Video-Assisted Thoracoscopic Surgical Rib Fixation for Costochondral Separation Injury.

Costochondral separation is a rare phenomenon following blunt thoracic trauma that can also be associated with secondary injuries. We present a case with complete costochondral separation of the right second rib with concomitant mediastinal compression. Definitive treatment was provided through video-assisted thoracoscopic surgical plate osteosynthesis.

A Simple Novel Method to Elevate the Sternum in Thoracoscopic Thymectomy.

The right-chest thoracoscopic surgery approach has become the preferred operation for the treatment of thymic disease. Due to the limited space in the anterior mediastinum, achieving en bloc resection of the tumor is a common difficulty. Here, we report a simple method to lift the sternum by single hook. This method has a lot of advantages, including providing a wide view of the anterior mediastinum for clear vision and being an easy operation.

[Spontaneous hemopneumothorax treated with video-assisted thoracoscopic surgery; report of two cases].

Case 1: A 41-year-old man developed right chest pain. Computed tomography (CT) revealed right tension hemopneumothorax. Chest tube was placed in the pleral cavity immediately. Because of continuous bleeding and inadequate lung expansion, video-assisted thoracic surgery (VATS) was performed on day 11 afer admission. Thoracoscopic view clearly showed bleeding point at the thoracic apex. Hemostasis was performed and peel was removed for expansion of lung. Case 2: A 38-year-old man developed right chest pain and dyspnea. CT revealed right hemopneumothorax. Because of massive continuous bleeding, e

Optimization of sympathectomy to treat palmar hyperhidrosis: the systematic review and meta-analysis of studies published during the past decade.

OBJECTIVE: The systemic review and meta-analysis of the studies published during the past 10 years was designed to optimize the surgical procedures of video-assisted thoracoscopic sympathectomy (VTS) to treat palmar hyperhidrosis (PH). METHODS: Citations from 2000 to 2010 were included regarding the following aspects: selection of ganglia level for VTS, comparison of different techniques for VTS, evaluating clinical efficacy of intraoperative intrapleural analgesia, and postoperative intrapleural drainage. Major clinical outcomes are defined as: cases with postoperative resolution of symptoms

[Standard Approach and Education of Video-assisted Thoracoscopic Surgery for Anterior Mediastinal Tumors].

There are various approaches to surgery for anterior mediastinal tumors, including median sternotomy, multi-port and single-port video-assisted thoracic surgery, and robot-assisted thoracic surgery. According to the 2017 Annual Report of The Japanese Association for Thoracic Surgery, mediastinal tumor resection is about one-tenth of lung resection. Therefore, we consider that it is necessary to standardize the technique at each institution to acquire stable minimally invasive surgical techniques. We reported on our center's techniques and innovations in minimally invasive surgery for anterior

Case Report: Thoracoscopic secondary cytoreductive surgery for high-grade serous ovarian cancer.

The objective of this video is to show the feasibility of secondary cytoreductive surgery for recurrent high-grade serous ovarian cancer. The procedure targets recurrences in the cardiophrenic lymph nodes and the right diaphragm involving the right basal pleura. Using video-assisted thoracoscopy, the thoracic recurrences were dissected and removed. Minimally invasive thoracic secondary cytoreductive surgery proved to be safe and feasible and facilitated rapid post-operative recovery, ensuring timely access to subsequent second-line adjuvant chemotherapy.

Malignant pleural mesothelioma mimics thoracic empyema: A case report.

BACKGROUND: Thoracic empyema and malignant pleural mesothelioma (MPM) are distinct medical conditions with similar symptoms, including cough, chest pain, and breathing difficulty. We present a rare MPM case mimicking thoracic empyema. Physicians must consider MPM risks for patients exposed to building material who exhibit lobulated pleural effusions, indicating thoracic empyema. CASE SUMMARY: A 68-year-old retired male construction worker suffered from shortness of breath and chest tightness over 10 d, particularly during physical activity. A poor appetite and 4 kg weight loss over the past 3

Atelectasis secondary to two separate video-assisted thoracoscopic wedge resection of the lung lobe and pleurodesis under general anesthesia in the same patient: a case report.

BACKGROUND: Intraoperative atelectasis is a common pulmonary complication during thoracic surgery, yet recurrent mucus-mediated atelectasis during lung re-expansion in the same patient undergoing sequential two video-assisted thoracoscopic wedge resections of the lung lobe and pleurodesis successively is extremely rare. CASE DESCRIPTION: This report describes a 15-year-old previously healthy adolescent who developed recurrent intraoperative atelectasis secondary to extensive viscous mucus plug obstruction during two successive video-assisted thoracoscopic wedge resections and pleurodesis for

Impact of Regional Anesthesia on Subjective Quality of Recovery in Patients Undergoing Thoracic Surgery: A Systematic Review and Meta-Analysis.

OBJECTIVES: Regional anesthesia can be effective for managing pain after thoracic surgery. This study evaluated whether it can also improve patient-reported quality of recovery (QoR) after such surgery. DESIGN: Meta-analysis of randomized controlled trials. SETTING: Postoperative care. INTERVENTION: Perioperative regional anesthesia. PATIENTS: Adults undergoing thoracic surgery. MEASUREMENTS AND MAIN RESULTS: The primary outcome was total QoR scores 24 hours after surgery. Secondary outcomes were postoperative opioid consumption, pain scores, pulmonary function, respiratory complications,

Bilateral thoracic duct ligation for persisting postoperative chylothorax.

Herein, we report the case of a patient with persistent postoperative chylothorax despite right supradiaphragmal ligation of the thoracic duct. Computed tomography lymphangiography after lipiodol injection demonstrated a correctly ligated right thoracic duct but an anatomical variation with patent left-sided thoracic duct, which was successfully ligated afterwards by video-assisted thoracic surgery.

[A case report of lymphomatoid granulomatosis (LYG) with spontaneous attenuation].

70 years old woman was admitted to our hospital for the evaluation of multiple nodular shadows on chest X-ray. She had no clinical symptoms despite of mild increase of LDH and decrease of PaO2 on laboratory examination. Video associated thoracic surgery was performed to obtain lung tissue for the pathological diagnosis. Pathological findings suggested a diagnosis of lymphomatoid granulomatosis (LYG) for her lung nodules, which was confirmed by rearrangement of IgJH with nested PCR of lung tissue. Nodular shadows in her lung had a trend to regress in accordance with decreases of makers for lymp

[Videothoracospy in thoracic trauma and penetrating injuries].

Videothoracoscopy represents a valid and useful approach in some patients with blunt chest trauma or penetrating thoracic injury. This technique has been validated for the treatment of clotted hemothorax or posttraumatic empyema, traumatic chylothorax, traumatic pneumothorax, in patients with hemodynamic stability. Moreover, it is probably the most reliable technique for the diagnosis of diaphragmatic injury. It is also useful for the extraction of intrathoracic projectiles and foreign bodies. This technique might be useful in hemodynamically stable patients with continued bleeding or for the
