Patellar tendon versus doubled semitendinosus and gracilis tendons for anterior cruciate ligament reconstruction.

The results of intraarticular anterior cruciate ligament reconstruction with either the patellar tendon or the semitendinosus and gracilis tendons (four strands) were prospectively compared in a consecutive series of 60 patients with chronic injuries. A single surgeon performed arthroscopically assisted reconstructions in an alternating sequence. Preoperative and operative data revealed no significant differences between the two groups. After 28 months of followup there were no significant differences in the incidence of symptoms, and recurrent giving way was present in only one knee with semi…

The use of hamstring tendons for anterior cruciate ligament reconstruction. Technique and results.

We feel that some of the current prejudice against use of hamstring tendon grafts for ACL reconstruction has not been justified if one critically reviews the literature. In this article, we have tried to provide the reader with our current indications, present our current surgical technique, and review some of the outcome studies involving use of the hamstring tendons for ACL reconstruction. We also feel that some of the poor results of hamstring tendon ACL reconstructions previously reported resulted from the use of inadequate strength grafts (single-stranded grafts) and lack of rigid fixatio…

Rupture of extensor tendons due to osteoarthritis of the distal radio-ulnar joint.

Extensor tendons ruptured in 12 patients as a result of osteoarthritis of the distal radio-ulnar joint. Rupture occurred without warning in ten cases and was sequential in five. Perforation of the dorsal capsule of the distal radio-ulnar joint, allowing contact between the roughened ulnar head and extensor tendons, was present in every case. The capsular performation was demonstrated by arthrography, which may be used to identify patients who are at risk of extensor tendon rupture. Loss of independent extension of the little finger is a valuable clinical sign because rupture of extensor digiti…

[Surgical correction of congenital bilateral dislocations of the extensor tendons of the thumb].

A congenital bilateral thumb-contracture due to dislocation of both extensor tendons over the metacarpophalangeal joint was treated in a fourteen-year-old boy. Intraoperatively the extensor tendons were shifted to a dorsal position and restrained over the center of the metacarpophalangeal joint of the thumb. By this method the previously useless thumbs were given normal function and cosmetic appearance. This congenital anomaly could not be classified as any known syndrome.

Irreducible fracture of the wrist in a child. Entrapment of the extensor tendons.

A previously undescribed fracture-separation of the distal radial and ulnar physis is presented. A Salter-Harris Type II phys-metaphyseal fracture of both radius and ulna, with volar displacement, was found irreducible by closed means. At operation the extensor tendons were found entrapped between the metaphysis and epiphysis of both radius and ulna, which prevented reduction. Reduction was possible only after this entrapment was released.

Mersilene strip suture in repair of disruptions of the quadriceps and patellar tendons.

Disruptions of the extensor mechanism of the knee most commonly involve fracture of the patella. Less frequently, patellar and quadriceps tendons are ruptured either spontaneously or by trauma. Surgical reconstruction of these tendon injuries is complicated by the difficulty in completely neutralizing tensional force across the repair. To overcome this problem, methods have been developed to reinforce the repair with overlapping flaps or fascia lata. Others advocate wire sutures or loops to relieve the tension. These techniques prolong and complicate the initial procedure and in some methods r…

Spontaneous rupture of tendons. A complication of hemodialyzed patients treated for renal failure.

Rupture of ten major tendons occurred in 7 out of a total of 50 renal patients treated by chronic hemodialysis, in two renal units. The rupture occurred spontaneously without any special strain. Rupture was complete in five tendons and imcomplete in the other five. In 2 cases, rupture occurred simultaneously in both quadriceps tendons. In 1 case a single quadriceps and a triceps brachii tendon were affected together. In 3 cases one quadriceps tendon was affected and in the last case, one triceps brachii tendon alone was ruptured. All the patients suffered from long-standing renal disease with …

Tendon grafting using Gracilis and semitendinosus tendons for extensive flexor tendon defects following synovial sarcoma excision: A case report.

Soft-tissue sarcomas of the hand are rare. Wide excision often causes functional impairment. Reconstructing extensive flexor tendon defects after tumor excision is challenging because suitable donor tendons with adequate length and diameter are limited. We report a 39-year-old female with synovial sarcoma on the volar wrist. Tumor excision resulted in multiple flexor digitorum profundus (FDP) tendon defects. We used gracilis and semitendinosus tendons from the ipsilateral lower limb for reconstruction. The gracilis tendon reconstructed the index finger FDP. The semitendinosus tendon reconstruc…

Transversal calcaneal anchored Achilles tendon reconstruction with free semitendinosus tendon autograft for acute rupture of Achilles tendon: clinical evaluation.

Achilles tendon reconstruction is an effective method of repairing Achilles tendon rupture defects. We introduce a new approach for Achilles tendon reconstruction using transversal calcaneal anchored autogenous semitendinosus tendon graft. The study aimed to evaluate the clinical role of this new Achilles tendon reconstruction. We retrospectively enrolled patients who underwent Achilles tendon reconstruction using transversal calcaneal anchored autogenous semitendinosus tendon graft for acute Achilles tendon rupture defects from 2016 to 2021. The clinical and radiological results were assessed

Endoscopic-assisted achilles tendon reconstruction with free hamstring tendon autograft for chronic rupture of achilles tendon: clinical and isokinetic evaluation.

PURPOSE: To evaluate the clinical and functional outcome of endoscopic-assisted reconstruction of chronic ruptures of the Achilles tendon using free hamstring tendon autograft. METHODS: We present a case series of 15 patients who had chronic ruptures of the Achilles tendon (>6 weeks earlier) and underwent endoscopic-assisted reconstruction with a free hamstring autograft. The graft loop was passed through and fixed to the proximal stump of the tendon. The graft was then passed through suture to the distal stump and finally inserted into a tunnel in the anterior calcaneus to the Achilles tendo

Achilles Tendon Salvage.

Operative repair of the Achilles tendon is associated with complications such as wound issues, infection, and rerupture, presenting significant challenges for orthopedic surgeons. With a range of salvage techniques available-but no clear consensus or algorithm on the optimal approach-treatment must be tailored to each patient and case. This article explores various salvage options, including tendon transfers, V-Y advancements, gastrocnemius turn-down procedures, and allograft techniques, illustrated through case examples to provide practical insights.

Orthobiologics for Tendon Injuries.

The treatment of tendinopathies can be challenging due to the wide heterogeneity in their clinical presentation, variable disease chronicity, and the lack of standardized treatment approaches. Orthobiologics such as platelet-rich plasma (PRP), bone marrow aspirate concentrate (BMAC), and patch augmentation have been utilized with increasing frequency for the treatment of tendon injuries, but the evidence to support their routine use is mixed. This review summarizes the current clinical literature evaluating the efficacy of PRP, BMAC, and patch augmentation in the treatment of common tendon dis

[Achilles tendon pathologies].

Achilles tendon pathologies are a frequent problem in the clinical practice. A distinction must be made between insertional and noninsertional tendinopathies. In addition to intrinsic risk factors, such as diabetes mellitus, there are also extrinsic risk factors, such as overuse and running. Sonography, X‑ray imaging and magnetic resonance imaging (MRI) are established imaging methods, depending on the question. The primary treatment includes a conservative approach. First-line treatment includes administration of nonsteroidal anti-inflammatory drugs (NSAID), physical rest and the use of assis

Peroneal tendon disorders.

Pathological abnormality of the peroneal tendons is an under-appreciated source of lateral hindfoot pain and dysfunction that can be difficult to distinguish from lateral ankle ligament injuries.Enclosed within the lateral compartment of the leg, the peroneal tendons are the primary evertors of the foot and function as lateral ankle stabilisers.Pathology of the tendons falls into three broad categories: tendinitis and tenosynovitis, tendon subluxation and dislocation, and tendon splits and tears. These can be associated with ankle instability, hindfoot deformity and anomalous anatomy such as a…

Tendon Transfers for Tetraplegia.

It is estimated that 65% to 75% of patients with cervical spinal cord injuries could benefit from upper extremity tendon transfer surgery. The goals of surgery are to restore elbow extension, as well as hand pinch, grasp, and release. Patients who have defined goals, actively participate in therapy, and understand expected outcomes, appear to have the highest satisfaction following tendon transfer procedures.
