[Videorentgenocinematographic analysis of pharyngeal phase in the act of swallowing].

The course of the pharyngeal phase was analysed in physiological conditions. The mobility of anatomical structures taking part in the phase was evaluated, taking into consideration the time sequence in which the elements of the phase followed. To objectify the evaluation of the analysed phenomena, authors of this paper proposed and used linear and time parameters also used by other authors involved with these issues.

Videoendoscopic swallowing study for diagnosis of Zenker's diverticuli.

The videoendoscopic swallowing study (VESS) provides an efficient, cost-effective method for swallowing evaluation in the clinic. Previously thought to be of little value for disorders of the proximal esophagus, this retrospective study (n = 33) demonstrates a high level of sensitivity (85%) and specificity (86%) of VESS for detecting Zenker's diverticuli in patients with symptoms of swallowing dysfunction. Furthermore, the key finding of postswallow reflux into the hypopharynx reliably distinguishes cricopharyngeus dysfunction from diverticuli in this population. This study shows that VESS ca

Sensorimotor modulation of human cortical swallowing pathways.

1. Transcranial magnetic stimulation over motor areas of cerebral cortex in man can activate short latency bilateral cortical projections to the pharynx and oesophagus. In the present paper we investigate the interaction between pathways from each hemisphere and explore how activity in these pathways is modulated by afferent feedback from the face, pharynx and oesophagus. 2. Comparison of unilateral and bilateral stimulation (using interstimulus intervals (ISIs) of 1, 5 or 10 ms between shocks) showed spatial summation of responses from each hemisphere at an ISI of 1 ms, indicating that cortic

Clinical measurement of swallowing in health and in neurogenic dysphagia.

We studied clinical features potentially related to dysphagia and three indices from a timed test of swallowing--average volume per swallow (ml), average time (s) per swallow and swallowing capacity (ml/s)--in 181 screened healthy adults and 30 patients with motor neurone disease (MND). In healthy adults, age, sex and height accounted for 44.3% and 55.6% of the variance of log average volume per swallow and log swallowing capacity, respectively. Symptoms and signs were more prevalent in the MND group and were associated with reduced swallowing capacity and reduced average volume per swallow; r

Timing of glottic closure during normal swallow.

BACKGROUND: The purpose of this study was to determine the temporal characteristics and patterns of glottic closure during normal swallow using concurrent videofluoroscopy and videoendoscopy. METHODS: Three swallows each of four bolus types were examined in eight healthy volunteers with the endoscope in two positions: at the level of the uvula and at the laryngeal vestibule. Data analysis compared: (1) temporal relationships between laryngeal behaviors and oropharyngeal swallow events and (2) airway conditions at the time of key oropharyngeal events. RESULTS: Although arytenoid adduction and

A swallow induction avoidance procedure to establish eating.

Swallow induction has been used to shape swallowing behavior in dysphagic children and to accelerate swallowing in nondysphagic children with profound mental retardation who display primitive swallows. Swallow induction may be considered a type of prompt. This project coupled swallow induction with a modified delayed prompting paradigm to establish eating in a 3.5-year-old girl. Coupling these procedures produced prompt swallowing and established oral consumption. Follow-ups at 1, 2, 6 and 12 months demonstrated maintenance and further improvement of the newly acquired feeding skills. Implicat

Ovine fetal swallowing and renal responses to oligohydramnios.

Amniotic fluid (AF) volume regulation is dependent on a balance between fluid production and fluid resorption. We examined the effects of reduced AF volume on AF production by fetal urine and resorption by fetal swallowing and the response of these parameters to AF volume replacement. Eight time-dated pregnant ewes (125 +/- 1 days gestation) were studied before (day 1) and after (day 3) AF and fetal urine drainage. Drainage resulted in a significant decrease in AF volume (415 +/- 89 to 157 +/- 36 ml). Fetal urine osmolality increased (139 +/- 10 to 286 +/- 33 mosmol/kgH2O), while urine flow di

Solid bolus swallowing in the radiologic evaluation of dysphagia.

Patients with dysphagia, heartburn and chest pain are regularly referred for radiologic evaluation of swallowing. The liquid barium swallow has been of great value for the biphasic evaluation of the pharynx and esophagus. Though many patients complain of dysphagia specifically for solids, solid bolus swallow is usually not part of the evaluation. For the present study we therefore included the use of a solid bolus with a diameter of 13 mm and interviewed the patients carefully for any symptoms during this tablet swallow. Of 200 patients examined, the tablet passed through the esophagus without

Topographic representation of the pharyngoesophageal segment during swallowing.

Motility of the pharyngoesophageal segment (PES) was monitored by cineradiography (50 frames a second) during barium swallow and analyzed by a computerized topographic mapping of sagittal wall motion. Through measurement of wall displacement on 20 consecutive levels of the PES, distention, contraction, and peristalsis were studied. Topographic mapping of three patients with normal PES motility, delayed opening of the cricopharyngeus, and incomplete opening of the cricopharyngeus, was performed. These topographic mappings show that "narrowing" at the level of the cricopharyngeus reflects expans

Swallowing disorders following skull base surgery.

The modern otolaryngologist-head and neck surgeon has the technical ability to perform a wide range of surgical procedures at the skull base. Associated with some of these operations are swallowing deficits secondary to cranial nerve paralyses or anatomic disturbances. The skull base surgeon must physically and emotionally prepare patients preoperatively for these functional disabilities.

Swallowing-induced tachycardia: electrophysiologic and pharmacologic observations.

A 38-year-old man developed palpitations after swallowing. Intracardiac recordings and esophageal manometry were obtained during episodes of swallowing-induced tachycardia. These studies demonstrated that the site of origin of the tachycardia was the high right atrium and that the onset of the tachycardia occurred prior to the arrival of the peristaltic wave in the esophagus. Evaluation of the tachycardia revealed that the likely mechanism for the tachycardia was triggered automaticity. Autonomic blockade and other pharmacologic interventions failed to prevent episodes of tachycardia. Swallowi

Interaction between licking and swallowing in the drinking rat.

Licking behavior of rats appeared to be interrupted periodically for about 20 ms. Behavioral and electrophysiological evidence is presented which indicates that during this period swallowing occurs. The hypothesis was tested that swallowing initiation depends on the volume of water that has accumulated in the oral cavity during drinking. No support for this hypothesis was obtained when the flow rate of water through the drinking tube was manipulated. Alternative hypotheses are discussed.

Relationship between swallow rate and salivary flow.

Recent studies indicate that swallow-induced, primary peristalsis is a major determinant of normal esophageal acid clearance. However, factors that regulate the rate of spontaneous swallowing in normal subjects are incompletely understood. We postulated that the rate of salivary flow influences the rate of spontaneous swallows. To test this hypothesis, we did a total of 60 studies measuring salivary flow or the rate of spontaneous swallowing in 10 healthy volunteers, age 10-30 years. Saliva was collected by expectoration. Swallow rate was recorded by a small, transnasal catheter stationed in t

Symptomatic swallowing-induced paroxysmal supraventricular tachycardia.

A 55 year old woman with palpitations during ingestion of food or drink was evaluated with surface, esophageal and dynamic electrocardiographic techniques. Recordings documented the consistent induction of supraventricular tachycardia by swallowing; no definable gastrointestinal or other cardiac abnormality was present. Nine previously reported cases are reviewed, with special reference to the role of vagovagal reflexes in the pathogenesis of this unusual clinical syndrome.

Barn swallows and purple martins convert precursors to long-chain polyunsaturated fatty acids: implications for conservation of riparian- vs inland-nesting habitats.

For aerial insectivorous birds, whose populations have declined significantly in North America, long-chain polyunsaturated fatty acids (LC-PUFA) that are more abundant in aquatic-emergent insects than terrestrial insects, are important for the development, somatic growth, and health of young birds. Some bird species, however, can convert shorter chain PUFA that occur abundantly in terrestrial insects into LC-PUFA. Our study aimed to evaluate the ability of two aerial insectivore species to synthesize their own LC-PUFA. We hypothesized that terrestrially associated aerial insectivores rely on h

Significant Temporal Association of Esophageal Air Events (Supragastric Belches, Air Swallows, and Gastric Belches) With Hiccups: A Case Study in an Adolescent.

We previously reported our ability to objectively assess the strength of temporal relationships between gastroesophageal reflux (GER) disease-like symptoms with esophageal air events (EAEs) (air swallows, gastric belching, and supragastric belching) using impedance-pH monitoring. Recently, we similarly evaluated an adolescent female who presented to our clinic with symptoms of recurrent hiccups and with what appeared to be numerous EAEs. Hiccups were associated with GER (symptom association probability [SAP] = 100%), air swallows (SAP = 99.7%), supragastric belches (SAP = 100%), and gastric be
