BACKGROUND: Erysipelothrix rhusiopathiae is an established animal pathogen, which may cause infections in humans. It is a gram-positive rod and found in the tonsils or the digestive tracts of animals. The bacterium is occupationally related, as usually only people with frequent animal contacts are infected. We report a case of a patient who was admitted with an infectious tenosynovitis with bloodstream infection due to E. rhusiopathiae, and to our knowledge, this is the first report of a tenosynovitis with systemic manifestation associated with this bacterium.
CASE PRESENTATION: A 52-year old…

BACKGROUND AND OBJECTIVES: Although morning stiffness has long been recognized as a characteristic feature of rheumatoid arthritis (RA), it … of morning stiffness. We also determined the activity of intra-articular synovitis in 11 joints and tenosynovitis in 8 tendons/tendon compartments in the same hand by using power Doppler (PD) ultrasound with a semiquantitative score (0-3).
RESULTS: For intra-articular synovitis, swollen/tender joint counts more strongly correlated with total PD scores (ρ = 0.3 …

BACKGROUND: de Quervain's tenosynovitis is a painful condition of the wrist which leads to difficulties in performing activities of daily living.
AIMS: This systematic review was conducted to examine the effectiveness of steroid injection therapy as compared to splinting for treatment of de Quervain's tenosynovitis in adults.
METHODS: The following databases were searched for relevant studies, MEDLINE, EMBASE, CINAHL, AMED and PsycINFO (via NHS Evidence), and Cochrane Library (via Cochrane Collaboration). Synonyms and free texts were used to locate studies. The reference lists of articles we…

We present the case of a young woman in whom the association between sarcoidosis, extensor digitorum longus tendon tenosynovitis, and the presence of numerous rice bodies within the tendon sheath revealed a new clinical manifestation of sarcoidosis, in the absence of pulmonary or lymph nodes involvement. The most important aspect of this case is the assessment of tenosynovitis using multimodal ultrasound (grey scale, color/power Doppler, strain elastography, contrast enhanced ultrasound) which demonstrated specific findings that may facilitate further recognition of this clinical condition.

Tuberculous tenosynovitis is a rare form of extrapulmonary tuberculosis characterized by an indolent course and nonspecific clinical findings, frequently leading to delayed diagnosis. Paradoxical inflammatory reactions during or after anti-tuberculosis therapy may further complicate clinical management and mimic disease relapse. This is a case study regarding a 54-year-old female veterinarian presented with a four-year history of progressive swelling and induration of the right index finger extending to the palm, accompanied by gradually worsening limitation of finger flexion and hand grasp. M…

Gout attacks and tophi due to hyperuricemia are common, but rarely occur in extraarticular flexor tendons. Milky white fluid containing urate crystals, referred to as urate milk, may incidentally accumulate in the joint cavity and tophus of patients with gout. We report a rare case of gouty flexor tenosynovitis in the finger, characterized by urate milk, which closely resembled pyogenic tenosynovitis. A 54-year-old man with a history of gout attacks presented unprovoked right ring finger pain with marked erythema, swelling, and limited range of motion of the palmar side of the proximal interph…

Tenosynovitis with psammomatous calcification (TPC) is an extremely rare condition. It was first described as a characteristic subtype of idiopathic calcifying tenosynovitis, with only 40 cases reported to date. Here, we present a case of TPC affecting a female patient in her late teens, with no relevant medical history. She presented with discomfort and pain in the right first toe. A 10-mm mushroom-like calcified mass was observed in the metatarsophalangeal joint on radiographs. The surgical specimen revealed chronic synovitis with calcification. Numerous psammomatous bodies are observed in t…

BACKGROUND: The etiology of de Quervain's tenosynovitis (dQ) has been based on conflicting small case series and cohort studies lacking methodological rigor. A prospective case-control study was conducted to analyze the most common risk factors for dQ.
METHODS: Between January 2003 and May 2011, 189 patients surgically treated for dQ vs. 198 patients with wrist ganglia (WG) (controls) were identified in our clinic's electronic database. Sample characteristics, exertional, anatomical, and medical risk factors were compared between groups.
RESULTS: dQ vs. WG differed by average age (52 vs. 43 …

An immune reconstitution syndrome (IRS) occurs in between 10% and 25% of patients starting highly active antiretroviral treatment (HAART). A 49-year-old patient presents a tenosynovitis 6 weeks after HAART initiation. In our patient, exhaustive tests for infectious, inflammatory and drug-related causes of tenosynovitis were negative. The improvement obtained with high-dose Non-steroidal anti-inflammatory-drug (NSAID) therapy and the patient's immunovirologic profile, supported a diagnosis of tenosynovitis associated with immune reconstitution, a form of IRS that has not previously been describ…

BACKGROUND: Although cases of tenosynovitis associated with foreign bodies are documented in the literature, there is no reported instance of the treatment of foreign body-associated tenosynovitis with ultrasound (US)-guided steroid injection. The use of corticosteroid injection under US guidance for flexor tenosynovitis associated with foreign bodies is a noteworthy subject.
CASE REPORT: In this case report, we present the contribution of US-guided targeted steroid injection to the treatment of flexor tenosynovitis of the third finger of the left hand associated with a foreign body.

A 58-year-old woman was referred for right wrist pain, with suspicion of stress fracture of the radial styloid. 99m Tc-HDP bone scintigraphy, using new 3D-ring CZT system, was performed. Blood pool SPECT/CT revealed intense hyperemia along the first extensor compartment of the right wrist, suggesting De Quervain tenosynovitis, while late-phase SPECT/CT showed right thumb arthritis. Ultrasound imaging confirmed De Quervain tenosynovitis. Our case highlights the potential of blood pool SPECT/CT using new 3D-ring CZT system to detect relevant extraosseous findings, especially common tendinopathie…

Mycobacterium virginiense is a slow-growing mycobacterium first described in 2016 as a novel species within the Mycobacterium terrae complex. To date, 8 cases of M. virginiense tenosynovitis or osteomyelitis have been reported, though none have been relapsed cases. A previously healthy 70-year-old Japanese man presented to a local orthopedic clinic with pain, swelling, and stiffness in his right middle finger. Initially diagnosed with trigger finger, he received multiple steroid injections. He was ultimately diagnosed with M. virginiense tenosynovitis. Despite synovectomy and antimicrobial the…
