The treatment of pediatric deformities is relatively common in the typical foot and ankle practice. Talipes calcaneovalgus deformity represents one of the more prevalent deformities. This postural deformity, which is present at birth, is characterized by marked dorsiflexion and valgus position of the foot in relation to the leg. It is essential that an accurate diagnosis is established early and conservative treatment instituted. In most cases, the deformity is highly responsive to conservative therapy consisting of manipulation and casting. Deformity not identified at birth or soon thereafter

The treatment goal for pediatric cavovarus deformities is to neutralize plantar pressure distribution, reduce hindfoot varus deformity, and avoid or postpone ankle, midfoot, and hindfoot arthritis. If nonoperative treatment is not sufficient, surgical realignment must be discussed. Promising improvements in decision making and operative techniques have been published. To avoid disappointment owing to recurrence or failures of operative procedures, selection of the appropriate and preferably single operative procedure remains the most crucial factor for success. This article focuses on current

The TARP syndrome (Talipes equinovarus, Atrial septal defect, Robin sequence, and Persistent left superior vena cava) is an X-linked disorder that was determined to be caused by mutations in RBM10 in two families, and confirmed in a subsequent case report. The first two original families were quite similar in phenotype, with uniform early lethality although a confirmatory case report showed survival into childhood. Here we report on five affecteds from three newly recognized families, including patients with atypical manifestations. None of the five patients had talipes and others also lacked

A six-day-old girl, born to normal non-consanguineous parents presented with mask like facies with a small mouth giving a 'whistling' appearance. Other dysmorphic features include deep set eyes, broad nasal bridge, long philtrum and 'H' shaped cutaneous dimple on the chin. There was congenital windmill vane hand position and severe talipes equinovarus deformity. The above features are characteristic of Freeman-Sheldon syndrome also known as Whistling Face syndrome. Ultrasound scanning during 8(th) month of the pregnancy showed the fetus to have facial abnormality and bilateral clenched hand an

Foot deformities are common in arthrogryposis multiplex congenita. In this group of 52 patients with this diagnosis, 43 had foot deformities. The involvement was bilateral in all cases, and the most common type of deformity was talipes equinovarus (72 feet). The primary treatment in 52 of these patients was operative. Brockmann's procedure was the preferred method during the earlier years and posteromedial or posteromedioplantar release since 1974. Recurrences of the deformity are common; 36 operative procedures were done in 15 feet for recurrence of talipes equinovarus. The primary operative

PURPOSE OF REVIEW: The Coleman block test is a key component of the foot and ankle physical examination in children and adults. This review describes the Coleman block test and summarizes its implications in surgical decision making as well as its limitations.
RECENT FINDINGS: The Coleman block test is a classic technique for evaluating whether the hindfoot deformity in a cavovarus foot is flexible or rigid. It involves placing a wooden block underneath the lateral column of the foot to remove the effect of the plantarflexed first ray of the cavus deformity and assessing whether the hindfoot

The aim of hindfoot fusions in the cavovarus foot is to establish a painless, plantigrade, balanced and stable foot. A comprehensive clinical and radiographic assessment enables the surgeon to fully understand the patient's deformity and plan a reliable surgical strategy for deformity correction. Pre-operative planning and intraoperative techniques are discussed.

BACKGROUND: The outcome of a constant joint preserving procedure for painful plantar callosities with cavovarus foot remains unclear.
METHODS: Eleven patients (11 feet) who underwent lateral displacement calcaneal osteotomy (LDCO), dorsiflexion first metatarsal osteotomy (DFMO), and plantar fasciotomy (PF), simultaneously were included. The presence of painful callosities, heel alignment of standing (HA), and the Japanese Society for Surgery of the Foot ankle/hindfoot (JSSF) score were evaluated. Radiographically, the talonavicular coverage angle (TNCA), lateral talo-first metatarsal angle (L

The cavo varus foot is a complex pathology due to skeletal deformity and neuro-muscular unbalance. The key concept for a successful treatment is to consider the whole foot and ankle complex from a bone and soft tissue perspective. Undercorrection is the main issue in cavo varus foot management, which may be attributed to intrinsic correction defects of the described calcaneal osteotomies or to a lack of understanding about the pathology and the subsequent algorithm of treatment. The authors disclose their daily algorithm of treatment, considering the foot and ankle complex and the role of calc

Experiments, carried out by the author, have shown that the pathogenesis of congenital talipes is based on congenital muscular disbalance between flexors-supinators and extensors-pronators at the expense of strengthening of the former and insufficiency of the latter. An object of treatment of the considered developmental defect is restoration of myogenic balance. The existing methods of conservative treatment result in preservation of insufficiency of the anterolateral group of the crus muscles. Only the method of Vilensky V. Ya. allows to completely solve the posed problem. Operations on the

BACKGROUND: Tibialis anterior tendon transfer surgery forms a part of Ponseti management for children with congenital talipes equinovarus who, after initial correction, present with residual dynamic supination. Although retrospective studies support good outcomes, prospective longitudinal studies in this population are lacking.
QUESTIONS/PURPOSES: We assessed strength, plantar loading, ROM, foot alignment, function, satisfaction, and quality of life in patients with clubfoot that recurred after Ponseti casting who met indications for tibialis anterior tendon transfer surgery, and compared the

The cavus foot represents a complex spectrum of deformity ranging from the subtle idiopathic to the severe sensorimotor neuropathy and other neuromuscular deformities. The successful surgical treatment of the cavus foot depends on a fundamental understanding of the underlying multiplanar deformity, inherent muscle balance, and the rigidity of the hindfoot. The location of the deformity is described and understood according to its multiple apices. These deformities are addressed with osteotomies or arthrodesis directed at the apices of deformity. Simultaneously, correction of muscular imbalance

The cavovarus (cavus) foot is one of the most perplexing and challenging of all foot deformities and may prove to be one of the most difficult conditions to treat. This deformity is characterized by increased plantar flexion of the forefoot and midfoot in relation to the hindfoot resulting in high foot arch. Because cavus foot rarely occurs in an isolated form, the term "cavus foot" rather describes a part of a complex multiplanar foot deformity. Because the underlying disease is mostly neurogenic characterized by muscle imbalance in almost every case a combined bony and soft tissue surgery is

The cavovarus foot (CF) is a complex three-dimensional foot deformity. In addition to primary forms, secondary forms can be distinguished. The diagnosis of CF is made clinically; however, anamnestic information, a targeted examination including neurological status and at least radiological imaging using the hindfoot-centered imaging technique are required to determine the treatment. Conservative treatment for CF consists of the provision of insoles up to the adaptation of an orthopedic custom-made shoe, depending on the severity of the deformity. The indications for a surgical procedure are pr

The cavovarus foot is challenging to treat. The deformity is typically progressive with an unpredictable natural history. There are concurrent deformities in the fore-, mid-, and hindfoot: the medial arch is elevated, the first ray is plantarflexed, and the heel is in varus. Muscle imbalance and joint contractures are common. Successful correction requires confirmation of the underlying diagnosis and the application of principles to select the appropriate surgical procedures.
