.

Progressive supranuclear palsy (PSP) is a primary tauopathy characterized by atrophy and neuroinflammation of the brainstem, the basal ganglia and, to a lesser degree, the cortex. This study investigates the association of regional atrophy (structural MRI), neuroinflammation ([11C]-PK11195 PET), peripheral markers of neurodegeneration [plasma neurofilament light chain (NfL)] and clinical severity [PSP rating scale (PSPRS)] with survival in people with PSP. Fifty-nine people with PSP underwent longitudinal structural MRI, surviving on average 3.2 years from the first scan (MRI cohort). Sixteen

Progressive supranuclear palsy against everyone else: a look into the specificity of MRI markers.

INTRIDUCTION: Diagnostic accuracy of the clinical diagnosis of progressive supranuclear palsy (PSP) is suboptimal, due to its' clinical heterogeneity. Multiple MRI markers have been introduced in an effort to enhance clinical diagnosis. OBJECTIVES: This study compared the diagnostic accuracy of linear, planimetric and volumetric MRI markers for PSP diagnosis in different clinical settings. MATERIALS AND METHODS: A total of 414 patients were included, comprising 80 PSP and 334 non-PSP patients (CBD:38; MSA:45; PD:36; AD:65; bvFTD:30; PPA:18; DLB:15; PCA:14; NPH:15; MCI:14; controls:45). The significance of glial cell line-derived neurotrophic factor analysis in Progressive Supranuclear Palsy.

Progressive Supranuclear Palsy (PSP) is an atypical parkinsonism. Major subtypes of the disease: PSP-Richardson's Syndrome (PSP-RS) and PSP Parkinsonism Predominant (PSP-P) vary in clinical features, the pathomechanism remains unexplored. The aim of this work is to analyze the relevance of glial cell line-derived neurotrophic factor (GDNF) evaluation in the serum and cerebrospinal fluid (CSF) in PSP subtypes and to verify its significance as a possible factor in the in vivo examination. Authors assessed the concentration of GDNF in the serum and CSF of 12 patients with PSP-RS, 12 with PSP-P an

Serum Oligomeric α-Synuclein and p-tau181 in Progressive Supranuclear Palsy and Parkinson's Disease.

Clinical differentiation of progressive supranuclear palsy (PSP) from Parkinson's disease (PD) is challenging due to overlapping phenotypes and the late onset of specific atypical signs. Therefore, easily assessable diagnostic biomarkers are highly needed. Since PD is a synucleopathy while PSP is a tauopathy, here, we investigated the clinical usefulness of serum oligomeric-α-synuclein (o-α-synuclein) and 181Thr-phosphorylated tau (p-tau181), which are considered as the most important pathological protein forms in distinguishing between these two parkinsonisms. We assessed serum o-α-synuclein

Systematic assessment of square-wave jerks in progressive supranuclear palsy: a video-oculographic study.

BACKGROUND: The presence of frequent macro-square-wave jerks (SWJs) has been recently included in the diagnostic criteria for progressive supranuclear palsy (PSP). The aim of the current video-oculographic study was to systematically assess the presence and features of SWJs during a brief fixation task in PSP, in comparison with Parkinson's disease (PD) patients and healthy controls (HC). METHODS: Thirty-eight PSP patients, 55 PD patients and 40 HC were enrolled in the study. All patients underwent a video-oculographic (VOG) examination including a 5-s fixation task, and the number, duration

Astrocyte tau deposition in progressive supranuclear palsy is associated with dysregulation of MAPT transcription.

Progressive supranuclear palsy (PSP) is a neurodegenerative disease characterized by 4R tau deposition in neurons as well as in astrocytes and oligodendrocytes. While astrocytic tau deposits are rarely observed in normal aging (so-called aging-related tau astrogliopathy, ARTAG) and Alzheimer's disease (AD), astrocytic tau in the form of tufted astrocytes is a pathognomonic hallmark of PSP. Classical biochemical experiments emphasized tau synthesis in neurons in the central nervous system, suggesting that astrocytic tau inclusions might be derived from uptake of extracellular neuronal-derived t

Ethnic background and distribution of clinical phenotypes in patients with probable progressive supranuclear palsy.

BACKGROUND: Progressive Supranuclear Palsy (PSP) is a sporadic neurodegenerative disease without a clear geographic prevalence. Cohorts studied in the UK and India showed no higher prevalence of atypical parkinsonism in South Asian patients. We describe the ethnic and racial background of PSP patients in the Greater Toronto Area (GTA), Canada. METHODS: A prospective observational study of patients with clinically probable PSP evaluated at the dedicated Rossy PSP program. Demographic and clinical data were collected at baseline including PSP phenotype. Results were compared with the latest dem

Influences of motor speech impairments on the presentation of dysphagia in progressive supranuclear palsy.

PURPOSE: The purpose of this study was to examine whether differences in motor speech features are related to presentations of dysphagia in progressive supranuclear palsy (PSP) given the sparsity of data examining this relationship. METHOD: Motor speech disorder (MSD) type and severity along with specific swallowing variables were analysed to obtain insights among these relationships in 73 participants with PSP. RESULT: Results revealed that most participants (93%) had dysarthria, with 19% having co-occurring apraxia of speech (AOS). Greater MSD severity was related to more severe pharyngeal

Differential cholinergic systems' changes in progressive supranuclear palsy versus Parkinson's disease: an exploratory analysis.

Prior studies indicate more severe brainstem cholinergic deficits in Progressive Supranuclear Palsy (PSP) compared to Parkinson's disease (PD), but the extent and topography of subcortical deficits remains poorly understood. The objective of this study is to investigate differential cholinergic systems changes in progressive supranuclear palsy (PSP, n = 8) versus Parkinson's disease (PD, n = 107) and older controls (n = 19) using vesicular acetylcholine transporter [18F]-fluoroethoxybenzovesamicol (FEOBV) positron emission tomography (PET). The Overlap in Neuroimaging Findings Between Idiopathic Normal Pressure Hydrocephalus and Progressive Supranuclear Palsy.

BACKGROUND: The neuroimaging findings of brain stem atrophy resembling progressive supranuclear palsy (PSP) radiology are common in idiopathic normal pressure hydrocephalus (iNPH) subjects. Besides, recent studies report the existence of iNPH-like MRI findings in PSP subjects. We aimed to comparatively investigate the neuroimaging indices of iNPH and PSP in our patient groups in a detailed methodology. METHODS: Ultimately, 19 probable PSP patients and 18 patients with a definite diagnosis of iNPH were enrolled. The subjects were recruited retrospectively from those who had been admitted betwe

A novel diagnostic marker for progressive supranuclear palsy targeting atrophy of the subthalamic nucleus.

INTRODUCTION: Despite great progress in radiological diagnostic tools for neurodegenerative disorders, their diagnostic accuracy has been unsatisfactory. One of the pathological hallmarks of progressive supranuclear palsy (PSP) is atrophy of the subthalamic nucleus, which has not attracted much attention for imaging analysis. METHODS: The clinical data of patients with PSP, multiple system atrophy (MSA), Parkinson's disease (PD), and corticobasal syndrome (CBS) who underwent brain magnetic resonance imaging at our department between June 2019 and March 2020 were retrospectively reviewed.
