Home care and end-of-life hospital admissions: a retrospective interview study in English primary and secondary care.

BACKGROUND: Enabling death at home remains an important priority in end-of-life care policy. However, hospital continues to be a more prevalent place of death than home in the UK, with admissions at the end-of-life often negatively labelled. Admissions are frequently attributed to an unsuitable home environment, associated with inadequate family care provision and insufficient professional care delivery. AIM: To understand problems in professional and lay care provision that discourage death at home and lead to hospital admissions at the end of life. DESIGN AND SETTING: A qualitative study o

A Novel Objective Measure for Terminal Delirium: Activity Scores Measured by a Sheet-Type Sensor.

CONTEXT: Delirium is a significant concern in end-of-life care. Continuous monitoring of agitation levels using objective methods may have advantages over existing measurement scales. OBJECTIVES: To examine whether an objective measure of activity scores measured using a sheet-type non-wearable sensor (Nemuri SCAN [NSCAN]) was correlated with agitation levels measured using the modified Richmond Agitation-Sedation Scale (RASS) in terminally ill patients with cancer. METHODS: We conducted a single-center, prospective, observational study in a palliative care unit using the NSCAN to measure ac

Impact of Specialty and Nonspecialty Palliative Care on Quality of Dying With Alzheimer's Disease or Related Dementias: A Systematic Review and Meta-Analysis.

BACKGROUND: Older adults with Alzheimer's disease and related dementias can benefit from palliative care (PC). Whether specialty and nonspecialty PC have the same effect on outcomes is unclear. We examined the effects of these 2 interventions on comfort, symptom management, satisfaction with care, and potentially burdensome transitions, including hospital admission, emergency department visit, intensive care unit admission in the end-of-life, and in-hospital death. METHODS: This PRISMA-adherent systematic review involved a search of PubMed, Medline, EMBASE, Cochrane Library, ProQuest, and CIN

Transitioning From Curative to Palliative Care Goals in Pediatrics: Common Pitfalls and Clinical Pearls.

If recent medical advances have positively impacted children's lives, access to this new knowledge and technology raises ethical questions. Medical progress has also contributed to prolonging suffering in those afflicted with life-limiting conditions. Despite the growing tension between the need to discuss end-of-life issues, particularly advanced directives, and the widespread discomfort in engaging in such discussions, the question of death remains frequently evaded. The transition from curative to palliative goals is fraught with misunderstandings and moral distress. This transition of care

Updates in palliative care of patients with gynecologic malignancies in 2026: early integration, resource models, and emerging therapies.

PURPOSE OF REVIEW: This review summarizes recent evidence in palliative care for patients with gynecologic malignancies, focusing on early integration, models of care delivery, and updates in symptom management and palliative procedures. RECENT FINDINGS: Early palliative care involvement, particularly more than 3 months before death, is associated with less aggressive end-of-life care and better alignment with patient preferences. Resource-adapted models, such as stepped approaches and telehealth, expand access to specialist palliative care while preserving quality. Advances in symptom manage

Evaluation of an end-of-life teaching programme for unregistered domiciliary care staff.

BACKGROUND: With increased focus on people being supported to die at home, and increased numbers of people predicted to die in the coming years in the UK, it is recognised that domiciliary carers need to be trained and supported to give end-of-life care. Recent reports suggest that this is not happening. AIM: To introduce and evaluate a training programme to upskill unregulated domiciliary care agency staff and integrate them into the palliative care teams, supporting registered nurses in caring for end-of-life patients. METHOD: A training course was devised and implemented. This report cove

Post-Traumatic Stress Disorder and End-of-Life Care: A Well-Being Review.

Post-Traumatic Stress Disorder can be a debilitating comorbidity for a patient on end-of-life care. Combat veterans make up a vast majority of patients diagnosed with Post-Traumatic Stress Disorder and are therefore a vulnerable group requiring a comprehensive approach to their health care management. This paper addresses certain challenges the hospice and palliative care providers may encounter and offers solutions to ensure the patients maintain a high quality of life.

Serious Mental Illness and Hospice Care.

There are a small number of articles in the literature discussing palliative and end-of-life care in the SMI population. Most tackle the questions relating to competency to refuse care in end-stage anorexia or terminal medical conditions. This is a case review of a 55 year old patient with a complex psychiatric and medical history, who despite extensive treatment and long hospitalizations has failed to regain any ability to care for her basic needs. She has exhausted all available treatment options and her prognosis is extremely poor. The mental health community is resistant to discussing and/

Equitable inclusion of patients with cancer on the palliative care register: a systematic review in primary care.

BACKGROUND: GPs are instrumental to palliative care in the UK and most practices maintain a register of patients with palliative care needs. However, many people with incurable cancer who could benefit from palliative care are not included on this register, making this a potential source of end-of-life inequity. AIM: To identify barriers and facilitators to recognising the palliative care needs of patients with cancer in the UK and understand how these factors may have an impact on those vulnerable to end-of-life inequity. DESIGN AND SETTING: A mixed-methods systematic review. METHOD: Eight

Strategies for Family-Centered End-of-Life Care in the Adult Intensive Care Unit.

When patients transition from curative to comfort-focused treatment, family-centered care at the end of life can improve outcomes for intensive care unit (ICU) families. Nurses make important contributions across 3 key phases: admission and the hope for recovery, realization of poor prognosis, and the active dying phases. Barriers such as conflicts and organizational constraints can be addressed through various strategies, including advocating for early palliative care, encouraging regular family meetings, involving spiritual care, and providing bereavement support. Finally, nurses must also a

Managing lymphoedema and lymphoedema-related complications in end-of-life care.

Lymphoedema is a condition often seen in palliative patients, characterised by impaired lymphatic drainage, which leads to swelling in affected areas. In the context of end-of-life care, managing lymphoedema and its associated complications, such as lymphorrhoea and infections, is crucial for enhancing patient comfort and quality of life. This article discusses the various management strategies for lymphoedema in palliative care, including compression therapy, manual lymphatic drainage, skin care and gentle exercise. Attention is given to complications such as cellulitis and lymphorrhoea, emph

End-of-life care and advance care planning for outpatients with inoperable aortic aneurysms.

OBJECTIVE: A significant proportion of patients with abdominal and thoracic aortic aneurysms (AA) do not proceed to intervention after reaching treatment threshold diameter due to a combination of poor cardiovascular reserve, frailty, and aortic morphology. This patient cohort has a high mortality; however, until this study, there exist no studies on the end-of-life care conservatively managed patients receive. METHODS: This is a retrospective multicenter cohort study of 220 conservatively managed patients with AA referred to Leeds Vascular Institute (UK) and Maastricht University Medical Cen

Specialized Palliative Care and Intensity of End-of-Life Care Among Adolescents and Young Adults with Cancer: A Medical Chart Review.

Purpose: Adolescents and young adults (AYAs) with cancer often undergo aggressive end-of-life (EOL) care. We evaluated whether specialized palliative care (SPC) involvement is associated with the receipt of intensive EOL care among AYAs. Methods: This retrospective study included patients with cancer treated between the ages of 15 and 39 years at a university hospital, who died during 2009-2022. The primary outcome was high-intensity EOL (HI-EOL) care, which was defined as ≥1 session of intravenous chemotherapy <14 days from death or during the final 30 days of life, ≥1 hospitalization at an i

COMPLETE (Communication Plan Early Through End of Life): Development of a Research Program to Diminish Suffering for Children at End of Life.

While overall survival has improved significantly for children with cancer over the past 75 years, cancer remains the leading cause of death from disease among children and adolescents. Further, despite the many advances in medical and nursing care, children with cancer still experience significant physical and emotional suffering over the course of their illness, especially at the end of life (EOL). Children endure significant rates of high-intensity medical interventions (e.g., intubation, intensive care unit admission) at the EOL despite many parents, adolescents, and young adult patients i

Care of Geriatric Patients with Advanced Illnesses and End-of-Life Needs in the Emergency Department.

Older patients with advanced illness are presenting more frequently to emergency departments (EDs). These patients have complex needs, which challenge busy EDs tuned to provide emergent, life-sustaining interventions, and rapid dispositions. This article outlines communication skills to assess patient goals so that the ED provider can create a care plan that matches level of medical intervention with patient wishes. Furthermore, this article outlines symptom-based care for the actively dying geriatric patient in the ED, specifically, acute pain, dyspnea, terminal delirium, secretions, dry mout

[Palliative Care at Home by Anesthesiologists].

Governmental policies recommend the palliative care at home for cancer patients. However, there are some barriers to discharge cancer patients from the hospital who want to receive end-of-life care at their homes. Anesthesiologists whose main job is to perform general anesthesia in the operating theater usually have little contact with general practitioners giving community palliative care. So it is important to communicate each other to make opportunities to discuss an organized system and care plan for these patients, and to improve information transfer.
