BACKGROUND: Tracheomalacia with anterior great vessel compression is a common disorder in infants and children, which can lead to life-threatening airway occlusion. In this study, a large number of patients underwent anterior aortopexy to provide a more normal distal airway.

OBJECTIVE: Orthostatic hypotension is usually a benign event. However, some patients are disabled by frequent syncopal events, and vertical transportation during helicopter rescue, for example, may even be fatal. Bladder suspension for urinary incontinence secondary to bladder neck hypermobility is highly successful. We performed laparoscopic-assisted colposuspension combining features of Burch retropubic and transvaginal needle colposuspension for the treatment of genuine stress incontinence in 10 women. In six patients affected by congenital ptosis, a disposable epidural anesthesia needle was used instead of a Wright needle for frontalis suspension surgery. For each of the patients in this study, the use of a disposable needle permitted smaller supra-brow area incisions with a minimal scar. The needle also created a smaller breach in the plane of levator aponeurosis and preaponeurotic fat, causing less tissue edema and bleeding than with a Wright needle.

PURPOSE: The Burch suspension is an effective treatment for stress urinary incontinence due to hypermobility. To decrease the associated morbidity and hospital stay, we attempted the procedure laparoscopically in 46 women.

Patients with proven stress incontinence were subjected to surgical treatment which combined both sling and Stamey-Pereyra techniques. A 3-4 cm long and 1,5-2 cm wide sling of Bulgarian polyamide mesh was placed under the urethrovesical junction using a small vaginal incision. Bladder neck suspension operations are commonly performed for stress incontinence. Cure is achieved by increasing proximal urethral pressure by repositioning the bladder neck in a supported retropubic position. We reviewed the charts of 206 patients who underwent the Raz bladder neck suspension between January 1984 and June 1990 for stress urinary incontinence. Mean followup was 15 months. Overall, our results demonstrated a successful outcome (cure or rare stress urinary incontinence not requiring protection) in 186 of 206 patients (90.3%). A high-performance liquid chromatographic analysis of phenylpropanolamine (PPA) in human plasma is presented. Using direct UV detection the method is sufficiently sensitive to 25 ng/ml for PPA. Single-dose pharmacokinetics of PPA in 10 healthy volunteers taking controlled release suspension (C) or tablet (T), each containing 150 mg PPA was compared. The study described here has shown that the (C) AUC(0-infinity)/(T) AUC(0-infinity) = 1.02 (P greater than 0.4). The controlled-release product of PPA is equivalent to PPA tablet.
